Continuity mistake: At the diner, before Mrs. Duncan comes to confess about murdering her husband, Brennan takes a sip of tea. Her glass at this point is less than half full. When Mrs. Duncan walks in, the same glass is now almost full.

Visible crew/equipment: When River punches the Doctor, laying him out, just as he drops to the floor we can see the actor's green tape mark at River's feet. (00:33:20)
You Can't Miss The Bear - S1-E1
Continuity mistake: When Nancy goes into her guest room with Silas and Quinne in it, the things she are holding change position in between shots. (00:28:35)
Continuity mistake: When Izzie is looking at the patient board, there are some lines on it which have been wiped out. But in the next scene we actually see Meredith walk to the board and wipe these exact same lines off (even though they were previously rubbed out...) (00:34:05)

Continuity mistake: When Haley is sitting on the couch she has a small piece of cupcake left, but as her father picks her up and puts her on his shoulders, she now has a whole cupcake missing just one bite. (00:12:30)
The Crossing - S1-E2
Visible crew/equipment: After Hank Dale tells Steve Pinkus that he wants to know "How it feels to get away with murder," Hank throws a punch, and in the next wide shot we can see the second cameraman's arms holding an additional camera, presumably to film the close-up, at the right side of the screen. Continuity mistake: When Kellerman orders Hale to give him his weapon, Kellerman pulls out his own weapon with his left hand. In the next shot, his gun is in his right hand, and he then transfers it back to his left. (00:36:45)
Visible crew/equipment: During the scene where Dennis and the Waitress, Mac and Dee successively walk into the back office to talk to Charlie near the beginning of the episode, in some of the shots, you can see outside the office door, and you can see the top of the set has no ceiling, and even see part of a stage-light.
Continuity mistake: When Charlie is in his office talking to Larry, while going over the potential killers and victims, he writes the word "Killers" on the board and circles it. Then, when Charlie notices that Larry has left and says "gone", the writing is different. The 'k' is wider, and most noticeably, the left side of the circle is gone, and the loop in his 's' disappears. Once Charlie goes back to working, the word changes back to how it was originally written. (00:38:20)
Continuity mistake: The dancer is eating some M&M's on Pam's desk. In one shot, there are some M&M's on the table in front of Pam. In the next, they are missing.
Continuity mistake: When Reid is talking there's a big crack at the top of the window that might be a problem since they're supposed to be flying. Then a few seconds later they cut back to him and it's filled with putty. (00:05:00 - 00:06:00)
Continuity mistake: In the opening titles (for series 1-3), Stan runs outside and picks up a newspaper. He reads the headline and throws it back on the ground. But by the time Stan drives off, the newspaper has disappeared. (00:00:10)
Continuity mistake: At the end when they are confronting Harmony, they pick up the water glass for DNA testing. In the next shot over her shoulder, the glass is still on the table. (00:36:55 - 00:38:09)
Tsuinzu gâdo - S1-E5
Other mistake: Van transfers the bunch of condiments from the cart to the table. Amongst said condiments, a big jar of "MASTARD." (00:07:00)
Continuity mistake: When Katara is trying to heal the wounded earthbender, she gives Sokka the idea to take down The Drill from the inside. Sokka then stops her from saying anything while she is sitting down, and she is wearing her necklace. When she stands up in that same shot, the necklace is gone, like they forgot to animate it. (00:07:35)
Continuity mistake: When Dean says he's going to talk to Amanda to get a grip on her mental state he has his head back on the seat and is looking straight ahead. When the shot changes he's now looking at Sam. (00:27:20)
Continuity mistake: When Carl is staying with the son Earl in his motel room after he separated from his wife and starts going back and forth for the room, there are two socks on the floor by the bed, one of which changes position in another shot.
Visible crew/equipment: Just before Chief Johnson and Sgt. Gabriel get out of the car at the victim's parents' house, a crew member and lighting equipments are reflected in his sunglasses. (00:07:20)
