Continuity mistake: When Spartan and Huxley arrive at the cryogenics place to battle Phoenix and what remains of his gang, Phoenix sees them on the monitor walking into the building and you see Spartan on the left and Huxley on the right. The camera angle changes to an inside view and Spartan is now on the right.
Continuity mistake: When Spartan and Phoenix are fighting in the sewers, Edgar Friendly shoots his gun at Phoenix and you see the catwalk that Phoenix was on tip to its side and Phoenix dropping down to the ground with the plank of the catwalk right below him. When Phoenix lands on the ground the plank is nowhere to be seen. It should have been right below him.
Continuity mistake: When John Spartan is frozen in the cryochamber, at the beginning of the movie, if you watch during the credits - while the time is going by the small circular monitor that is on his chest, it changes from the left side to the right side of his chest in almost every shot.
Other mistake: During the scene where Phoenix is questioning the leader of the future society in his office, he says at least one word (hell) that he should have been fined for, and isn't. We see that there is a ticket printer in the room in a later scene, when Savage sets it off during his own confrontation.
Plot hole: When Spartan jumps through the window in the excavation scene of the museum, he crashes into a room with bottles of drink on the shelves. Surely this room too would have been filled in and the window smashed during the earthquake, and the museum wouldn't have bothered to excavate it as it can't be seen from above due to the blanked out window. So what is that room doing there?
Continuity mistake: When Phoenix first gets out he makes his way to a computer and the police come to arrest him. Phoenix takes one of the policeman and pushes his head through the side glass pane and walks away to fight the other officers, but when Phoenix goes back over to the computer the glass is not broken.
Factual error: Simon Phoenix uses the warden's removed eye to escape from the cryoprison. The eye is on a pen, and since the retina includes the back portion of the eye, the pen's nib would have not only destroyed the shape of the retina, but blocked the scanner from reading the complete retina as well. There's no way the retinal scanner would have let him pass.
Character mistake: In the scene where John Spartan goes into the sewers, he has a conversation with a Spanish-speaking woman. At the end of their conversation the woman says "Gracias" (correct Spanish) to which Spartan replies "Prego", which is Italian. Up until that point Spartan spoke perfect Spanish.
Other mistake: When Simon Phoenix kicks through the windshield of the police car during his first fight with the SAPD, the windshield easily breaks into several large segments, as opposed to spider-webbing like real windshields made from safety glass do. Glass that would completely shatter and collapse into the car from a simple kick would be completely unsafe and illogical for a society that's supposed to be more technologically advanced than ours.

Continuity mistake: In the opening credits when Spartan is frozen, both hands are up either side of his head. In the very next shot his left hand is by his head, but his right arm is out stretched and in a down position, impossible as he is frozen. Also his mind altering sensors are on the left side of his head and body before freezing, next shot they are on the right side of his body.
Other mistake: The Verbal Morality Code seems to be applied in an almost haphazard fashion. Several times throughout the movie, both Spartan and Phoenix swear, and do so near an area where they should be vocally reminded of breaking the code, in fact have been previously reminded, and in spite of them not moving from that location are not warned again in the face of repeated swearing.
Continuity mistake: When the Cryo-Prison worker opens the cabinet that holds the cylinders that contain the blue "freezing balls" to freeze Spartan's slush, the slot on the center left is empty. When the shot changes to the worker pulling the cylinder out, the slot to the lower left is now the empty one.
Revealing mistake: During the slo-mo portion of the fight between Spartan and Phoenix, Snipes intentionally slowed down his movements to allow Stallone more time to smash the glass tube; this is not consistent with the other fights in the film where Snipes was always quicker with his actions.
Factual error: In the scene where Wesley Snipes has supposedly poured gasoline all over the floor, he lights a torch and holds it inches from the floor threatening to light it if Sylvester Stallone comes any closer. Had it been actual gasoline, with that much of it the fumes would have exploded as soon as he lit the torch. It's the fumes that are explosive, not the liquid.
