Continuity mistake: As House describes the lady's symptoms, in the front shot Chase has a small book/pamphlet in his hand. The camera changes to a side shot in the middle of House's description and suddenly the book is on the table and no longer in Chase's hand. This happens right in the middle of House's line and Chase doesn't make any quick movements to account for this discrepancy.
Continuity mistake: As Cuddy is making the deal with House, how much time off from Clinic duty for a week off his vicodin, while the camera is behind Cuddy you can see there is bright sunshine on House's head. The shot switches to a camera behind House and now his head is shaded.
Continuity mistake: House is about to inject Ezra Powell (Joel Grey) with what is supposedly a lethal dose of morphine. In the shots where they pan out to show House addressing the room at large, the syringe is very full, almost completely. When they zoom in on House holding the syringe, it is only filled a fraction of that much - maybe a quarter to a third. This doesn't happen just once, but every time they switch between shots.
Continuity mistake: During the scene where Dr. Chase and Dr. Cameron are out having a drink, Cameron picks up Dr. Foreman's cell phone. Before she does she takes a drink of her soda. This drink drops the level of drink to half full. When Foreman returns a few seconds later she suddenly has a glass that is 3/4 full. There was no time for anyone to give her a refill.
Continuity mistake: When the young swimmer is about to dive, you get a close-up of both her heels going over the edge. Then, it shows a further shot with only one heel over the edge. Later in the same scene, the girl is underwater and you see her hair flowing around her face. When she lifts her head out of the water, her hair is tied up.
Continuity mistake: In the beginning of the episode, when Cuddy presents the case to House, she has her cell phone in her left hand when handing him the file. She doesn't have it in her hand when she slides the file closer to him. When the camera faces her, you can see her arms are bent up while talking to him, and see the phone. When the camera is facing House, Cuddy's arms are hanging down by her sides, no phone.
Continuity mistake: Becca, the pregnant woman, says that Cuddy must be mad at her for having been a drug addict and possibly causing her baby's lungs to be underdeveloped. While she's talking, her right hand is rubbing her belly. In the close up, the hem of her hospital gown just barely reaches the middle of her tummy. In the immediate next shot, it is much further to the right side.
Continuity mistake: Throughout both parts of season 4's finale ("House's Head" and "Wilson's Heart"), Amber is always shown getting hit from the back and sitting facing House, so her seat's back is to the windows. Yet the last time we see the accident, that is, right after she took the flu pills, she is definitely sitting neatly sideways to the window (i.e. the seat is perpendicular to it) and getting hit from the right side. It most likely was done for artistic reasons, as the back shot, shown multiple times, makes for a very neat effect, with her blonde hair flowing around her like sunrays.
Continuity mistake: At the start of the episode, when 13's date falls off the bed, she knocks the bedside lamp onto the floor. As 13 runs to the bedroom, the lamp is laid on its side on the bedside cabinet. As 13 bends down to attend to her date, the lamp is back on the floor.
Continuity mistake: Right at the end, when House is apologizing to Wilson, watch the bright spot on the TV frame on the wall behind House. In the long shots, the spot is right at the top left corner of the TV. In the close-up shots, the spot is closer to the middle. This is visible every time the shot changes from long to close-up. Light spots do not move depending on the position if the viewer. The only way a light spot changes is if the light source changes position or the reflecting surface changes position (try reflecting a flashlight beam off a mirror and then, leaving the flashlight and the mirror alone, move around. The reflected spot of light will not change position).
Continuity mistake: The runaway has an attack when her mother shows up and coughs blood on the table. There is an emesis basin at the far end of the table she throws up on. The shot changes twice and now there is FAR more blood on the table and the basin has moved to the middle of the table. Before the first shot change, Taub is reaching for the basin, however, after the change he is moving away without enough time to move the basin. The shot changes again and the patient is now in the bed with Adams holding the basin under her mouth. There is not enough time in that shot change for her to move from the foot of the bed to in the bed.
Continuity mistake: House is in the cafeteria eating chips with Adams discussing a case. Park walks up and gives Adams a cup of coffee. The cup of coffee disappears and reappears during the whole scene. The coffee is in front of Adams, then isn't. Towards the end the coffee is far to the side rather than where it originally was set.

Continuity mistake: When Dr. Cameron is walking with Dr. Charles to go for a procedure, he barely has any facial hair before they enter the stairwell, but once they reach the stairwell his facial hair has increased quite a bit. His hair is also parted to the left prior to entering the stairwell and once in the stairwell it is parted on both sides.
Continuity mistake: A third of the way into the show when Hadley and House are arguing while walking out of House's office, Hadley's necklace watch is backwards and out of the top of her blouse. When they stop walking and switched to a shot of House then back to Hadley, her necklace watch is right-side front and inside the top of her blouse.
Continuity mistake: During the scene in which House is offering up liquor-flavored ice cream he sets the containers in a row down the table 1-2-3-4-5. He then puts spoons in the three containers closet to Foreman, 1-2-3. The shot changes and before anyone touches the spoons they are now in containers 1, 2, and 4.
