Continuity mistake: Emily Sloane's left ring finger was shipped to Arvin Sloane in one episode in Season 2, when they are reunited in the Philippines, the left ring finger is bandaged and missing. Later, in the episode when she is shot, when she tells Arvin the CIA is listening, the left ring finger is there.
Other mistake: For secret agents, Sydney and Dixon are not very dedicated to secret identities. At several points they refer to each other by their real names in front of other people while they are under aliases. Examples include The Confession party and The Coup Las Vegas scenes.
Continuity mistake: When Sydney is tortured, she loses some teeth and a stream of blood flows out the right side of her mouth. When she returns to SD-6 and speaks with the director, the blood is gone (fair enough). Outside on the street in the following shot, the blood has returned, but there's no reason to believe that she is still bleeding.
Factual error: At the start, just after Sydney has the phone call regarding Will, she goes to meet her dad and says that they can save Will, if they get the Rambaldi document from the CIA and the ampule from SD6. In fact, it is the other way round; the CIA have the ampule, and SD6 have the page.
Continuity mistake: Season premiere, 'Authorized Personnel Only', part 1: When Sydney and the other agent are in the bathroom at the club, Sydney begins to smear black dirt on the agent's tee shirt for his punk transformation. The first smears that she applied differ when he exits the bathroom.

Revealing mistake: In episode 3x10, Remnants, when Lazare shows up to talk to Will, we see that his left hand has been cut off. When Lazare is about to leave, he has his hands at his sides, and we can see that both arms are the same length, but the left one should be shorter, because it has no hand.
Continuity mistake: When Sloane is talking to Sydney towards the end, he slides some documents towards her. After that shot, when the shot's of Syd we can see that Sloane's holding his hands near his face, but in front shots of Sloane they're down by the desk.
Continuity mistake: When Roger Moore is showing Sloane the bank accounts, he says "5 deposits in excess of $45 million...", but the shot just before that shows deposits of (in thousands), $9,362, $7,215, $9,010, $8,300, and $9,250. That makes $43,137,000, which isn't in excess of $45 million.
Plot hole: Vaughan discovers Lauren is a mole when he goes through her briefcase and discovers the wig, gun and fake passport she used in Germany a few hours before - surely even the worst secret agent in the world would have enough sense to get rid of such incriminating evidence as soon as possible, and not carry it back to the USA (sneaking it onto a plane, presumably) and leaving it in the room where the man you're lying to sleeps. Lauren is a highly skilled agent, if the Covenant selected her for a mission this important, so it's out of character (without an explanation) for her to be so sloppy.
Continuity mistake: In the parking garage, after Sydney knocks the machine pistol out of the hit man's hands, it lands with the barrel pointing away from the two of them and remains in that position during their entire fight. When Sydney grabs it off the ground, the barrel is pointing toward them.
Plot hole: In 'Phase One' Sydney tells Will during the day to "Get Francie and get out of town" because she realises there's someone after her. Later that night Francie is shot at her restaurant and replaced by Allison (the clone), but Will was supposed to have taken her away already. There's no question of him not finding her - the restaurant would be the first place he'd look for her.
Factual error: Francie alters Will's cell phone so she can download items from the CIA's computers via it. In order to start the link, she calls him on the cell phone, asks him to look up a recipe on his computer, and then this cell phone is able to download a 3 gigabyte file. The phone Will uses has a maximum data transfer rate of 14.4 kbs; wouldn't Will get suspicious after he's forced to surf for this recipe for 57 hours while that gigantic file downloads?
Continuity mistake: In 'The Abduction', when Sydney shuts down the traffic lights Sark's car is in the middle of the gridlock. When Sydney is fleeing with the stolen briefcase seconds later Sark pulls up beside her to save her, but how did he get out of the huge gridlock?
Continuity mistake: At the start when Sydney is being held and talking to Khasinau, there is blood coming from the corner of her mouth. At this point it reaches the bottom of her chin, but moments later when she is talking to her mother, the blood is noticeably shorter.
Factual error: When Sydney goes to Uzbekistan, it isn't Uzbekistan. The restaurant where the meeting occurs is not anything made by Uzbeks, and it is too remote to be made by foreigners. The land around is also very lush, unlike anything in the deserts of Uzbekistan. The people are speaking Arabic, instead of Uzbek and Russian, and the sign on the wall is also Arabic. The guards are also wearing what appears to be a Russian uniform, even if the badge is correct.
Factual error: When Sloane is talking to Emily on the telephone, Sloane's phone emits a call waiting beep and the camera focuses on the phone's screen. There are two factual errors here: 1) The phone icon for "Line 1" near the top right hand corner of the phone display is in the "on-hook" position indicating that there is no call in place. 2) The call information is displayed incorrectly. It is obvious that this is a Cisco XML card with the text entered.
Continuity mistake: In this episode Nadia Santos tries to find out who's the baby that her mother Irina Derevko is holding on a picture. She first shows it to Jack Bristow and later to Arvin Sloane. Later Jack asks to see to picture again and tells Nadia it's Irina's niece. At the several moments the picture is visible the picture is flipped, showing the baby on the left arm or right arm of Irina.
Continuity mistake: When the new head of SD-6 invites the security agent to play back what Sloane wrote, the screen is already displaying the letter. We only see the entirety of this letter later, once the "playback" has been completed. It can't be that the boss has done it once already, as he's surprised to see the final text.

Character mistake: In season 4 episode 13, "Tuesday," when Marshall is meeting with Ulrich, he is both able to understand and reply in German. In season 5 episode 15, "No Hard Feelings," Marshall almost misses the opportunity to change the street light from red to green, because the text is all in German. Jack has to tell Marshall which word is green, indicating that Marshall does not understand the German language.
