Other mistake: Just after the assistant warden's right arm has been chopped off by an inmate, part of his shirt sleeve goes missing. The part of his arm still attached to his body has a sleeve starting at his shoulder, and stopping an inch above where his arm stump ends, leaving an inch of skin exposed. And when the chopped off arm is shown, the arm is shown to have a sleeve starting at his elbow, going down to his wrist, but there are a few inches of his upper arm above his elbow left exposed. This means that a couple inches of his sleeve from where part of his upper arm was located, have gone missing without explanation. (01:20:25)

Continuity mistake: When the armoured guards appear, a guard holding a mace attempts to swing it at Ricky whilst he jumps from a staircase. In the shot of Ricky jumping down from the staircase, he is holding his shield so that it is positioned away from him, but in the next shot, the guard suddenly has his protective shield directly in front of him. Revealing mistake: Just after Ricky has killed the bald inmate, he then goes to punch the assistant warden in his face, resulting in his one real eye falling out. In the close-up shot of the assistant warden getting punched, a fake prop dummy can be seen instead of the real assistant warden, most likely used so that it would be easier to create the practical effect of his eye falling out. Continuity mistake: When the assistant warden falls down the stairs whilst being confronted by the inmates, in one shot his body is rotated towards the left and is face to face with some inmates nearest to the staircase. When the camera changes angles, his body is more rotated to the right and is now directly facing the staircase bannister to be face to face with more of the inmates. (01:20:16)

Revealing mistake: When the assistant warden and the bald inmate walk into Ricky's cell after not seeing him inside, Ricky reveals himself, grabs onto the bald inmate and punches part of his head off his body, killing him. In the shot where Ricky punches the inmate in the side of the head, we can see that the actor has been replaced with a dummy. (01:19:17)

Continuity mistake: When the assistant warden starts searching Ricky's cell, he opens the two doors to the toilet stall to see if he is in there. Whilst Ricky jumps down from the ceiling, the assistant warden has the left door on the toilet stall held open and the right door on the toilet stall closed. When he notices Ricky in the next shot, the left door on the toilet stall is now closed and the right door on the toilet stall is now wide open. (01:19:09)

Continuity mistake: When the assistant warden and the bald inmate are walking down the corridor towards the end, two inmates from inside their cells put their hands on the bars of the cell doors to see what is happening. The inmate standing on the left has both of his hands on the bars and the inmate standing on the right has both hands on the row of bars positioned below where the inmate standing on the left has his hands. When the camera changes angles, the inmate standing on the left no longer has his hands on the bars and the inmate standing on the right now has his hands positioned on two separate rows of bars. Revealing mistake: When a couple of the inmates are watching the assistant warden drag Fa out of his cell by sticking his hook hand into his mouth, it is apparent that the man being dragged by the hook hand is a fake motionless prop dummy and not an actual person. (01:18:36)

Continuity mistake: When Ricky is being tortured by the warden, after the warden removes the tape covering his mouth, Ricky spits out some blood as many of the razors inside his mouth. The razors that were shown sticking out of his cheeks are not shown moving, but after it cuts to the warden with some of the razors stuck to his face, the shot cuts back to Ricky and the razors in his cheeks are no longer there. Even the ones that weren't shown being spat out. (01:16:40)

Continuity mistake: Wong Chun stuffs a load of razors in Ricky's mouth and begins hitting him. After the warden asks Ricky a question and Wong Chun stops hitting him for a moment, we can see that Ricky's left cheek looks fine (aside from the blood running down his head). In the next shot, the razors are suddenly sticking out of his cheeks that were not there in the previous shot. (01:16:27)

Other mistake: When Ricky's girlfriend lands on the ground after jumping off the roof, she is shown landing on her front, with her face and body shown hitting the ground. When Ricky is shown looking at her body at the morgue, her face has virtually no signs of injury what so ever. (01:11:29)

Continuity mistake: When Ricky's girlfriend jumps off the roof, her body is positioned so that her legs are facing towards the building, but when she falls to the ground in the next shot, her body is suddenly facing the opposite direction, as we can see her legs are facing away from the building. Revealing mistake: When Ricky's girlfriend commits suicide by jumping off the roof of a drug dealer's apartment building, the body that is shown falling to the ground is a dummy and not a human body. (01:11:28)

Continuity mistake: When Ricky punches the drug lord in the side of the head, whilst the drug lord begins to fall over after Ricky punches him, there are no visible injuries shown on his head. When the drug lord is shown on the ground in the next shot, there is a huge wound and lots of blood on the side of his head that would have been visible in the previous shot. (01:12:25)

Stupidity: When Ricky's girlfriend awakens inside the drug dealer's apartment, she runs off as soon as she realises what danger she is in. She runs through the apartment building, up the stairs to the roof and then commits suicide by jumping off the roof. Apparently, when she was running up the stairs, she never considered trying to escape the building or see if there was anyway of getting downstairs. (01:11:15)

Audio problem: Just after the warden tells the inmates if Ricky can stay buried alive for seven days, he'll consider him a free man, his son runs up to the hole Ricky is in. He is heard laughing whilst running up to the hole, but his chest and stomach aren't shown moving along with the laughing, implying the laughs were dubbed in afterwards. (01:07:50)

Other mistake: Just after Tarzan has been crushed and killed by the ceiling being lowered to the ground, Ricky finds a photo of Tarzan and his mother that slipped out of the jail cell, just in front of the entrance beforehand. There is no way this picture could have slipped out of the cell. He was standing nowhere near the entrance to the cell whilst he was being crushed and anytime he was shown standing near the entrance whilst he was fighting with Ricky, so nothing could have happened for the photo to unintentionally slip out of his pocket and out of the cell. Other mistake: When the ceiling in the jail cell has lowered all the way to the ground, Tarzan is crushed by it and killed. Despite Tarzan's body being crushed by the ceiling, the ceiling touches the ground incredibly smoothly. The lowering of the ceiling is in no way delayed or slowed down by Tarzan's body being in the way, and the sound of the ceiling touching the ground is in no way muffled or affected by Tarzan's body being crushed.
