Continuity mistake: In the scene where Anakin arrives on Mustafar and tells R2-D2 to stay with the ship, he pulls the hood of his robe over his head with two organic hands. His right hand should be mechanical. [This mistake is mentioned in the audio commentary: this shot is taken from a shot of Obi-Wan later in the film when leaving Padme's apartment. Still a mistake, though.]
Continuity mistake: During the Anakin/Obi-Wan fight scene Obi Wan switches lightsabers with Anakin. When Anakin is strangling him we can definitely see that Obi-Wan is holding Anakin's lightsaber in his right hand (note the hilt design), but when the shot is reversed Obi-Wan is holding his own lightsaber in that hand.
Continuity mistake: When Anakin is in the medical centre lying on his back being transformed into Darth Vader, you can see the main part of the mask being lowered into place but Anakin is not wearing the neck section of the mask that it connects to. We then see the main part lowering directly downwards, in the next shot the neck section is in place.
Continuity mistake: In the scene where Anakin returns to Padme's apartment after the fight at the Jedi temple, watch R2-D2 in his compartment in Anakin's ship as the camera angle changes. In some shots, R2's "legs" are flush with the hull of the ship, and in others, they are well above it.
Other mistake: To escape from the elevator, Anakin cuts a hole in the ceiling. Later, when they rescue the Chancellor, they almost get crushed by another elevator (it is another one. Each elevator has a number. Obi Wan said the number of this elevator before and it was different than the one that had the hole). However, when a shot of this elevator is shown, it has the same hole that the other elevator has (better seen in slow motion).
Continuity mistake: When Palpatine is talking to Anakin in his office, there is a table at the end of the hall with a large object in the middle, and a gold cup on each side. When they leave the office and are talking in the hall, and a thin purple lamp/object appears on the end of it for a few shots, and vanishes whenever seen from then on.
Character mistake: During the opening space battle, Obi-Wan's ship falls under attack by buzz-droids, who proceed to cut into his fighter, disabling a number of systems and leading Anakin to try some decidedly dangerous moves to get rid of them. Both Anakin and Obi-Wan are acknowledged experts in the use of the Force and both have been shown to use the Force routinely, even for relatively mundane activities. It's also been firmly established that the Force works on droids and that the Force-user can still affect others through windows, viewscreens and so forth. Yet neither thinks to use the Force to deal with the buzz-droids.
Continuity mistake: When the Emperor says "I have waited a long time for this moment, my little green friend," Yoda can be seen lying beside his cane on the purple carpet. When they show him standing up, he and the cane are now further back toward the wall on the tan part of the carpet.
Continuity mistake: When Mace Windu and Palpatine fight, Windu backs Palpatine into the corner of the window. Note how close Windu is to Palpatine the whole time. Then from Anakin's perspective, Windu takes a few steps toward Palpatine, his saber pointed at Palpatine. When we saw it from the earlier perspective Windu never took those three steps toward a stationary Palpatine.
Other mistake: During the scene of Anakin's transformation into Darth Vader, there is a close up shot of his face just before his helmet is put in place. All of his face is shown as being badly burned with the exception of his eyelashes, which are still impossibly there. Subjecting one's face to any heat source hot enough to sear one's face will also burn away one's eyelashes as well.
Continuity mistake: After Anakin has told Windu that Palpatine is the Sith Lord, Windu gets on his ship which takes off. We see the ship's shadow moving on the ground right next to Anakin. When it cuts the shadow from the ship is gone. When it cuts again to a wider shot we see the ship's shadow suddenly appear again.
Continuity mistake: When Mace Windu and Palpatine are in his office after the lightsaber battle, Palpatine scrabbles into the corner as Windu paces towards him, ending up directly over him with lightsaber pointed at his throat. As Anakin walks in, Palpatine is already backed into the corner and Windu needs a few paces to get to him.
Continuity mistake: In the scene where Plo Koon gets blasted by his Clone Troopers, the exterior shots show he is in an Episode II starfighter (closed/pointed bow, no small rear window on cockpit canopy- best seen as the Emperor gives the order), yet the interior shots of his cockpit show he is inside an Episode III starfighter (forked bow, small rear window on canopy- same as Obi-Wan's starfighter).
Continuity mistake: When the Emperor and Yoda flip backwards out of the pod from the "kickback" of the Force lightning the Emperor falls onto the sloped side of the pod.When they show him again he is hanging on the side of the pod with handles.(If you look at the pods each has a sloped side and a part with guardrail or handle)
Continuity mistake: When Obi-Wan kicks Anakin for the second time as they fight on the huge table, as the shot ends, Anakin is about to land on the right side of the table. In the next shot, he lands on the other side, and in the following shot, we see the impact of him landing again. Also, his position as he lies on the table keeps changing between shots. This is easy to see in the wide shots, and in the close-ups, where it can be seen from the pattern and the lit strip on the table. Also, notice that as his lightsabre flies into his hand, his hand is on the edge of the table, but in the previous and following shots, he was lying too far away from the edge to reach it.
