Continuity mistake: After Lincoln and LJ have been recognized by the waitress, they speed off down a road. During their conversation in the car, whenever we see LJ from (his) right side, his hair is blowing in a strong draft, and is blown away from the temple. In all other shots, his hair is completely calm, and is hanging down and forward, covering his right temple completely.
Other mistake: When Michael climbs up the rope to get to the room below the infirmary, the rope is hanging from the manhole cover, but he still manages to lift the cover whilst hanging from it. In the next shot where he looks down the manhole, its clear that there is nothing other than the rope he could have held on to while lifting the cover. An impressive accomplishment, defying the laws of gravity.
Plot hole: The Kansas police arrest Bellick for, and he is subsequently convicted of, Geary's murder based in part on a threatening voice mail he left on Geary's phone. But the message's time and date would match the records of Bellick's admittance into the hospital, thus proving him innocent of the crime.
Revealing mistake: Near the end, when Michael is unscrewing the sink before kicking the wall in, watch his hands in the foreground while he is talking to Sucre. Just when Sucre says "But what you were doing up there in the infirmary." Michael starts using his fingers to take the bolt out. You can see that there is no bolt, just him turning air, with a mystery 'unscrewing' sound effect.
Continuity mistake: When Steadman is watching the agent put the bags that contain the remains of Veronica Donovan into the trunk of the silver car, the license plate changes between shots. The first time we see the car, the plate reads "1029Q", the second time (close-up shot as the agent closes the trunk) it reads "9417X".
Plot hole: Gretchen, Lincoln, and Sophia were able to visit Scofield and Whistler in Sona without Gretchen being recognized. She was taken in for questioning in episode 9, but she managed to escape and kill General Zavala and one other guard. The other guards knew that she was the one who killed Zavala but she was still able to sign in for visitation.
Continuity mistake: Season 2, episode 6. While the guys are digging in the garage, Michael tells Sucre about the website they can use, as C-Note walks in. He closes the door behind him, but misses the catch, and you can see the door start to swing open again behind him as he walks further into the room. Cut to Lincoln for a second, then to a wide shot, and the door is now firmly closed.
Revealing mistake: In the scene where Sarah crashes into the agent's buggy, you see they have been replaced with dummies. When the car first hits, you see one of the dummies fall out, but they are both seated in the next shot. Also you see that it is not Sarah driving but a man.

Plot hole: In this episode, Kellerman insists that there is no way of identifying Stedman. The brothers ask about DNA, dental records, etc. Kellerman states that they were erased. However, Stedman's DNA can always be tested and compared to that of the presidents. DNA will show that they are siblings. There is no "erasing" that.
Factual error: Linc arrives at the 'Bolshoi Booze' location at 17:45. It is an 8 hour drive from Trinidad Colorado to that location, which is right on the Texas - New Mexico border. That means that they would have had to have left at 09:45 that morning. But they don't appear to have stayed a night at Linc's father's place - and in none of the other plot lines has it been night. They couldn't have covered that ground in the time span of the plot.

Revealing mistake: In the episode where Scofield, Sucre and T-Bag are driving in Panama, the trees in the background have no leaves - like Autumn/Winter in the US. And even though there of course are trees in Panama which shed their leaves, there is no time of year when all trees are leafless - like in the temperate zones. That shot was definitely not filmed in Panama.

Continuity mistake: Season 2, Episode 10. When Michael and Sara are driving towards Mahone down Butterfield Road, both cars are seen in one shot and both cars are on a gravel road. In the next clip, seen from Mahone's end of the road, his car is on asphalt.

Continuity mistake: When Michael is sitting in the park he's staring at the front page of the newspaper. As the camera follows a man in the park you can see that Michael has opened to the midpages, but in the next shot Michael is looking at the front page again, before opening it another time.
Continuity mistake: All throughout the car chase, the car Michael and Lincoln are in has a license plate on the front bumper. But after Bellick has forced their car off the road, the car makes a jump before hitting a tree, and while it is in mid-air you can see that the license plate has disappeared.
Continuity mistake: During the car chase in the beginning of the episode, Bellick's car swerves to avoid hitting a truck as they go over a bridge. The next shot is from in front of the cars, and in the background you can see the bridge and a stretch of road behind it, but the truck has vanished.
Continuity mistake: Series 2 - Episode 10. When Sara is to meet Michael at 1681 Butterfield Road Sara looks in the mailbox. She closes the lid, yet the mailbox is seen standing with the lid a bit open. In the next couple of shots the mailbox is completely open, and when they jump in the car and leave, the lid is now closed.

