Continuity mistake: In the beginning of the movie, when the little girl is falling, they film her falling all the way down backwards. When they flash to the young Simon's face and the priest and back to the girl she lands face down.

Continuity mistake: In the scene where Emma drives to the house where she finds her formula in order on the table, when she walks in, she leaves the door open, then the camera goes to Val Kilmer's face, then back to Emma, and behind Emma, the door is shut.

Visible crew/equipment: At the beginning of the car chase, when the bad guys exit the parking garage, you can see an entire film crew reflected in the door of their car. (00:55:20)

Continuity mistake: When Val goes into the Russian building to take the micro-chip he changes to a drunken Russian man and then to a tourist in the space of a few minutes, but when they show him as a tourist he had stubble on his face and that would not be possible to produce in such a short time.

Factual error: Simon recovers awfully quick from the effects of hypothermia. In the hidden room, he is shaking like a leaf and can barely speak. Even when the old woman shouts where they are, he still looks a bit out of it. However, once they escape into the underground tunnels, he appears to be in perfect health and shows no signs of discomfort for the rest of the film.

Continuity mistake: When Val Kilmer is impersonating Tretiak, he hands his phone to the real Tretiak with the antenna to the back. The shot then switches to the other side of the room, and the antenna has moved to the front of the phone.

Continuity mistake: When Agnes comes back for her kiss, she's standing safely on the inside of the railing. Then, the dogs approach and she's well above the railing. Then, she's magically transported to standing completely on the outside of the railing. The whole thing only takes a few seconds. She doesn't have time to move positions that quickly, nor did she need to. She was about to easily kiss Simon when they were on the opposite sides of the railing, but how could she fall? So, they moved her location. (00:05:55)