Continuity mistake: Before the lightsaber battle between Grievous and Kenobi, the area where they are standing and where Kenobi "drops" the weight on the magna droids is a solid floor. The whole preparation-for-battle-scene does not include much movement from the actors. As soon as Grievous begins to move in on Kenobi, both of them are suddenly standing on some sort of bridge, without even passing the dropped weight.
Continuity mistake: Just before the lightsaber battle between Mace Windu and Darth Sidious, we see the lightsaber of Sidious ejected from his sleeve and he grabs it with his finger down the length of it. We then instantly cut to a wide shot and he's suddenly gripping it with all his fingers curled in.
Audio problem: After Anakin kills Count Dooku, Palpatine says, "Remember what you told me about your mother and the Sand People." In the next shot, his head is shown from behind, but if you watch closely, you can see that his jaw is moving as if he were talking, but nothing is heard. Use zoom for confirmation.
Continuity mistake: When Padmé is in her apartment and Anakin is in the Jedi Council chamber, they look out across the city toward each other. The position of the setting sun and the direction of its light across their locations is not consistent given what we are shown about their relative positions and the directions they are facing.
Continuity mistake: When Obi-Wan jumps to the high ground during the duel with Anakin, in the shot where he lands, his lightsabre is held parallel to the ground. In the next shot, it is pointing downwards, and in the following shot, it is pointing slightly upwards, and his left arm is positioned differently from the previous shot.
Deliberate mistake: In the final stages of the Obi-Wan/Anakin battle, they are fighting while floating over lava on a piece of metal. Just before Obi-Wan jumps to the high ground, we get a close-up shot of Obi-Wan (lightsaber pointing left off-screen) and then Anakin, (lightsaber pointing right off-screen) In this shot, Anakin's right hand is uncovered/real, and his left is gloved/mechanical, when it should be the opposite.
Continuity mistake: In the scene following the crash landing of Grievous's flagship where the shuttle is approaching the Senate's "parking garage" (not the Senate building itself or the Jedi Temple as others have said), the shuttle is approaching a section of the landing platform that is void of any other ships. However, in the following shot, another shuttle with different coloring and larger size is in plain view.
Continuity mistake: During the Anakin vs Obi-Wan fight, there is one bit where they are balancing on a thin pole. The shot starts off above them and you can see that there is nothing underneath them. They take a few steps backwards while fighting, but then they both jump off on to one of the arms of the structure they are fighting on.
Continuity mistake: When Obiwan and Anakin are fighting on the narrow walkway, lava splashes over it and causes that portion of the building to fall into the magma river. As it is falling, there is a significant amount of lava on the severed end of the walkway, which ends up as the "top" of the tower once floating on the river. For the rest of the scene this lava is gone, and none is shown falling on or by Obi-Wan and Anakin as they scramble along the side of the tower.
Continuity mistake: When Obi-Wan and Anakin are fighting on the table in the control centre, they are standing/crouching in a different position from the previous shot after every time they kick each other. For example, when Obi-Wan kicks Anakin for the second time, as the shot ends, both of his feet are on the table, but in the next shot, one of his feet are up again.
Continuity mistake: After Anakin has been turned down as a Master in the council, he is walking with Obi-Wan. While they are walking you can count them passing at least five windows. When it cuts to Obi-Wan saying "Anakin, I'm on your side, I didn't want to put you in this situation", you can see, by looking at the sun shining through the windows, that they have only passed three windows.
Continuity mistake: General Grievous, whose two arms can split into four, has a total of twelve fingers (six on each hand when his arms aren't split). However in some shots (such as when he orders his ship's bridge crew to fire the emergency booster engines in an early scene) we can see he has five on each hand when his arms aren't split. In other shots, he has the correct number of fingers.

Continuity mistake: During Anakin and Padmé's conversation on Mustafar, in the first shot, they are standing near some sort of container. They continue to stand in the same place in the following shots. When Anakin sees Obi-Wan and becomes furious, both of them take several steps away from the container, but when Anakin says, "You brought him here to kill me.", they are standing near it again. In the following shots, it is nowhere to be seen.

Continuity mistake: After discussing whether Anakin is the chosen one or not, the ship Yoda, Obi-Wan and Windu are flying on goes in for landing. When it goes down there is no trace of clones on the ground where it is about to land. When it cuts several clones awaits them. Also two huge vehicles suddenly appears on the ground.
