Continuity mistake: While Bishop and Torres are locked in the two jail cells, Bishop knocks out the glass from her barred window, but when Torres breaks the old toilet and says, "I really think we're stuck in here," Bishop's cell window glass is perfectly intact - we can see the reflection of all the bars. Continuity mistake: When Gibbs and McGee are alone in the kitchen, in the closeups of McGee when he raises his left hand (such as when he takes the watch from Gibbs) he is not wearing a ring, but in the consecutive shots of the duo McGee is wearing a ring on his left hand (also odd, since it looks like McGee's wedding band). Continuity mistake: During Delilah and Tim's dinner, when Delilah places corn on the cob onto Tim's plate note the positions of the broccoli and shrimp. In the following shots the broccoli and shrimp repeatedly switch to opposite positions. (00:13:45)
Continuity mistake: During their first day's investigation of the double homicide Tony is wearing a grey suit, white shirt, and burgundy tie. On the second day, while Tony is wearing a dark suit, light blue shirt and dark blue tie, when Gibbs tells Tony to go see what Ducky wants in Autopsy, Tony is back to wearing what he wore on their first day. Then at Tobett's house, Tony is again wearing the light blue shirt and dark blue tie. (00:04:30 - 00:31:00)
Continuity mistake: When Tony is sitting at McGee's desk talking to Delilah, Tony's holding the phone to his left ear with his left hand, but in the overhead shot Tony's holding the phone to his right ear with his right hand. (00:15:50)

Continuity mistake: After Senator Kiley asks Gibbs to keep his name out of it until after the vote, it cuts to Ziva holding Carrie McLellan's Academy graduation photo (gold frame looks like an 8x10). In the closeup Ziva places the framed photo back on the shelf, but that photo vanishes from the shelf in following shots. When Gibbs walks in, it seems that photo is on the table beside the couch. Continuity mistake: After Ducky finds the needle in the MRI machine it cuts to Gibbs stepping on the bubble wrap, and as Abby happily rolls toward Gibbs, in the two shots facing her the headband magnifiers are flipped down over Abby's eyes, but in the shot facing Gibbs the magnifiers are flipped up over Abby's head. Continuity mistake: When Ziva and Tony head out to bring in Lieutenant Napleton, in the first shot Tony grabs his jacket then holds it in his left hand, as he reaches down with his right hand to grab his backpack. Next shot Tony is now holding his jacket in his right hand, as he's reaching for his backpack with his left hand. (00:15:45)
Continuity mistake: Director Shepard has long nails with a French manicure, but when McGee gives her the envelope from Gibbs, in the closeup Jenny's nails are short and unpolished while she holds the old photo of herself. (00:43:10)

Continuity mistake: While Caitlin and Tony are at the hospital in Jeremy Davison's room, when Michelle walks in there are two closeups of Jeremy handcuffed to the bed rail. In the first closeup the handcuff is around his wrist with the double bar facing up, but in the second closeup as Caitlin uncuffs him it's the opposite way, with the single bar facing up. (00:27:55)
Continuity mistake: When Abby walks into the lab carrying the box with the Beretta Cougar she is barehanded, and two shots later she's wearing her latex gloves as she reaches for the bullet, but next shot she's barehanded again as she picks up the bullet to show Gibbs. Then, when Abby thanks Gibbs for getting her out of the sensitivity training, the latex gloves suddenly reappear, but they disappear in the next shot. (00:12:10)
Continuity mistake: When Gibbs asks Abby to estimate how much time Wilkerson has in the car trunk, just as Abby says, "Depends on the age of the car," this shot is flipped, note Abby's neck tattoo switches sides. (00:10:25)
Continuity mistake: While Gibbs and McGee are searching the dock area for Ziva and Tony they find the dead terrorist, and in all shots of the deceased his arms are raised, but in the one overhead long shot the arms are down by his sides. (00:23:25)
Continuity mistake: When Gibbs and McGee go to Captain Vetter's home, Gibbs holds up his ID with his left hand in the shot facing Vetter, but the ID is held up in his right hand in the next shot facing Gibbs. (00:17:45)
Continuity mistake: When Nasser is brought to the interrogation room to meet with Agent Cassidy, at first the handcuff is around Nasser's left wrist with its single metal bar facing out, but when Cassidy says, "Now please," Nasser's handcuff is on the opposite way, with its double bar facing out. Continuity mistake: During season 4 when Ziva gets worried about Tony being 'sick', she is researching y. Pestis, and telling Gibbs how worried she is about Tony having tests and the long-term effects of plague. In season 5, first episode, when Ducky comes to tell the group that the body is not Tony because "he didn't have plague", Ziva looks confused and says "before my time?"
Answer: I hadn't seen the backdoor pilots yet but while it wasn't explicitly stated, at least onscreen, I heard somewhere Vivian got either fired from NCIS due to jeopardizing a case, got resigned from the agency, or transferred to the FBI.
