Continuity mistake: Daniel's interaction is inconsistent across the episode. Initially, Daniel emerges through the Stargate with no ripples in the event horizon or audible noise. Then he walks down the metal ramp making noise with each footfall. Later in the episode he returns through the Stargate with an audible noise and ripples in the event horizon. He remained "out of phase" for the entire segment of the episode.
Continuity mistake: When O'Neill and Carter arrive at Washington, Carter says "Don't worry, Sir. These were my old stomping grounds." O'Neill replies with "I'm sorry to hear that." But later when the reporter approaches O'Neill in the bar the recording says "My condolences."
Continuity mistake: In the episode "The Shroud," during the scene where Jack is talking to Daniel aboard the Odyssey, there is only 1 star on each of his shirt collars, indicating that he is a Brigadier General, instead of the proper two stars for a Major General, as he does in every other scene of the episode.
Continuity mistake: In the scene where SG-1 tries to get their note through the gate, watch Daniel. The camera cuts to him just before Jack slides down the zip line, and you can see small dots on Daniel's face - presumably the marks left when shot by the security system. However, Daniel has been taking cover since the alarms sounded and only gets hit when he tries to get to the gate after Jack is brought down. The next time Daniel appears on camera, his face is clear again and stays that way till the point in the scene where he, too, is killed by the security system.
Continuity mistake: When the Jacks are talking to each other right at the end, the camera goes back and forth from looking over each of their shoulders at the other one. If you look closely, the false Jack's earlobe structure changes when we see him from behind.

Continuity mistake: When Teal'c walks into the Goa'uld force shield on the planet, he is forced off and he almost does a backflip. You can see that he would have hit the floor head/neck first. But when the shot changes, you see him land on the floor sliding on his back, as though someone had just pushing him over.

Continuity mistake: Jacob Carter repeatedly mentions blending with Selmak 4 years ago, placing it in Series 4 (each season constitutes a year - when Carter leaves the SGC to join Atlantis at the conclusion of season 10, she talks to Teal'c about how they have spent 10 years together). However the blending occurred in Series 2 (6 years ago).
Continuity mistake: When Daniel returns to the SGU in the alternate reality and Cathrine shows him the map of where the Goa'uld attacks on Earth are, we can see that there are no attacks in the north of Europe, but when we see a closeup Oslo, Stockholm and Helsinki have suddenly been added.
Continuity mistake: During the surgery, when they attempt to remove the Goa'uld, they start a timer. The timer starts at 00:00:00, then it blacks out (obviously, you don't want to be bored by major neck-surgery), and then the timer is at 01:19:49. However, seconds later (after Carter and Daniel's conversation), look at the timer in the background, it reads 00:23:46.

Continuity mistake: Kawalsky's rank changes. At the beginning of the episode, he wears the rank insignia of a Captain. However, when his is in dress blues for mission briefing he has on Major insignia. Then in the meeting after the Abydos, he's back to wearing Captain bars.
Continuity mistake: In the episode there is a stopwatch on multiple screens, counting up. When Sam and Jack come back to base and are talking to General Hammond in the briefing room the screen shows 58:00:07, then in the next scene in Sam's lab her screen shows 54:19:07.

Continuity mistake: When Col. Mitchell is talking to Col. Carter in the control room, the initial shot shows Carter appearing on a white computer display. The scene then cuts to a close up of the conversation and the display is black. It is also much thinner than the white display.
Continuity mistake: As the female soldier approaches the gate when its first activated, an alien device is thrown through. You can see in the shot that the device will land much further away from her foot than what is shown when the shot changes.