Continuity mistake: When Count Dooku drops the platform on the unconscious Obi-Wan, Obi-Wan is not lying on the same part of the floor as in the previous shot of him, seen, for example, from the pattern on the floor. Later, when Anakin walks over to him, his position is different again, and the platform has moved as well. He is also lying under a different part of the platform.

Continuity mistake: When Anakin and Padmé are talking on the couch after Anakin is appointed to the Jedi council, they keep changing position between shots, depending on the angle of the camera. The easiest way to see this is to watch the red cloth hanging behind them on the couch. In some shots it is behind Padmé, in other shots, it is behind Anakin.
Continuity mistake: When Obi-Wan and Yoda arrive at the Jedi temple to find all the slain Jedi, there is a wide shot showing Obi-Wan walking several feet ahead of Yoda. Obi-Wan then raises his hands and is about to remove his hood when it cuts and now Yoda is suddenly right next to Obi-Wan.
Audio problem: In the scene where Anakin turns to the dark side, Darth Sidious says, "You're fulfilling your destiny, Anakin." As he says this, his head is shown from behind and is in shadow, but the words don't match the motion of his jaw, and his profile shows that his mouth is moving after the words are heard.
Continuity mistake: Just before leaving in his spacecraft to go and fight against general Grievous, Obi-Wan Kenobi is seen being briefed by Clone Commander Cody. During the briefing, you can see that at least one of the clone troopers on the right does not have the same head as the Clone Commander. But, when Obi-Wan climbs onto the wing of his spacecraft and enters it, the clone troopers on the right suddenly all have the same head as the Clone Commander.
Continuity mistake: When Anakin and Obi-Wan are on their way to find Palpatine, destroyers appears shooting at them. In the shot behind the destroyers while they are shooting we can see the door behind Anakin and Obi-Wan are closed. When it cuts to a shot of them the door has suddenly opened a foot in a second.
Continuity mistake: In the beginning, when Obi-Wan and Anakin are returning after saving Palpatine, you can see their ship approaching the platform is about to go under a large shadow. But in the next shot, it takes quite a while for the shadow to even touch the ship, and the shadow is of a different position/size. (The shadow is from an over-hang above them, for other ships I assume).
Continuity mistake: In the Palpatine/Yoda lightsaber duel, during the second time they lock sabers, Palpatine's blade is pointed down, then in a closeup of his face, his blade is at an upward angle. Then when it cuts back to Yoda, his blade is pointed down again even though blades were constantly locked. This may only be noticeable in widescreen.
Continuity mistake: When Ki-Adi-Mundi is killed on Mygeeto, as the clone troopers stop and point their blasters at him, the area where they're standing consists mostly of sand. In the following shots, the sand is gone and there is only snow there (best seen in the shot from behind them). Even though it's snowing, the snow couldn't have covered up the sand that quickly.
Continuity mistake: When Palpatine appoints Anakin to be on the Jedi Council, as the first shot ends, they are quite far away from the large window and the column next to it. In the next shot, they are much closer to them. In the following shots from behind Palpatine, the background changes completely; for example, the window is gone, and a metal object and a shelf have suddenly appeared behind Anakin. Then, when Palpatine says, "I'm depending on you" they are suddenly standing in the doorway.
Continuity mistake: When Anakin, Obi-Wan and Palpatine are about to crash land we get a view inside the cockpit of the aircraft with the pilot saying "We'll take you in". If you look out the window we can see he is flying on the spaceship's right side while spurting water on it. When it cuts we can see that none of the aircrafts are spurting water on the right side of spaceship until a few seconds later.
Continuity mistake: On Utapau when Obi-Wan is looking down upon Grievous and the Separatist leaders, you can see Grievous is very far away (judging by the floor panels, perhaps 10 floor panel widths taking into account the diagonal angle at which Obi-Wan is looking at Grievous). Obi-Wan proceeds to jump straight down, but in the next shot where Obi-Wan lands, Obi-Wan is only 1 floor panel width away from Grievous. Then, in the next shot which shows Obi-Wan surrounded by droids, he is now 2 floor panel widths away from Grievous.
Continuity mistake: As Anakin and the clone troopers attack the Jedi Temple, C-3PO says to Padmé, "The Chancellor's office indicated Master Anakin returned to the Jedi Temple." Padmé is holding her hand on her chest, and then begins to bring it downwards. In the next shot, her hand is near her throat.
Continuity mistake: When the Emperor addresses the Senate, as he says, "In order to ensure the security and continuing stability", his hands are held at waist level. As the next shot begins, his hands are higher up. Then, when the Senate applauds, he begins to raise his hands. As the following shot begins, his hands are held in a much lower position. No one moves that quickly.
