Revealing mistake: When they are at the Nutmar camp in the mountains, Val Kilmer tells Willow "Get on that shield!" but when they go over a 6, 7, 8 foot drop when they are sledding down the snow, you can clearly see runners on the shield.

Character mistake: When Tom Hanks starts to play on the floor piano he throws the bag he is carrying aside and when he goes to leave he doesn't pick it up. He leaves it on the floor.

Visible crew/equipment: In the scene where the souls are coming out of Freddy's chest, in the first close-up on the bottom left of the screen you can see a man's hand holding a black stick to manuoever the little arms. It's real fast but noticable on standard play. (01:20:45)

Continuity mistake: When MAC is blown by the helicopter, he lands on a man's windshield, and the man has to start dodging traffic. Between shots the traffic he is dodging changes. (00:10:45)

Revealing mistake: Any time when the two girls are in hell's corridors and running around, you see these corridors as stone walls, yet whenever they back up against one of these walls, you can clearly see they are merely rubber sheeting overlayed on a framework as the rubber sheeting bends and contracts very clearly.

Visible crew/equipment: In the scene after the storm, the ship moves in the sand. Look at its front and you'll be able to see a wire that moves the ship. It's very visible.

Factual error: At the end, when Cyndi Lauper touches the glowing pyramid, goes into a trance, and starts translating the inscription, Jeff Goldblum pulls her off and she doesn't finish it. One of the bad guys says that what Cyndi already said will be enough to help him decode the rest and that it's just a matter of "cartography" now. He should have said "cryptography" (skill of decoding secret writings) and not "cartography" (which has to do with making maps).
