Continuity mistake: The lettering on Smithers desk name plate changes between shots. While he is taking the call from Mr Burns mother you can see his name "W.Smithers" in black letters. The shot after seeing Burns who keeps falling asleep the letters are now red.
Continuity mistake: In the 'Nightmare on Evergreen Terrace' segment, in Bart's dream you see a shadow of what looks like someone with four knife-fingers. Then when we see who it is, it is Willie holding a rake. But the rake has five tines, not four, so the shadow is wrong.
Continuity mistake: When Maggie (sitting on Marge) appears on "Tyrano-Vision," Homer waves his hand in front of her. However, we can see that he isn't leaning over too far, and in the next shot it is clear that both Bart and Lisa are sitting between Homer and Marge, so there is no way he could have reached over to her in the first place.

Continuity mistake: Just before Marge tries to sell Skinner the tie rack, she is writing on something. She then puts her hands down by her waist, and then brings one up to turn it on and both up to pull the motor out. The point is that the pencil and the item she had in her hands have simply vanished, and we never saw her put them down either.

Continuity mistake: When Professor Frink is explaining where Homer is, in "Homer3", Ned Flanders is leaning behind the couch between Bart and Marge, and Chief Wiggum is standing up. When Homer complains, Ned is between Marge and Chief Wiggum, who is now sitting down on the couch.
Continuity mistake: After Homer dodges the cannonball near the end of the episode, the blue platform that he stood on goes further away from the edge of the stage between shots, and when Homer hugs Marge the platform has disappeared and Peter Frampton's cooler has become closer to Homer.
Continuity mistake: At Asa's funeral Burns and Grandpa both produce a key to unlock the compartment in the Hellfish monument, but even though they both take their keys out of the keyholes after turning them, all 9 keys are in place in the following shots.
Continuity mistake: When Homer is stuck in the hole after saving the plant, Marge is seen in the background wearing lipstick. When Mr. Burns asks if there is anything he can do for them, there is a closeup of Marge and her lipstick is gone. When they cut back to Homer, Marge is again wearing lipstick.
Revealing mistake: When Lisa is explaining to the curator of the museum why she didn't tell the crowd of what she learned about Jebediah Springfield, if you look in the background none of the people on the far sidewalk are moving, almost frozen in place with some of them with their arms in the air yet everyone and everything in the parade keeps moving right along.

Factual error: In "Nightmare On Evergreen Terrace", Martin has a dream, in which he speaks Latin. At one point, he says a verb that doesn't exist: "morire". The words is "mori" (to die), "moritur" (noun dies) or "moreris" (you die). The "moirie" is also on the blackboard behind Martin.
Continuity mistake: When Lisa first discovers Jebadiah's confession, she is holding the fyffe and wearing his racoon hat, the screen fades to black (I'm guessing for the ad break) but when it comes back Lisa is standing in exactly the same position, still reading the confession, still holding the fyffe, but no longer wearing the hat.
Continuity mistake: When Apu awakes, he has a closed note pad, and the table looks small. In the next high-angle shot, Apu's book is open and face down, with paper on top of it and a pen beside it. The table is also bigger, with Bart sitting there with his breakfast things too.
Continuity mistake: In the early scene where the Simpsons are gathered around the table to eat, you can see that they are eating slices of pie. However, on some of the long shots, the food on their plates changes and looks more like mashed potato and vegetables. Also, in the same scene, in one shot of Lisa, you can see Bart's hand over her plate, but when the angle changes, it is behind her plate.
Continuity mistake: As Bart is forced to show Marge the item (picture frame) he is hiding, he pulls it out of his jacket and he hands it to Marge. As he pulls it out you can see there is nothing in the frame or on the border of the picture frame. After Marge's reaction to the photo we see a close up of the frame and there is now a picture in the frame and some colorful decorations around the frame. Whilst the frame could have been the backside when he pulled it out he never turned it over when handing it to her, and Marge never turned it around when looking at it.
Continuity mistake: During the 'Nightmare on Evergreen Terrace' segment, when Bart is running from Willie, he plans to sink Willie in the sandbox. In the first view of the sandbox it is isolated on a field, yet when Willie is sinking in the sand there is a jungle-gym beside the sandbox.
Continuity mistake: In class, when Lisa is sitting at her desk, pondering the Jebadiah mystery, her tray of cookies and glass of milk is very visible. Then when Lisa says "The missing piece of the puzzle. I've got it." and runs out of class, the cookies and milk are no longer there.
Factual error: When Chief Wiggum opens the revolver's chamber, there's 5 rounds with one open slot. In the next shot, we see the reverse end of the chamber and it's drawn in such a way to indicate those 5 rounds all have bullets in them still, meaning the empty slot is from when Maggie shot Mr. Burns. But revolvers don't eject spent shells automatically and no one handled the gun after it dropped on the floor. So the empty casing should have still been in there when Wiggum opens it up for the first time.
Plot hole: The director of Radioactive Man says to Bart "we found our new Fall Out Boy. That's what I would say if you weren't an inch too short. NEXT!" But Milhouse is the new Fall Out Boy, and he's blatantly smaller than Bart. Bart was standing right next to Milhouse, so the director could see the difference.

Continuity mistake: In "Attack of the 50 Foot Eyesores", when Homer answers the door and finds the Lard Lad statue in front of him you can see there is a door knocker on the door and a green doormat on the step. But when he answers the door a second time, the door knocker and doormat have gone.
Continuity mistake: In "Attack of the 50 Foot Eyesores", the executive goes to the piano and you can see a penholder on his desk. But the camera angle changes to behind the piano in the next shot, the penholder has moved position and there are now more items on the desk.
Continuity mistake: In "Nightmare on Evergreen Terrace", when the teacher hands out the papers before the test, you can see that in the desk sitting next to Bart is a student wearing green sunglasses and has blue hair. In the next shot, Martin is suddenly beside Bart.
Continuity mistake: After Moe yells at Ralph, there's a woman in the background staring at Moe with an angry look on her face. She turns her head away from him after some seconds, but in the very next shot, she is looking at Moe with the same expression on her face.
Continuity mistake: As Chief Wiggum is being interviewed about the fugitive, his arm is shown to be in a cast due to the police van crashing at the Krusty Burger Drive-Thru earlier on. Yet when they get to the hospital, his arm is completely fine.

Continuity mistake: When Homer says he's going to the cinema, we see his drinking bird working in front of the computer. It makes a key press on the lower right of the keyboard, however on the following close up shot of the bird pressing the Y key it is a standard qwerty keyboard layout.

Continuity mistake: After the boy's car gets crushed, Bart sells Milhouse's glasses for $600. Then Milhouse knocks over the Coke bottles and the clerk gets the money back and keeps the glasses. The next scene, where they try to get on a train that goes to Springfield, you can see Milhouse in his glasses and throughout the rest of the episode.