Continuity mistake: During the Sidious and Yoda battle, the Emperor throws down several pods at Yoda that pile up on the floor.The pile changes in the number and position of the pods between scenes. This is especially noticeable when Yoda's lightsaber is shown falling to the ground, almost no pods are on the floor verses many pods when Yoda falls.
Continuity mistake: After the crash landing on Coruscant, Anakin, Kenobi, and Palpatine take a shuttle to the Jedi Council. In the first shot as the shuttle is landing, all shadows are parallel with the edge of the landing area. In the next shot when the shuttle doors open, all shadows have shifted 45-degrees clockwise.
Continuity mistake: In the first shot of the scene where Yoda leaves Kashyyyk, as Tarfull begins to walk towards Chewbacca, Yoda is several metres away from them. In the next shot, he is much closer to them, and Tarfull, who is still walking, is further away from Chewbacca than in the previous shot.
Continuity mistake: As Anakin is reentering the elevator from the top, you can see the hole being very close to the entrance door of the elevator. As we see from the subsequent shot, Anakin is landing between Obi-Wan and the aft elevator wall (Obi-Wan does a half turn to face Anakin), whereas according to the hole, Anakin should have landed either directly on top of Obi-Wan or between Obi-Wan and the elevator door.
Continuity mistake: When Obi-Wan gets on the intercom with R2 the first time on Grievous' ship, as it cuts to R2 backing up against the corner hiding from the droids we can see that there is no light casting his shadow. When it cuts to R2 again two shots later, bright light suddenly appears, casting his shadow.
Continuity mistake: When Anakin, Obi-Wan and Palpatine are falling down the elevator shaft and use the hook grip to swing themselves in the elevator door, the land on the light on the floor from the ceiling. When it cuts to them getting up they have moved two feet away from the light.
Continuity mistake: In the first shot of the scene where Luke and Leia are born, Padmé's head reaches the top of the black area of the bed. Her head is seen in all the following shots, and her position doesn't change. But when she says, "Oh, Luke", her head has moved about 10 cm down from her previous position.
Continuity mistake: When Palpatine and Windu are fighting they end up standing stuck face to face with their lightsabers. At first Palpatine's lightsaber is pointing to the right side of Windu. When it cuts to a close-up of Windu Palpatine's lightsaber is pointing to Windu's left.
Continuity mistake: When Yoda is done with the holographic meeting with the rest of the Jedi Council about sending Obi-wan after Grievous, he gets up and walks to the trooper behind him. We also see Chewbacca follow him, stop, and cross his arms. When it cuts to the trooper saying, "The droids have started up their main power generators," we see Chewbacca still walking and stopping, but this time he doesn't cross his arms, he places them on his hips.
Plot hole: In the scene where Mace Windu and the other three Jedi attempt to arrest Palpatine, the first Jedi that Palpatine kills practically has his back turned to him, which is not something a Jedi Knight who is trained to be mindful of his surroundings would do. As we can see in the film, it takes Palpatine several seconds to stand up from his chair, draw out his lightsaber, threaten the Jedi, leap over his desk, and then lunge at them, meaning the Jedi had plenty of reaction time to avoid this.

Audio problem: When Obi-Wan speaks on R2-D2's radio, the sound editor cut a lot of frequencies, bass and middle, to make it sound like a radio. But when R2-D2 puts the radio in his shell, "magically" there is a lot of low middle in the voice. It does sound like Obi-Wan is talking from inside R2-D2, if it wasn't a radio, but in this case it is and shouldn't sound much at all, when the shell cuts the high frequencies. No low frequencies + No high frequencies = No frequencies.
Revealing mistake: After the battle between Obi-Wan and Anakin/Darth Vader, Vader lies badly burned on the ground slowly crawling up. Palpatine's shuttle flies by, as seen by a shadow. However, even the lava in the shadow turns dark, like the lava is glowing from an external light source instead of glowing within itself.
Continuity mistake: During the fight scene between Obi-Wan and Grievous, Grievous has the lower half of his left hand and upper half of his right hand severed during the saber duel. However, as he gets into his vehicle, the damage done to his right hand reverses itself and he is missing the lower half instead.
Continuity mistake: In the fight scene between Obi-Wan and Grievous, look closely at the lightsabers in Grievous' right hands as he ignites them. The one on top is an Obi-Wan hilt(blue) and the bottom is an Anakin hilt(green). Throughout the fight, these two sabers alternate colors constantly between shots. When Grievous' hand is severed the second time, he drops the Obi-Wan(green) and retains the Anakin(blue).
