Character mistake: When Beast is showing Gambit and Rogue the news report about Cannonball, it is shown to be featured in a magazine called Philologist's Quarterly. When Beast leaves the room, he remarks "Ah, there's my 'Philologist's Quarterly'." as if he didn't already see it.
Other mistake: When Wolverine is carving "HERO" into Cocteau's tombstone, he is shown to be using all three claws on his right hand to do so, but the carving looks like it was only carved with one sharp point. If he was using all three claws, each point of articulation for each letter should have been tripled.
Continuity mistake: When Red Skull is leaving Wolverine and Captain America alone, he sets Captain America's shield on a table and Wolverine is shown to be hanging from stocks by his wrists with Captain America on his left and the table on his right at roughly a 45 degree angle. In a wide shot after Red Skull has left the room, the shield has vanished from the table and Wolverine is now several feet from where he originally was, with Captain America roughly at his 12 o'clock and the table roughly at his 3 o'clock.
Other mistake: The Red Skull reveals that Cocteau is the one who sounded the alarm on Wolverine and Captain America, but at the time the alarm starts going off, there are no switches on the walls for him to have pressed, not to mention he was shown to have been standing in the middle of the corridor out of arm's reach of either wall.

Other mistake: The non-English wording on Cocteau's tombstone at the beginning of the episode is gibberish. It says "Pace et Rfouisat." When it is shown again at the end of the episode, it reads "Page et Resquiat." It appears that they were attempting to say "Rest in Peace," but they got their French and Latin confused. The proper French translation would be "Repose en Paix" and the proper Latin translation would be "Requiescat in Pace."
Continuity mistake: When Arkon states that Storm is the conqueror of his heart, he is holding up a goblet with his left hand and extending his right hand towards her. Not only is the bracelet on his right hand gold when it should be silver (the bracelet on his left hand remains silver in this shot), but his right hand looks rather effeminate. Arkon and Storm were sitting right next to each other and no one was shown to be standing between them in any other shot, so the right hand in question couldn't have belonged to another person.
Factual error: When Rogue tricks the Friends of Humanity into firing on their own helicopters, two men are shown to be immediately parachuting from the helicopter at an altitude of at most, 100 feet. This wouldn't really be possible because jumping from such a low altitude would require the person to immediately deploy the parachute, which in this case would cause the parachute to be torn to shreds by the helicopter's rotor. If a person jumped from that altitude and deployed the parachute after free falling enough to clear the rotor, they would have fallen too far for the parachute to have been effective in allowing them to land safely, if the parachute even opened before they hit the ground.
Continuity mistake: When Rogue is talking to Wolverine through her watch communicator, in the closeup shots the watch is shown to be around her glove, on her wrist. In the wide shot, the watch is either around her sleeve just beneath her glove or it has disappeared entirely.
Continuity mistake: When Sarah resuscitates Cyclops, they are by the edge of the swimming pool. When the shot changes to Cyclops blasting through the screen door, the pool is nowhere to be seen, nor is the puddle of water that was under Cyclops in the previous shot.
Plot hole: When Killgrave's bodyguard dumps Cyclops face down in the pool and leaves him for dead, it is night time. When Sarah is shown to have successfully resuscitated Cyclops, it is broad daylight. If Cyclops had been floating face down in the pool for several hours then he would surely be dead. No amount of CPR would have brought him back.
Continuity mistake: The layout of the living room in Killgrave's house varies depending on the shot. Sometimes the half-circle couch is next to a staircase and sometimes it is not. The piano is also in a different spot on the floor each time it is shown. There are also chairs that appear and disappear throughout the scene.
Other mistake: When Killgrave is standing in front of the projector, the projector image is cast onto his person matched color for color, except for when his own image in his true purple-skinned form appears and the image cast onto him maintains a normal flesh color.
Continuity mistake: When Killgrave is walking up the porch steps to reclaim Rusty, Cyclops and Sarah are standing near the edge of the porch and Killgrave nearly makes it to the top before the shot changes. When the shot changes to Sarah telling him about Rusty's trouble adjusting, Killgrave is shown to be walking a considerable distance to get to Sarah despite the fact that the he was two or three steps away from her in the previous shot. Killgrave is also shown to be walking on level ground, which means either the porch has become a lot longer off-screen or that the animators forgot he was ascending the steps.
Continuity mistake: When the X-mansion is shown at the end of the episode, the landscaping around it is different than what was shown early in the episode. When the mansion was first shown, there is a shrub in the shape of a circle on the lawn across from the mansion, but at the end of the episode, the circle-shaped shrub has turned into some bushes. The bushes and trees around the mansion are also different looking and at the far side of the mansion the shrubs have grown taller. Additionally, when Wolverine is walking away from the mansion at the end, a hedge-wall has appeared next to the mansion. It's actually common for the landscaping outside the mansion to vary drastically from episode to episode. This is the first time I noticed it changing within the same episode.

Continuity mistake: When the group is standing together in a circle at the end, Xavier is sort of in the center of the circle and Rogue and Storm are several feet to his right. When the shot changes, Rogue and Storm are suddenly right next to Xavier.
Continuity mistake: When Xavier and the others are approaching Cyclops, Storm, Rogue and Wolverine, Magneto is shown wearing his helmet and Gambit is shown wearing his trenchcoat. When it changes to an overhead shot of the group, Magneto is no longer wearing his helmet and Gambit is no longer wearing his trenchcoat and his normally-black headgear is colored blue.
Other mistake: When Forge says "Now we need offensive power," he opens up a containment bubble with no one inside it. The bubbles are transparent, so he would have known whether or not someone was inside it and it would make no sense for him to open up an empty bubble since he was in need of reinforcements.
Continuity mistake: When Xavier is first shown in his containment bubble, he is shown with his hands resting at the center of his hover-chair with his left hand on top of the right. When the shot changes, his hands are suddenly separated and at the sides of his chair.
Continuity mistake: When the group is talking to Cameron Hodge's Phalanx facsimile, sometimes Forge is behind Beast on his left, Mr. Sinister is behind Forge on his left, and Warlock is behind Forge and Mr. Sinister. Other times Warlock is behind Beast on his left and Forge and Mr. Sinister are not in the shot.

Continuity mistake: The Phalanx-civilian that is dressed like a biker that attacked Beast and the others outside the restaurant in Milford Township, PA at the end of the previous episode is suddenly outside the Empire State Building at the very beginning of this episode.
Continuity mistake: When Beast is trying to escape the Phalanx in Wolverine's jeep, he has to drive down a considerably long road just to reach the gates to the mansion. In all previous episodes, the road leading to the mansion is barely longer than a driveway.

Continuity mistake: When Sabretooth is terrorizing the police and civilians, it is broad daylight outside. When Beast and Jubilee are shown approaching the scene in the Blackbird just before dispersing tranquilizing gas, it is twilight.

Revealing mistake: The shot of police cars surrounding Sabretooth and him throwing one of them into the air is a recycled shot of the very first shot in the series from the season 1 episode "Night of the Sentinels, Part 1." It's even revealed a few minutes later in the episode that this isn't actually Sabretooth, but a Phalanx facsimile that has taken his form, which makes the fact that Sabretooth's actions are being recycled all the more dubious.
