Revealing mistake: If a player has a rocket launcher on their back as a secondary weapon, sometimes after getting killed, the death animation has the character throwing his head back. The back of the long rocket launcher tube goes into his head.
Revealing mistake: When fighting Gohma on Dragon Roost Island, when Link uses the grappling hook to grapple Valoo's tail, some of the rope goes through the tail as it wraps around.
Revealing mistake: If you get a slow motion bullet cam shot on a zombie wearing a helmet or hat, and it's a head shot, you can see the hat fly off when the bullet hits, but often you will see the layers of the hat pass through the zombie's head with no collision.

If you press the control button to block, you see the camera man's hands, both arms, come up to block. However the camera he's recording with stays in place. It's a camera that uses VHS so it wouldn't be strapped to his head. He should drop the camera if he lifts both hands like that, but is putting them in front of the camera.
Suggested correction: It's possible it was a non-VHS camera that was transferred to a VHS.
That would make no sense considering the context and situation. This is a 'haunted house' and with this being a Resident Evil game which is a time line set where the end of the world happened in the mid 90's when VHS was still in wide spread use.
Clearly a mistake. For example why would you record something and then transfer it to outdated technology?
Revealing mistake: Marriage Counselling: When Michael walks into his house at the start of the cutscene, keep an eye on the bouquet of white roses, shown in the hallway to the left of shot. In the very first frame, the white roses have been animated very brightly. After that first frame, an animation error occurs on-screen, where the white roses suddenly shift to a much more shadowed white.
Revealing mistake: Intro Mission 2: Lots of the destructible objects in the hospital can be broken simply by just jumping on them. Some of these objects are large plant pots with a small green tree in them. When you jump at them, the foot only has to just touch a leaf of the plant, and it causes the whole vase to shatter.
Revealing mistake: When Clayface disguised as Joker is holding Talia hostage in the movie theater, they are both standing on stage in front of the screen with a movie playing. However, they are not casting shadows on the screen. Also, no shadow appears on the screen when Clayface takes on his true form.