Continuity mistake: In the scene where LJ comes to visit Lincoln and admits to breaking the coffee table, LJ's hand moves between shots. When Lincoln is shown, you can see LJ sitting in front of him with his chin resting on his hand. When the shot changes to close-ups of LJ, his hand is down and completely out of the shot.
Continuity mistake: When Lincoln and Michael are talking on the little pier, Michael talks about Westmoreland and his relationship to the truth. Lincoln has his head turned towards Michael through this, but as Michael says "There'd be no reason for him to lie about it", the angle changes and Lincoln is suddenly staring straight ahead, with no time to turn his head between shots.
Continuity mistake: Before being sent to psyche ward, Michael had brutally hurt his right hand by repeatedly punching it at the cement wall of the cell. But moments later,seen in psyche ward after being cleaned up, there's no sign of any hurt on that hand, even no gauze covering at all. We can see his normal good hand.
Continuity mistake: When agent Mahone is looking at the photocopied newspaper articles, at first he is holding a single sheet of paper, and you can read the headline in reverse through the paper: "Life Sentence for Mob Boss Abruzzi". We then get a close-up of the articles from his point of view, and he is now holding two sheets of paper, where the topmost reads "Governor's Daughter Wins Humanitarian award". Mahone places this paper on his desk, and you can see that he is once again only holding the one "Life Sentence." paper he had to begin with.
Continuity mistake: Bellick's partner, Geary, is wearing a light grey T-shirt, with a very prominent sweat stain on the front. Throughout the episode, this stain constantly changes shape and size, even within two shots of the same scene, and sometimes it disappears altogether. This is best seen when him and Bellick are herding the prisoners to their car. In the close-ups of him, his shirt is completely dry, in long shots the sweat stain is there.
Continuity mistake: In Episode 14, "The Rat", when Lincoln and Dr. Tancredi are in his cell together, he says "How it gonna feel when they tighten the belts". He then turns his head so that he is looking the doctor in the eyes, but when the angle changes, he is staring directly in front of himself, and his head is not turned at all.
Continuity mistake: Episode 14 - John Doe. Bellick is in the prison, and is being threatened by 'Banks.' As Banks and his crew walk away, in two different shots, you can see an elderly gentlemen (looking like Charles Westmoreland) walk across the screen, left to right. He does this twice, without there being time for him to run around.
Revealing mistake: After Pope enters Scofield's cell to see the escape hole, we see him standing outside the cell looking out at everyone throwing toilet tissue etc. If you watch closely right after we see a guy pulling his pillows toward him to tear up, it cuts to another inmate who is throwing stuff out as well. Only thing is, we see the cell door slides to the side slightly which would indicate that the cell doors where not locked. In the event of an escape, lockdown would happen immediately, with all cell doors locked with the prisoners inside their cells.
Continuity mistake: When Michael is supposed to be in Pope's office holding the support for the Taj Mahal so it didn't come unglued, Abruzzi placed the makeshift key in the lock of the door, allowing Michael to escape to get back with the other inmates. When Michael opens the door and it's unlocked, the key is in the deadbolt. As he shuts the door behind him, the key is in the lock of the door knob.
Continuity mistake: When Steadman takes out his gun, he presses it to the back of Veronica's head. She does not flinch or shy away from the gun, but in the next shot, a close-up of Steadman, the gun muzzle is pointed at the camera with Veronica off-screen, and so the gun is suddenly not touching her.
Plot hole: When Scofield comes back after visiting his wife, Fernando announces his arrival. However the problem is, everyone is inside digging the hole. As Fernando walks in along with Scofield, it leaves no one outside watching for guards. The guys are at least smart enough to keep the watch outside instead of all staying inside. If a guard shows up unnoticed, they are screwed.
Factual error: During the last several minutes when the Governor and warden are discussing the current situation and the status of the governor's daughter, a cut away shot is shown stating that it is the Capitol Building Springfield, Illinois. The Capitol building shown is in fact the Capitol of Connecticut, in Hartford CT.

