Frasier (1993)

The Botched Language of Cranes - S2-E6

New this month Plot hole: Several times throughout the show, it's bought up that there is a seven second delay on Frasier's show at KACL, which, among other things, would allow Roz to "dump" the last seven seconds of what's been said and prevent it from being broadcast. When Roz notices that Frasier is still on the air during his rant, it takes her about nine seconds to get back to her console, so she should have been able dump the last seven seconds of his rant, leaving only the first few words of Frasier's rant (likely no more than "Good grief, have you...") going to air, which would be much less awkward compared to his entire rant.

The Newsroom (2012)

5/1 - S1-E7

Continuity mistake: The stock footage of the airplane that Don, Sloan and Elliot are on is of a Boeing 747, a very large widebody plane. However, the internal shots of the plane suggest that it is actually a much smaller narrow body plane.

Frasier (1993)

Something About Dr. Mary - S7-E16

Audio problem: After Mary says "Except I bet you didn't walk through a metal detector to get to class", Frasier in that shot does not move his mouth in line with the dialog that he speaks, but instead appears to be laughing. Additionally in the next shot, although his mouth now matches his dialog, he is half way out of his seat despite being sat in it in the previous shot.

Gilmore Girls (2000)

Just Like Gwen and Gavin - S6-E12

Continuity mistake: When Lane spins the Doggy Swami wheel, the number 5 is at the top, and Lorelai removes the piece of paper under it. When it changes to the close up shot, the wheel now has the number 20 at the top with the piece of paper under it drawn.

Dick Puts the Id in Cupid - S5-E11

Other mistake: While talking to his therapist, Dick tells her that one of the mishaps that he caused in his relationship with Mary was showing up to a fundraiser naked. While true, this doesn't happen until Episode 13 (Rutherford Beauty) - two episodes later. This was due to the fact that most of Season 5 was aired out of production order.

Dick on a Roll - S3-E12

Trivia: Dick is never seen walking in this episode and is only in an upright position when he moves from the chair in the lounge room to the wheelchair in the opening scene. This is because John Lithgow hurt his ankle playing tennis, and the episode was written to allow him to be in a wheelchair for the entire episode.

Will & Grace (1998)

New Lease on Life - S1-E2

Visible crew/equipment: In the scene where Grace shows Will the new apartment for the first time, you can see the boom microphone pop into the top of the frame several times.

Our Man Bashir - S4-E10

Factual error: As is typical for Star Trek maps of the world, New Zealand is missing from Dr. Noah's map of the world before anything is flooded. The Australian state of Tasmania is missing as well.

Doctor Bashir, I Presume? - S5-E16

Character mistake: In the scene in Sisko's office where he has contacted Rear Admiral Bennett, Bennett says "Two hundred years ago, we tried to improve the species through DNA resequencing, and what did we get for our trouble? The Eugenics Wars." This puts the wars in the 22nd century, and differs from every other mention of them, where they occurred in the late 20th century, not the 22nd century as Bennett implies.

Bad News - S6-E13

Trivia: Throughout the entire episode Bad News (Season 6, Episode 13), there's a number in every scene. These numbers are a countdown for the news that Marshall will get at the very end of the episode.

Glee (2009)

Audition - S2-E1

Continuity mistake: While Will is holding Quinn to prevent her from attacking Santana after their fight, as Santana walks away, in one shot Quinn has her body is towards Will's: in the next, she's suddenly turned around and her body is now the other way.

Frasier (1993)

Match Game - S11-E18

Continuity mistake: The scene where Charlotte comes to Frasier in the restaurant to tell Frasier his date has cancelled is poorly edited between shots. Watch the folder once she has pulled it out: in almost every shot, the side facing the camera changes, and when Charlotte gets up, it disappears completely.

Silly Love Songs - S2-E12

Continuity mistake: When Lauren drags Puck into the storage closet for Seven Minutes In Heaven, there are hearts on the wall which appear to be decorations for Valentine's Day. As the Seven Minutes In Heaven happened prior to Sectionals, which was well before Valentine's Day, these shouldn't be there.

Mattress - S1-E12

Other mistake: When Finn and Rachel sing Smile in the Choir Room in Mattress (Season 1, Episode 12), Finn doesn't appear to look at any other page but the first one in the music given to him by Rachel. It is unlikely that the entire score for the song was on the first page.

Y2dicK - S4-E17

Factual error: Mary gives Dick directions to press Command-Q to close something. As Dick is using a Compaq computer which would be running Windows, he would not have a Command key and would instead have a Control key (which he actually is pressing) as the Command key exists only on Apple Macintosh computers. Ctrl+Q on a Windows machine doesn't do anything.
