Continuity mistake: When Sherlock goes to look out the window at John leaving (after the tiff about the head in the fridge), his dressing gown is hanging off his right shoulder, exposing his t-shirt. Next there is a wider shot of him from behind, and his dressing gown is on straight, covering both shoulders properly. Then there's another closeup and it's off the right shoulder again. Continuity mistake: At Buckingham Palace, Watson has just sat down next to Sherlock. There is a red pillow between then on the couch. It moves, sometimes sitting straight and sometimes tilted, between shots throughout the scene. (00:14:45)

Continuity mistake: Near the end, when Mycroft is talking to Watson, he folds his hands and places his head on them. However, in the next shot, his hands are a couple inches away from his face.

Question: Which episode contains the line "I'm not insulting you, I'm describing you?" Google searches turn up nothing but the quote itself, and no further information on the episode or the rest of the scene. If anyone knows the line that led up to it, that would be even better.