Factual error: When they have escaped, and the dogs are barking at them as they hide in the bushes, this couldn't have happened the way the show presented it. The spot that they escaped is on the main road (Collins Street, also, it was to the left of the screen from where Joliet Jake came out in 'The Blue's Brothers) The dogs are barking at them as they hide in the bushes, and that location is on the -other- side of the Prison. There is NO way that they would be able to circle around the prison and get from one side to the other considering in the front, there are fences surrounding the main building, obstructing you from going that way. The back side of the Prison (There it a road next to a junk yard located right next to the Prison, the same road that is used to get to the 'Gate' the cons are looking at when the dogs are barking. That road would have been full of cops, since all the cops are in the parking lot. It would have been more believable if they would have went across the street to where the women's facility was, and were shown on camera looking across to the parking lot out front.
Factual error: They have made it a point that Scofield is incarcerated in a prison in Illinois, and even gets married so he can be eligible for conjugal visits during his stay so he can pass information or items to his "wife" or she to him. In Illinois, conjugal visits aren't allowed. Only something like six states allow them, and this isn't one of them. Totally a plot hole, and since this thing was so carefully planned out, I would think he would run across this information ahead of time.

Continuity mistake: In Season 2: Episode 9-"Unearthed" when Bellick is talking to T-Bag in the house, he pulls up a chair to sit down in front of T-Bag. When he pulls the chair out and starts to sit down, the chair is backwards, but in the next shot it's facing T-Bag.
Revealing mistake: When 5 of the escapees are heading to Oswego to get the supplies and Michael tells Lincoln to turn right, you can see it isn't a real car as the steering wheel doesn't turn - Lincoln just moves his hand along the wheel. Watch the grooves and you will see.
Audio problem: When Gretchen kills the guy in the prison he lets out a very obvious fake scream similar to a Wilhelm scream. Also, he is being choked around the neck, how is he able to make the scream? Take note of when you see this scene in the next episode's review. The guy doesn't make the scream in that version.
Continuity mistake: When Lincoln Borrows is secretly talking to his son in prison using a cell phone, he is seen wearing an inner blue plaid shirt for most of the scenes before. As the conversion starts and focus switches between the two, the inner shirt is visible and not visible. It disappears and re-appears three times during the conversation.
Factual error: When Mahone is at MSP International he gets the call about C-Note being in the diner in Benson, MN and says to "fax Fanklin's mugshot to every hospital in southeastern Minnesota." Benson is in West-Central Minnesota, almost 3 hours from Minneapolis, definitely not southeastern MN.

Continuity mistake: When the kid returns from the store and opens the trunk, Michael and T-bag both grab a shovel, Michael grabs the green and T-bag grabs the silver handled shovel. The next shot they have switched shovels and keep the switched shovels the rest of the scene.
Continuity mistake: When Mahone is in Scofield's apartment the show flashes back to an overhead view of Scofield planning on the wall while Mahone is tracing over the holes in the wall from where Scofield was tacking papers up. The mistake is in the flooring, the laminate flooring is a much lighter flooring than when Scofield was there two months prior.
Factual error: When Veronica has a conversation with Stedman, he tells her that he can't escape and that the glass is 2 inches thick. However, if the glass in the door was 2 inches thick, the wood door would have to be 5-6 inches thick to even support such thick glass. The door would be extremely heavy. And it obviously went unnoticed by Veronica as she entered the home with no effort.
Continuity mistake: At the car dealer where Don Self speaks to the seller, the seller says "How did you find me?" and they are inside the car dealership. Don responds with "I've never lost you". Suddenly when Don answers they are outside the building, like they teleported.
Factual error: Early in the episode, a flyover context shot is made. It is labeled "Salt Lake City Federal Building." In reality, the image is of the Will Rogers Coliseum in Fort Worth, Texas. In the background, you can see the community arts center and the University of North Texas Health Sciences Center.
Plot hole: They exchange Scylla for the life of the general's daughter. If they counted on him valuing his daughter over Scylla, which he did, they wouldn't have had to steal the cards or break into the vault in the first place. Just kidnap the daughter like they did, tell the general to gather the cardholders and hand over the cards, give them Scylla and let them out.
