Plot hole: Right after Bond escapes Blofeld and joins Tanaka and Kissy, they say they need to get to the Control Room (that Bond just left from), they look at the "front door" to the Control Room and Tanaka says "Impossible! It's too well defended!". Why did they not go back the way Bond just came from, through Blofeld's "apartment"? Instead, they fight their way across the entire launching complex and go through another entrance, which ALSO leads through Blofeld's "apartment".
Continuity mistake: When Bond and Kissy jump into the ocean to avoid the poison gas in the cave, you can see Bond's skin through his shirt. When they come ashore on Blofeld's volcano island, Bond takes off his shirt to reveal a grey ninja outfit underneath, complete with huge rubber suction cups for his hands and knees. Where did he hide all that?
Continuity mistake: At one point, during the scene with the helicopter dogfight, the tail wing of Bond's mini-copter is riddled with bullet holes (and yet the mini-copter continues to fly very efficiently), but for the rest of the scene, the tail wing is in perfect condition with no sign of the damage done to it.

Continuity mistake: In the scene at the end where Bond is trying to make Blofeld's spaceship blow up, you can see the timer showing the time till interception. On the 1st shot the timer shows 25 seconds. On the second shot it says 13 seconds. Then there is another shot and the timer shows 24 seconds.
Factual error: In the scene where the ninjas are attacking the volcano, they are armed with rocket guns. While the rocket guns, MBA Gyrojet Rocket Carbines, are real, their 13mm rocket cartridges could barely beat out a .45 ACP pistol cartridge in stopping power, not to mention they couldn't cause huge explosions like they did in the movie.
Plot hole: In the volcano base, Bond finds and frees three captured astronauts - two Russians and an American. But when they enter the SPECTRE astronauts' room, there are only two of them with Bond (and there is no indication that the third one was seriously hurt in the previous struggle with the guards); and after Bond is dressed in the spacesuit, they disappear from the story altogether.
Other mistake: When James Bond is in Tanaka's underground train enjoying the sake wine. One servant enters saying that the photograph is ready for viewing. This servant then walks to the door, which is on the left-hand side of the carriage. After going through the door, we can see the servant taking a left turn, but where to? Outside the moving train?
Revealing mistake: In the scene where the launch platform is moving in Blofeld's cave to move the rocket to the Volcano crater for launching, there is a close up of the platforms wheels. Some are moving in the right direction,others aren't moving at all,making it look like the plastic model that it was. If it was a real platform, the weight alone would have made all the wheels turn in the same direction.
Plot hole: Blofeld tells the bad guys that they had Bond and let him get away. The woman says "but Bond is dead. It was in all the papers." As we saw in a paper near the start, there was a well publicised photo of Bond, and when she met him he wasn't using any kind of disguise, so how did she not recognise him if she'd seen the papers?

Audio problem: In the volcano's control room, there is a husky guy in red uniform whose only purpose is to open and close the crater. Every time he opens or closes the crater he says with a deep voice, "opening crater, closing crater." When Bond kills him with the rocket cigarette to open the crater and let the ninjas in, and the guards have subdued Bond, the guard in white uniform who closes the crater says, "closing crater," and you can hear that it's the dead husky guy's voice.
Audio problem: During the scene where Bond has to make his way back into the control room, he asks Tiger Tanaka to cover for him. Tiger does so with an MP-40 submachine gun. It's the loudest gun on screen when Tiger is firing it and has the MP-40's distinctive clatter. So far, so good. However, watch the gun's muzzle flashes (the MP-38, MP-38/40, and MP-40 as well as the MP-41 all have cyclic rates slow enough to distinguish between individual shots). When Tiger fires the first burst out of the gun, it corresponds perfectly well with the muzzle flashes. However, after cutting to Bond and then back again to Tiger, something went wrong. The muzzle flashes don't correspond at all to the firing noises from this point onward. In fact, when Tanaka takes his finger off the trigger momentarily to presumably prevent muzzle climb or let the barrel cool for an instant, the firing noises keep going.
Visible crew/equipment: When 007 is on the "Little Nelly" and is about to fly over the village, there's a shot in front of him and you can see another helicopter in the background. This is not the helicopter that attacks Bond later - it must be the one used to film the action from behind.

