A Hole in the Heart - S4-E22

Continuity mistake: When Doug and Carol are talking in the Pedes ICU, from one angle his head is against the back of the rocking chair, but from the other angle, his head is leaning forward. This continues for the scene, from shot to shot.

A Hole in the Heart - S4-E22

Continuity mistake: Near the beginning of the show, Doug is examining baby Josh and puts his stethoscope in his ears. Kerry says, "Your actions are totally irresponsible", and the shot goes to Doug. There is no sign of the stethoscope anywhere. The next shot, from behind him, shows it's around his neck. He then takes it off and the next shot should show it in his hands - but it's disappeared again.

A Hole in the Heart - S4-E22

Continuity mistake: When Anna and Max are talking in the lounge, a piece of her hair is hanging over her ear when the shot is from behind her shoulder, but from the front angle, it's tucked smoothly behind her ear.

Robert Romano: They're going to be looking for a sacrificial lamb, and I have to tell you, right now you're looking pretty wooly.
