Plot hole: Frank Ochoa told miss Nicole that Paul killed 9 people in New York when in fact he killed 10. (00:47:00)

Character mistake: After Kersey has killed Nirvana in the hospital, the attendant arrives in the room. When the attendant finds Nirvana dead, Kersey tells him that he raped and killed his daughter. But it was PunkCut who raped her.

Revealing mistake: When Kersey tries to shoot Wilson at the hospital, he lifts up the table to knock his gun away from him. Kersey fires a shot at the table when it moves up, but a hole doesn't appear in the table. This indicates the gun was loaded with blanks (obviously) and no other effects were done for the scene.

Continuity mistake: When Kersey is writing the fake ID for the hospital he drops a piece of paper beside the trash can but the paper disappears in the open view, then reappears when Geri takes it.

Other mistake: After Kersey kills Wilson at the hospital he falls down at the wrong angle, because his arm was stuck in the electrical panel and he should fall with the arm inside the machine.

Other mistake: Towards the end when Kersey kills the 4 mugs at the park, he shoots the guy in the car then the car falls down off the cliff and explodes with no reason.

Revealing mistake: During the final fight sequence between Paul Kersey and Nirvana at the hospital, at one point you can see that Kersey is not Charles Bronson but his stunt double.

Continuity mistake: In the scene when Paul Kersey meets Geri at the rooftop Japanese restaurant, he drinks from his glass of Dom Perignon and puts his glass down on the table. When the scene changes he is again putting his glass down.

Other mistake: Towards the end of the movie when Paul Kersey goes into the doctors' locker room to steal an ID tag, he checks several lockers to see if they will open. All the lockers that he tries are unfortunately locked. The problem is there are no locks on any of the lockers. (01:13:55)

Continuity mistake: Paul Kersey boards a bus where the line number reads 990. When he gets off, the number is 99. He could not have changed buses because the number of the bus itself remains 8027 throughout. (00:54:50)

Question: Why did Paul Kersey move to LA from New York when there's already a high crime rate? Shouldn't he and his daughter have moved to a safer town or city with friendly people and less crime?