Continuity mistake: In the scene where Bond is trapped in the light airplane flown by Osata's female pilot, she sets the airplane on fire and bails out. As she's exiting the plane, its landing gear is down. Once Bond frees himself and tries to recover the airplane, the landing gear is back up.
Other mistake: When Bond arrives on the island with the fake volcano, Kissy tells him that nothing unusual has happened there, except the death of a diving girl when she ventured into a cave. Even if the construction of the launch facility in the volcano had been kept secret it would have been impossible to launch the large rocket twice without anybody noticing.
Continuity mistake: The first time we see the fake lake retracting in the volcano it is moving very fast, and before the cut it is almost totally retracted. But in the next clip when seen from below the opening is only half of the crater and the roof is retracting much slower.

Deliberate mistake: When 11 is dropped into the water and eaten by the piranhas, although the ravenous fish probably ate most of her and what was on her, there should at least be pieces of clothing that would rise to the surface in the flurry of activity. Not to mention that even though the water is a murky green all the blood from her body should have created a temporary, but very noticeable darkening of the water.

Revealing mistake: At the beginning of the film during Bond's "funeral", the shot cuts to a man watching the funeral from a boat through a pair of binoculars. The camera then moves to the newspaper next to him showing an "article" reporting Bond's murder. If you look closely at the "article" itself, you can see it was actually pasted onto the actual local newspaper. They probably bought a local newspaper and pasted the fake article over it to make it look authentic.
Continuity mistake: When Henderson is stabbed through the wall, Bond runs over and grabs him. There's a noticeable hole, and they're standing right by the wall. Then the angle changes, the hole's now smaller, and Bond's right next to the bed, which he can put Henderson on without taking a step.
Continuity mistake: During the car chase sequence, the radio antenna of the black sedan disappears and reappears between shots. It is most noticeable when the sedan is being lifted off. In the close-up as the magnet is attached to the car's roof, the antenna is visible. In the shots in which the car is shown from above, the antenna has completely disappeared.
Revealing mistake: Upon capturing the Soviet spacecraft, SPECTRE's spacecraft does some attitude maneuvering with small rockets on the side of the hull. The rockets fire visible exhaust from screen-left to screen-right, but flecks of the stuff slow to a halt and accelerate backwards, right to left, exactly as if the spaceship model were standing upright with the camera tilted sideways, which is exactly what was done.
Other mistake: Early in the movie, in the shot of the police vehicle screeching to a halt in front of the hotel Bond is staying at you can see a crowd of people huddled together on the sidewalk with a policeman making sure they stay put. (The producers had to order the area in front of the hotel closed off to civilians as a precaution as so no one would get hurt or get in the way of the filming).
Factual error: In the beginning scene involving the U.S. spacecraft, as the front door on the unidentified ship starts to close around the U.S. capsule you can see the reflection of red light across the hull of the captor's ship. Where is this light coming from? It can't be from the sun, because sunlight only appears white to our eyes.
Continuity mistake: In the scene where Bond is at the Sumo wrestling arena, when he is being escorted to his seat, the ref overseeing the fighting ring is wearing a red robe, but right after Bond takes his seat, the ref's robe turns to a light green color and stays that way for the rest of the scene.

Continuity mistake: After expending one of his exploding cigarettes to cause a commotion, Bond throws a switch to open the crater door; we see two shots of it opening from below. However, in the very next shot where Tiger and his ninjas are clambering on top of the door, it is not shown moving beneath them at all (this can be seen because of the proximity of the rocks next to the fake water).
Other mistake: The first few ninjas enter the volcano on ropes thrown down when 007 partially opens the crater roof. After the roof is closed, all their ropes should be collected to the right side of the screen after the roof is shut. But the ropes are not gathered to the right side. Similarly, ninjas next enter the crater through the hole blown in the crater roof. But in scenes following the crater explosion, you see ninjas descending into the crater on ropes tided to the under pinning of the crater roof instead of ropes coming through the blown out roof hole. Also, the blown out crater roof hole s not visible in the subsequent scenes.
Continuity mistake: When the hired thug is driving a disguised Bond to the Osato Building, the thug looks back at him. The forward view does not show the hood, wheel or dash of the car. After he goes back to driving, Bond peeks up to see where they're going, and now you can see the dash, and wheel. Also, when the thug looks around, the mirror shows only a blurred image of Bond, not the face and back of the thug's head. As Bond sees the building, the camera can now see only the thug's reflection in the mirror. Finally, they were in a downtown area with no dead end in sight. Then suddenly, there's the building. With no turns, that's a fast moving car.

