Stupidity: Why does Kirk let the men on the planet's surface nearly freeze to death? They've got shuttlecraft they could use to go down and rescue them.

The Enemy Within - S1-E5

Stupidity: When Sulu and the others were freezing on the planet's surface, a transporter malfunction interfered with their rescue. Kirk, Spock, etc., did not consider using the Enterprise's shuttle which could have flown to the planet's surface and rescued Sulu and the away crew. (00:30:00 - 00:35:00)

Mistakes

When Kirk and Sulu enter the records room, they pick the lock. Later when they beam the officer back down, he enters the room without unlocking the door. The room should be locked since they beamed him down in the "past" erasing their having been on Earth and in the records room.

Gene Roddenberry created the transporter as an easier (and cheaper) way of getting Enterprise crew members onto a planet's surface, rather than landing the ship on the planet.
