Continuity: Season 3, Episode 2 "The Box" - After Folivia shoots Joe and is dragging him to the bathroom to dispose of the body, she hears a knock on the door. During this shot, she is dragging him with his jacket over his head, which she is gripping. However in the immediate next shot, she is still dragging him by his jacket, only the jacket is much lower.

Continuity: In Episode 3-14 - 6B When Walter is using the hammer on the block of ice, the pattern that results is different when viewed from each side.

Visible crew/equipment: "Over There, Part 2": When Walter and Olivia leave the hospital in the alternate universe, William Bell pulls up in his vehicle. As he does, you can see the reflection of the crew on the metal bars in the front of the vehicle, as well as the metal side-view mirror.

Factual error: When the 747-aircraft of the fake German airline is being shown, you can see the German flag on the plane. But you also see an "N-xxxxx" registration which stands for USA. If it were a real German plane, it would have a "D-xxxx" registration.

Plot hole: When Dr. Bishop is using the special camera to capture the images on the dead woman's retina, he tells everyone to put on their goggles to protect them from the flashes. Once they find the image they need, everyone takes off their goggles, yet the camera is still flashing, and there are no ill effects.

Plot hole: When Charlie looks up the file on David Esterbrook, he says that the file shows that Esterbrook is directly responsible for INtREPUS' "controversial" R&D, including pre-natal gene therapy, human/animal hybridization, and viral warfare. Yet later, Olivia says she can find no dirt on Esterbrook who is, in her words, "cleaner than snow". If INtREPUS kept records of their illegal endeavours, then how come Olivia can't find them when the time calls for it?

Deliberate "mistake": Dunham comments that she works for Homeland Security. While the FBI is under the jurisdiction of the Justice Department, she works as part of a special Homeland Security task force; so saying she works for DHS is an accurate statement.
