Visible crew/equipment: When the President of U.S. is talking to his Secretary of State in order to replace the judge, the boom mic is visible at the top of the screen for the whole scene.

You can see a section of the wall, about the size of a picture, opening and banging back into the wall. It appears that they intended to hang a picture there and have it banging during the scene, but forgot to hang it up. (01:28:45)
Visible crew/equipment: When Bette Midler and Shelly Long have just found out Michael Saunters is the bad guy, there is a helicopter looking for them and they're standing in the outdoor shower stall. Bette Midler says "They're the good guys" and they run out to try to get the helicopter's attention. Right after they run out of the stall, you can easily see one or two men in the stall - they're either holding the stall up against the wind of the helicopter or they're there for some other unexplainable reason.
Visible crew/equipment: When Bartleby and Loki are walking through the airport terminal, there are several poster frames on the wall in the background. In one of the highly reflective poster frames we can see the camera crew.
Visible crew/equipment: When Mr. Hammond drives up to the school where his daughter died, you can see the reflection of the microphone in the left side of Mr. Hammond's car.
Visible crew/equipment: After Reggie touches the metal poles, he goes flying backwards. As he hit's the floor, you can see a thick white rope attached to his left foot. (01:11:10)

Visible crew/equipment: When Carol-Anne is running away from Kane's voice in the garage, she ends up standing in a puddle, only to have hands pull her in seconds later. But when the camera moves down Carol-Anne's body to the puddle, you can see the platform the actress is standing on.
Visible crew/equipment: When Geena Davis is being dunked in water, you can see an air hose she is breathing out of in one shot. It is when the bad guy is talking, and you see her submerged.
Visible crew/equipment: When the security guard opens and then closes the vehicle door, you can see a crew member's feet.
Visible crew/equipment: Beth attempts to convince Norman to inject himself with a tranquilizer by trapping him in a room and filling it with water. As the water rises, Norman frantically looks for a way out. There is a quick overhead shot of this scene that flashes on for just a second, and if you look around the perimeter of the room - you will see the entire crew, complete with camera and lights.
Suggested correction: This is simply not true. I have just watched that scene and didn't notice it, so I slowed it down to a third speed and still didn't see it.
Visible crew/equipment: When Brendan is talking to V.P. Trueman, a boom microphone is seen moving in the reflection of a picture frame to the left of Trueman.
Visible crew/equipment: When Kathy is bringing in the groceries and she asks George where Chelsea is, they run outside to look for her. As the scene goes along it pans past the truck and you can see the reflection of the camera in the truck.
Visible crew/equipment: When Kobayashi and Edie are meeting, what appears to be a boom mike is visible above Keaton's head when the group is seen through the window.
Visible crew/equipment: When Sean Connery first examines the girl's body, he touches her neck while wearing latex gloves, and rubs his thumb against his fingertip. Then he sniffs his fingertips, and at that moment the large shadow of a mike boom or some other equipment moves across his forehead.
Visible crew/equipment: When Sean Penn is picking out headstones, you can see the tracks and the wheels of the dolly for the camera being reflected in the headstone. (01:30:55)
