Plot hole: When Bo Jenson visits Richard Allen, he takes the elevator at the far south end of the ER wing, near the ER nurses' station, straight up to the 6th floor and then turns left to get to Richard's room in the short hallway, by the windows. When Richard jumps out the window and lands on the ledge, we see he's on the south side of Rampart's main building. Considering the actual floor plan of the hospital, this is completely impossible. The ER is located in the south lower wing and only has two floors, so Jensen could not have gone up to the 6th floor from the elevator he took.

Revealing mistake: When the critical boy who ingested the ant poison is being wheeled into the ER's treatment 3, we can see Johnny laughing, and obviously since absolutely nothing about the situation is funny, we know it's Randolph Mantooth just cutting up during the take.

Promotion - S3-E7

Other mistake: When John and Roy get back to the station, they head to the kitchen and John takes the hot pot of stew out of the oven with potholders, and when he quickly tries to lift the lid without the potholder, his pain makes it clear that it's burning hot, so he uses the mitt to lift the lid. There's a long metal serving spoon that was left in the stew, and John just lifts it up and starts serving the stew with no problem, even though that spoon should have been too hot to even touch.

Continuity mistake: After Roy's had his arm bandaged in the ER, Brackett points to the treatment room Johnny's in and says he's in 4, and Roy heads there. Then after speaking to Johnny, when Roy opens the door to leave Johnny's treatment room, we can see the treatment room facing Johnny's, and it's treatment 2, which means that Johnny is actually in treatment 1, not 4. Plus the fact that the boy with the heart problem is supposed to be in treatment 1 at that time.

Character mistake: When Brackett and Early are dealing with three squads at the base station at the same time, Squad 51 has two victims - father and son, Squad 18 has a pulmonary edema, and Squad 36 has only one bicycle rider with severe head injuries, because the second rider died, so the total is four out of five being dealt with. Back at the station, after Johnny asked Dixie about the condition of all the victims, he tells Roy that the father and son are both responding, so is Squad 18's pulmonary edema, and for Squad 36's bike accident, that "both" are coming out of the coma, which totals five victims. Then Roy responds, "Four saves out of five victims. That's pretty good." Johnny should not have said, "both" since it was just one rider.

Character mistake: Days after Drew's death (Roy and Johnny had two days off after Drew's death), during the meeting that Brackett has with the four paramedic units, one of 59's guys asks, "What do we do when we have this overload situation on the same frequency? Visible crew/equipment: When Squad 51 is dispatched to 'person caught in a mailbox', have a look at the squad's license plates when it rolls up on scene. The plates read "70324H" which is the California commercial plate number for Squad 51, not the usual prop dept's license plate "E999007." It was the prop dept's responsibility to change from the commercial plate to the prop plate, but this scene slipped by them. It's also interesting to notice that this scene takes place at a real intersection up the block from Universal Studios, which is the direction Squad 51 came from. Note the "Black Tower/MCA Tower" in the background, which houses Universal's executive offices.

The problem is that just as the branch nears the ground, we can see the production crew's thin double lines securing the branch at the top right of the screen, and then those lines vanish when Roy joins John.

Other mistake: When Tommy's on the ledge, and Roy and John are on the aerial ladder, just as Tommy jumps off the ledge and Roy catches him, we can see the young actor happy and grinning as he's being caught by Kevin Tighe, but within the very serious storyline of a 7 year old boy jumping from a ledge, looking "empty, tired and giving up", this doesn't fit. Obviously the young actor thought it was great fun to do a stunt like that.

Revealing mistake: When Roy brings the man with a laryngeal obstruction into the ER and is taken quickly to treatment 1, his head is tilted back and Early places a laryngoscope into the patient's mouth and says that he sees the obstruction. The problem is that we can see that the laryngoscope is actually at the side of the patient's face. Then, when Early uses the forceps to remove the obstruction, once again we can see that it does not go into the patient's mouth.

Revealing mistake: After Gage gets bitten by the rattler, when the driver of Tractor 2 begins ascending the cliff, there is a nice close-up as he passes and this young stuntie is most certainly not the older driver seen in this scene. Not to mention the watchless driver suddenly acquires a black watch and a plastic face shield attached to the helmet.

Other mistake: On site with the boy on the ledge, when Truck 85 raises the aerial ladder Roy stands by and waits to go up with Johnny behind him. Both Roy and Johnny are the only ones not wearing their turnouts, and both of their watches are on their left wrists, but when 85's aerial apparatus operator says "Desoto, up the aerial," the bare arm we see beside him is wearing the watch on his right wrist. This is not a flipped shot.

Continuity mistake: At the start, there's a long shot of the Land Rover driving down a long stretch of road towards the camera and then passing it. That road, landscape, and mountains are very distinctive. As Roy, John, and Chet continue talking and driving, when they reach the scene of a traffic accident, Chet sets up a flare behind the Land Rover and another flare behind the pickup truck, then Chet sees a car coming from the direction which the Land Rover was headed. The problem is that the road/landscape/mountains shown earlier in the long shot of the Land Rover, which had been driving from the opposite direction, is the same that's shown here, as Chet tries to stop the car. Then to top it off, when Chet first notices the second car in the distance, it's coming from the opposite direction of the first car, but once again the view is exactly the same.

But when the attendants exit, they carry the stretcher out of the back of the first ambulance then wheel it to the second ambulance, and the footrest is now red. It would seem that they merely transferred the patient from the first ambulance's stretcher to the second ambulance's stretcher while still in the ambulance, but they couldn't have done that because two full size stretchers cannot fit into the back of that first station-wagon style ambulance, especially with attendants back there, so they would have had to do the patient transfer on the ground outside. Hence the problem.
