Mr. Big (season 1, episode 1)

Mistake Continuity: When Max is phoning the Chief to let him know the mission's been accomplished, he takes the oars out of the oarlocks before opening his shoephone. The oars change from being out of the oarlocks in the closeups to still being in the water in the long shots for the rest of the episode.

Mistake Continuity: When the owner of the shop is vaporized, at first his hands are both in front of him. The shot changes and now one arm is instantly thrown back behind him.

Mistake Continuity: When Smart and 99 meet Mr Big for the first time, Max stops several feet from Mr. Big. The shot changes and now Smart is instantly quite a bit closer to Mr. Big.

Mistake Continuity: When Max is attacking the boat there is a shot from inside the cabin. Watch the door. Before Max shoots, there are a series of holes in the shade covering the door (they look like bullet holes). Max begins firing and there is another shot from inside and bullet holes begin appearing in the walls. The holes in the shade however, are gone.
