Continuity mistake: Steve and Danny are searching a suspect's house. Danny picks up a digital camera. He is going through some photos and the LCD display shows 53 photos on the memory card. The counter shows from 53/54 to 50/54. Then he shows Steve other photos and the counter shows only 4 photos. 1/4, 2/4 and 3/4.
Continuity mistake: When McGarrett is behind the curtain, preparing for the charity fight. McGarrett and the injured fighter, Victor Boriero, share a joke about the dislocated shoulder. Boriero stands behind Lori. McGarrett says "Besides, I'm the one who dislocated your shoulder." Lori giggles, turns to share the joke with Boriero. Boriero says "Yeah, I didn't forget." Lori turns back to face McGarrett (camera shot from behind), and in the next camera shot (camera shot from in front) she turns back to face McGarrett again.
Visible crew/equipment: When the gunman tries to flee in the ambulance Jerry rams the truck into the vehicle to prevent his escape, and when Captain Grover pulls the gunman out of the ambulance, we can see the boom pole/mic's reflection moving on the windshield.

Deliberate mistake: The police car Steve & Danny are driving on the bridge as they approach the freighter has 467 on the roof, which then becomes flipped & backwards a few shots later as they take a sharp right, only to become 467 again (as Danny has conversation with Rachel).

Factual error: The aircraft carrier towards the beginning is captioned as being the USS Enterprise (CVN-65), where Lt Rollins is stationed. It is obviously not the 'Big E', but a much smaller escort carrier or amphibious assault vessel. Notable differences are not only the sheer size difference, but also lacks the angled flight deck with the protruding catapult brakes, the second port side aircraft elevator and both the superstructure and bow shape are wrong.
Continuity mistake: Danny is taking to some Victoria's Secret models on the beach. They ask to see photos of his daughter. He is talking to them and he pulls out his cell phone from his pants pocket. Next shot, you see him reach for his pocket again and once again he pulls out his phone.
Continuity mistake: When the lab tech is talking about the plane part, she places a Surface Dial on the screen. Throughout the entire scene the Dial keeps moving around between instant cuts, sometimes more towards the right edge, sometimes more towards the center. In one shot it's right at the bottom edge, then in the very next shot it's further up the screen. And shortly afterwards, when the lab tech says "they're too precise", it's off the screen entirely and lying on the desk.

Character mistake: In episode 6-6 Steve explains to Danny that a 'klick' means a kilometre, 0.62 of mile, to which Danny replies: "Oh, thank you, G.I. Jerk, for the pedantic, uh I know what a klick is." In this episode, when Steve again uses the term 'klick', Danny replies: "You know, no-one can understand you when you say stupid things like klicks.", which prompts Kono and Chin to give the exact same explanation as Steve did in season 6.
Factual error: Five-0 brings up a map of all nuclear power plants in Western Europe that is completely inaccurate - most of the trefoil markers in Great Britain are placed nowhere near nuclear installations, and has 3 in Ireland (which doesn't have any nuclear reactors).

Factual error: In episode 17, Danny's Camaro is subjected to a number of bursts from a mini-gun, riddling it from stem to stern. In episode 18, Danny's daughter mentions that he had just had it repaired. There is no way an insurance company would not total that vehicle, since it would require replacing or using "Bondo" on virtually every body panel, not to mention mechanical repairs.
Continuity mistake: When Steve and Danny go to Dr. Yang's office and find evidence of a struggle, they follow a blood trail out the back door. Danny starts to put gloves on as he stoops down to pick up a wallet on the ground. He only has one glove on before the camera angle switches to the view from behind him, then suddenly he's wearing gloves on both hands as he reaches for the wallet.
Continuity mistake: A Texas Ranger spreads peanut butter on a suspect in a sugar cane field. The suspect is tied up on the ground and mostly on lying his back and right side. He spreads the peanut butter on his left arm, head and chest. In a later scene, the suspect is taking a shower and you see lots of peanut butter on his back, both his arms and the back of his head.
Continuity mistake: At the end of the episode, the surfers are in the bay to spread Ian's ashes. Ben lets go of Kono's hand and is handed the urn with the ashes by the man to his right. He removes the lid of the urn and hands only the lid back. Yet in the overhead shot immediately following, the urn is back on the board of the man to Ben's right, and there is nothing on Ben's board. He's holding Kono's hand again. In the next shot, he is tipping the ashes from the urn.
Continuity mistake: In scene where Chin starts reviewing footage of white van attacking, in first shot a wheel from the NSA guy's van has rolled off and comes to rest immediately in front of attacking van's driver side front wheel. In next shot as driver exits, loose wheel is missing.
Other mistake: An important key to the first season's plot is the "dancing men" code on the postcard. We see the postcard several times leading up to Ep 1:13. It shows 3 lines of code containing 4, 4, then 3 characters. In this episode, however, Steve brings out his workings on a yellow legal pad to show the others. There are more characters on his pad than are on the postcard, and some of the characters (for example those with both arms on one side of their bodies) do not even appear in his workings. He claims to have used frequency analysis to work out the code. If one uses frequency analysis on the actual code we see on the postcard, the result can never be Hiro Noshimuri.
Continuity mistake: When McGarrett chases the bad guys' Jeep, his Camaro skids through several huge dust clouds. Immediately after each cloud, however, the car is perfectly clean again. This is visible in several of the very rapid shots throughout the chase. After the chase ends, a coating of dirt and mud (which, incidentally, differs from the earlier shots) appears on the Camaro.

Continuity mistake: In the scene where McGarrett's dad's house is seen for the first time from ocean-side, deck on the top floor has furniture (wicker two-seat bench and two chairs under double windows with long table to the right) and the lower level has no furniture in grass to left of the ramp, and two large vases on either side of the ramp. Then two shots later (same scene) as Steve approaches under the crime scene tape, there is no furniture on the upper deck but now furniture to left of ramp and vases are missing.
