Carry Me Back to Old Tsing-Tao (season 1, episode 3)
Continuity: As they speak to the old man in the cottage, Kelly's hands alternate from being hidden below the table's edge to resting on top of the table several times between takes.
Continuity: The three bad guys are standing over Scott and Kelly after beating them up and stealing the gold. When the shot cuts to an alternate angle, all three of them instantly change positions.
Audio problem: During Scott and Kelly's conversation at the bistro table, so much equipment noise is picked up by the mike that portions of the dialog are drowned out.
The Loser (season 1, episode 6)
Continuity: During the interrogation, Manolo's coat sleeve is torn at the seam. But when he leaves the room, the sleeve has no tear.
The Time of the Knife (season 1, episode 8)
Continuity: After she's been roughed up by the bad guy, Jean's hair is a mess. But while she's talking to Scott and Kelly, her hairdo repairs itself between takes, becoming neatly combed, sprayed and coiffed again.
No Exchange on Damaged Merchandise (season 1, episode 9)
Visible crew/equipment: During the opening sequence, the bad guys' red Ford Galaxy reflects a bank of studio lights when the door is opened.
The Barter (season 1, episode 16)
Audio problem: When Scott tells the Tokyo police that two kidnappers are locked in the van, we hear his voice, but his lips aren't moving.
Return to Glory (season 1, episode 21)
Continuity: Kelly puts Scott's red sweater on a stick dummy to fool the sniper, who promptly shoots a hole through it. But after Scott puts the sweater back on, the bullet hole completely disappears.

Continuity: During Scott's conference room briefing, one of the government officials instantly changes seats between shots.
Revealing: During Scott and Kelly's scuffle in the restaurant, the stunt men's faces are visible several times.
One Thousand Fine (season 1, episode 28)
Continuity: Throughout his long trek through the jungle, the stains on Kelly's t-shirt change position, disappear and reappear repeatedly between shots.
So Coldly Sweet (season 2, episode 1)
Factual error: An American spy in the process of bugging a hotel room is stabbed by a Russian spy. But immediately afterward, as well as later when he's found, there is no blood visible on his white shirt or on the carpeting.
Lori (season 2, episode 2)
Revealing: In the final chase scene, the substitution of a stunt double is noticeable when Malachi Throne's character suddenly acquires considerably more hair.
To Florence, with Love (1) (season 2, episode 11)
Continuity: Maria the chamber maid is sitting down when Scott and Kelly start questioning her. But when the shot cuts, she's suddenly standing up.
Continuity: At the Florence train station, the luggage on the cart that surrounds the painting changes positions between shots, even though no one's touched it.
Rome ... Take Away Three (season 2, episode 16)
Audio problem: Scott and Kelly are talking as they wait to cross a Rome street - but Kelly's lips are completely out of sync with his voice.
Continuity: At the end, an overhead shot of Scott and Kelly shows them at the bistro table seated more than a foot apart. When we cut to a ground level shot, however, they're shoulder-to-shoulder.
Revealing: Kelly finds Tilde dead on her couch. But when he touches her, the "dead" woman's eyelids twitch.
Tonia (season 2, episode 17)
Visible crew/equipment: At the end, when Scott and Kelly leave the corridor, the microphone shadow moves across the front of Scott's sweatshirt.
Continuity: Scott enters the cantina wearing a yellow sweater. When he rescues Tonia from the bar brawl, his sweater is blue. In the next shot, it's yellow again.
The War Lord (season 2, episode 20)
Continuity: In the closing scene aboard the raft, we see Kelly with an unlit cigarette in his mouth. One shot later, the cigarette is not only lit, it's half gone.
Factual error: She's just been drugged and then hauled through a very dusty ancient temple on a harrowing chase - yet Katherine emerges at the end with her white clothes absolutely spotless.
A Room with a Rack (season 2, episode 21)
Continuity: In the dungeon, Scott holds the conquistador helmet in front of him with both hands. The shot cuts, and it has abruptly shifted to his right hand. In the next shot after that, the helmet is in his left hand.
Mainly on the Plains (season 2, episode 22)
Continuity: While they sit side-by-side on the park bench, Don Silvando places his hand on Kelly's shoulder. The camera angle reverses, and Silvando's hand is immediately in his lap - and Kelly's shoulder, which should still be in frame, has disappeared.
Continuity: The land rover Kelly and Scott are driving through the back country is covered with dirt and dust. But during their drive, it somehow washes itself completely clean.
Audio problem: The stick joust is supposed to occur in the middle of the Spanish countryside far from civilization - yet we can hear motors running in the background.
Get Thee to a Nunnery (season 2, episode 23)
Visible crew/equipment: When Kelly and Scott get out of the taxi at the castle, the entire camera crew and lighting array can be seen reflected in the car's doors.
Night Train to Madrid (season 2, episode 26)
Continuity: When everyone boards it at the station, the entire train is steel-gray in color. In a later shot of it en route to Madrid, however, every car is bright red.
Continuity: In the opening train compartment scene, two consecutive shots don't match. In the first, Scott is sitting back with both hands in his lap. In the next, he's suddenly leaning forward and has one hand on his cheek.
The Seventh Captain (season 3, episode 9)
Revealing: During his scuffle with the intruder, Kelly's stunt double's face is visible in several shots.
Continuity: Scott is wounded in the right ankle during an underwater fight. But in the hotel room afterward, he's bandaging his left leg.
This Guy Smith (season 3, episode 18)
Visible crew/equipment: As Kelly is getting dressed in the hospital room, someone's shadow strolls across the floor in front of him. Scott is standing still, and no one else is there, so the shadow must belong to a film crew member.
Continuity: In the hospital, Kelly's pajama top is open in most shots, showing the bandages across his ribs. But the pjs are closed and covering his chest completely in other shots.
Turnabout for Traitors (season 3, episode 19)
Continuity: The large black oil stain that Kelly's shirt acquires during the brawl in the alley completely disappears a short time later.
Happy Birthday... Everybody (season 3, episode 20)
Plot hole: After a chance meeting in Mexico, Tom Matthews invites Kelly and Scott to his home for dinner. The address is unlisted, neither knows where the other lives or is staying, and no address or phone information is exchanged before they part. But Kelly and Scott somehow find the house anyway.
Carmelita Is One of Us (season 3, episode 25)
Continuity: In full shots of Kelly holding the baby, it is entirely covered by the pink blanket. But in close-ups, the blanket is pulled back to completely expose the baby's head.
Revealing: When Scott picks up the baby and carries it out the door, the face of the plastic doll doubling for the real infant is visible.
Continuity: Chica carries the baby downstairs to the foyer, puts it in a blue pram and leaves for town. When Kelly and Scott arrive at the apartment building, the pram Chica has just left with is back in its place in the foyer.
Pinwheel (season 3, episode 26)
Continuity: When Wellington drags Melanie into the room where the Chinese buyer waits, he's holding her by the elbow. But when the shot cuts to a wider angle, her arm is instantly down at her side instead, and Wellington is standing apart, no longer touching her at all.
