Ye Gods (season 1, episode 13)

Mistake Continuity: At the cocktail party, Ettinger and Bacchus are sitting with their shoulders touching in close-ups, but they're more than a foot apart in all the full shots.

The Beacon (season 1, episode 26)

Mistake Plot hole: Barrows introduces himself as "Dr. Dennis Barrows," and Teddy asks, "What's a doctor?" As the isolated villagers have never heard of doctors, Teddy would have no way to know that "Doctor" was a title/profession and not simply part of Barrows' name. He should have asked, "What kind of a name is Doctor?"

The Little People of Killany Woods (season 1, episode 34)

Mistake Visible crew/equipment: When it's opened for Mulvaney, the hardware store's glass-paned door reflects several large, round studio lights.

To See the Invisible Man (season 1, episode 40)

Mistake Continuity: In the cafeteria, the "invisibility" mark on Mitchell's forehead has moved to a higher spot than it occupied in previous scenes.

Mistake Visible crew/equipment: When the woman ignores Mitchell and rushes into her apartment building, the glass door she opens reflects the camera, crew and studio lights.

Welcome to Winfield (season 1, episode 42)

Mistake Other: Veteran actor Elisha Cook's name is misspelled "Elisa Cook" in the opening credits.

Aqua Vita (season 2, episode 4)

Mistake Continuity: The water level in the dispenser rises and falls by itself several times throughout the episode.

The Convict's Piano (season 2, episode 11)

Mistake Revealing: In the close-up of Frost's hand while the doctor is stitching up his wound, you can see the delineated edges of the latex make-up patch used to simulate the injury.

The Card (season 2, episode 13)

Mistake Revealing: When Linda pulls the card out of her wallet in close-up, it's twice the length of an average credit card so that we can read the name printed on it. In the next shot, though, it shrinks back to standard size.

The Junction (season 2, episode 14)

Mistake Continuity: In 1912, Ray writes his letter in pencil. When it's delivered to Melissa in 1986, it's written in black ink.

Time and Teresa Golowitz (season 2, episode 18)

Mistake Continuity: As they talk, Prince and the pianist stand beside the apartment wall that's covered with Broadway show posters. There's no break in their conversation, but when the shot cuts, they're across the room in front of the window.

Song of the Younger World (season 2, episode 20)

Mistake Factual error: It's the 1800s, but Amy's skirt has a 20th Century zipper running down the back.

Mistake Factual error: The 19th Century reformatory has modern electric light bulbs hanging from the corridor ceilings.

The Trunk (season 3, episode 14)

Mistake Continuity: Willy pulls one bottle of champagne from the full case and gives it to Mrs. Cooper. In the next shot, as he heads down the stairs with the wine, the case is full again.

Something in the Walls (season 3, episode 19)

Mistake Continuity: When the psychiatrist plays back his tape of Sharon's counseling session, the conversation we hear differs noticeably from the one we heard in the actual counseling scene.

A Game of Pool (season 3, episode 20)

Mistake Factual error: Twice during shots of the tournament in progress, we see the 8-ball going into a corner pocket - a mistake that should have ended the game.

Mistake Continuity: During their game, Jesse points at Fats Brown and brags about being the best. When the camera angle reverses, however, Jesse is no longer pointing: both of his hands are on his pool cue instead.

Mistake Continuity: Jesse's right shirt tail alternates between tucked in and not tucked in, throughout the game.

Room 2426 (season 3, episode 22)

Mistake Audio problem: When Decker and the professor walk across the college campus, there's a badly-looped bit of dialogue that causes the professor's line to continue after his lips have stopped moving.

The Mind of Simon Foster (season 3, episode 23)

Mistake Continuity: When the pawnbroker closes his metal gate, he's facing the wire mesh. When the shot cuts, he's instantly facing away from it.