Continuity mistake: In the fight scene in the Osato building, Bond uses a couch as a battering ram against the guy that carried him in. After the third hit, Bond drops the couch after the attacker flips over it onto the floor. The next moment, he has the couch up ram-style, and the other man is back up, in the place where he can be hit again, a good five feet from where he was on the floor on his back.

Factual error: At the dock where Bond and Aki are investigating the Ning-Po, there are cylinders labeled as containing turpentine made by Osato Chemical, but which Bond figures out are really filled with liquid oxygen (LOX), presumably to be incorporated with rocket fuel in SPECTRE's mischievous meddling with other nations' spacecraft. To keep the LOX inside cold and liquid, the container needs to slowly vent oxygen to carry away heat which is constantly entering from outside. This venting would be visible as a stream of water vapor exiting a port, such as what's commonly seen leaking from the sides of rockets which are waiting on their launch pads. There is no such venting, which means the LOX inside will no longer be LOX by the time it gets to where it's going.
Continuity mistake: When Bond and Aki are attacked at the docks in Kobe, Bond tells Aki to go and contact Tanaka. Aki then runs to a rope to let herself down to ground level again. When she grabs the rope her long straight hair reaches down to the middle of her back, yet when the shot changes to her starting to climb down, her hair changes in structure and only reaches down to her shoulders. Clearly a stuntwoman wearing a wig.
Continuity mistake: In the sequence in which Bond and Aki are being chased by the black sedan, they pass a corner with a sign that reads: "Detour, road under construction" with a red arrow above pointing in two directions. They turn right at the next corner and turn again at the same corner with the same road sign. This second time the scene cuts sooner but it is the same corner. The shot was used twice.
Continuity mistake: As Bond leaves the building of Osato Chemical and Engineering, a black sedan with gunmen is slowly following close behind. Aki, who had been waiting for Bond, tells him to "get down" just in time to avoid the bullets which cause clouds of brown dust as they hit the concrete wall right behind Bond. The scene cuts to a shot in which we see the sedan speed by, but the clouds of dust have completely disappeared. When the scene cuts back to Bond the dust clouds are there again.
Continuity mistake: In the sequence showing the black sedan being lifted up by the helicopter, the number of driving lanes changes too rapidly between the shots. When the helicopter is approaching the sedan is driving on a highway with at least four lanes. Right in the next shot it is lifted off a road with only two lanes. In the shot from the helicopter we can see the white Toyota on a highway with four lanes, and the shots out of Aki's Toyota show a rather narrow country road ahead that doesn't even have any markings whatsoever.
Continuity mistake: When Bond is attacked by the ninja while he is training to be a ninja, Bond grabs the staff with the knife at the end, flips the ninja and stabs him on the left side of his lower back, but when it cuts to the next wide shot, it shows all the blood on the front of the ninja's training top. The only way there should be blood there was if the blade penetrated from the back and out the front, but the blade wasn't anywhere near long enough to go completely through a body.
Continuity mistake: When Bond jumps from the roof of the warehouse in Kobe both his shirt cuffs are outside his jacket sleeves. He starts correcting the right cuff, then there is a cut to the next scene and here he is still correcting his right cuff, but his left cuff is now inside the jacket sleeve.

Continuity mistake: In the scene where Bond is taken to Osato chemicals by the hired thug, right after the thug takes off his mask and they start to fight, the mask falls off onto the couch Bond was laying on, but when the thug goes to flip Bond onto the orange rug, the mask is now lying on the floor next to the couch, then afterwards (when Bond goes to pick up his trusty Walther PPK after setting off the alarm) the mask is lying on the couch again.

Factual error: When Bond is hiding on the monorail cart, it stops at the astronauts' quarters. After Bond gets out of the cart, a second cart passes by, even though the cart he arrived in is still on the track (out of shot). No one moved the cart before the second cart passed through.
Continuity mistake: When Bond takes the first sip of his martini at Henderson's place, the glass is about half full. You can see the level of the martini in the glass is below his finger. In the next clip there is more in the glass. The level is now above his finger, much closer to the rim.

Continuity mistake: During the scene where James Bond is running along the rooftop of Kobe Docks the camera pans away to give an arial view. 007 fights off the bad guys and rounds a stairwell opening with the door open. As the camera picks him up from ground level, or at least roof top level it shows James Bond opening the door before going through.
Continuity mistake: In the scene where Bond first arrives in Japan and is walking down the street, he passes by a woman (Aki) looking at something behind a store window, she is holding a small gold purse with a smooth gold band around the part that opens, but after she opens it and the shot shows a close up of the concealed mic inside, the gold band is now rough and twisted.
