Mistake Visible crew/equipment: In the episode when Bruce is working in the arcade, when the two old men drive off to kill the man in the other car, the entire cast and crew, as well as the camera and camera man are reflected in the car.

Mistake Revealing: In the episode "Prometheis", when the Prometheis group is viewing the video of the Hulk destroying the helicopter, it is obvious the footage used is the same from when we see the Hulk destroying the helicopter. The video uses camera angles exactly like those used when the explosion actually occurs, and camera angles not avalible to the other helicopters.

Death in the Family (season 1, episode 2)

Mistake Continuity: When Hulk throws a tree root at the helicopter, it smashes off the tail rotor. In the next couple shots, as the helicopter is spiralling down, we see the rotor is intact.

Terror in Times Square (season 1, episode 6)

Mistake Visible crew/equipment: When the two old guys drive off to kill the other man near the end, the entire crew, the camera and camera man are visible on the car doors.

Never Give a Trucker an Even Break (season 1, episode 9)

Mistake Continuity: There is a scene where the female trucker is driving her truck and she intentionally hits a brown car. The problem is that she had already passed by this brown car a few seconds ago when David was getting into her truck through the window.

Mistake Continuity: When David is in the telephone booth trying to call the police he mentions that he can only read the first few digits of the phone number. Then there is a shot of the center of the dial which is almost all black and we cannot read any digit. A few seconds later another shot shows the center of the dial which is now white and we are able to read the first few digits.

The Waterfront Story (season 1, episode 12)

Mistake Revealing: Near the end of the episode, after the Hulk jumps from the wooden crate, he walks around barefoot. But right after the pickup hits the car, we can see that the Hulk is wearing green shoes to protect his feet.

Another Path (season 2, episode 7)

Mistake Continuity: When the old Asian lady is hitting the Hulk using a wooden spoon, he is wearing brown pants. After escaping through the window, the Hulk is seen running on the street wearing blue jeans. But for the rest of his transformation he is wearing brown pants again.

Killer Instinct (season 2, episode 9)

Mistake Revealing: When the Hulk is inside the steam room and he throws his fists through the door, he loses fairly big patches of green make-up on both forearms in the process. (There is no CGI used in this 70s era TV show.)

Escape from Los Santos (season 2, episode 11)

Mistake Continuity: During his second transformation, David tears his white shirt. When the Hulk is pulling Holly out of the cliff, the white shirt is nowhere to be seen. But when he takes her in his arms, he is wearing the torn white shirt.

Wildfire (season 2, episode 12)

Mistake Revealing: When Adler fights with David close to the detonator just before his second transformation, the shots are filmed too close. It makes it really obvious that a stuntman is replacing David for most of the fight.

Mistake Continuity: When Mike is on his balcony with Linda, we see that he is talking to 12 workers. But, in the next shot, a 13th worker wearing an orange hat and an olive green jacket magically appears almost completely to the right. However, when the 12 workers leave the area, the 13th worker cannot be seen anywhere.

Mistake Continuity: When Adler moves David into the barrel and loads him into the truck before his first transformation, David's head is at the bottom of the barrel. But when the Hulk emerges from the barrel he is already standing on his feet.

A Solitary Place (season 2, episode 13)

Mistake Continuity: When the Hulk throws Mr. Malone in the river during his second transformation, the first shot is too long and we see that he would fall a few feet from the closest river shore. But in the second shot he is flying like crazy and almost lands on the remote river shore.

Mistake Continuity: When David is leaving at the end, he is wearing a blue shirt under his coat. Moments later, he removes his coat but he is wearing a different color shirt with a square motif.

Mistake Continuity: When the Hulk throws the tire during his second transformation, the first shot shows a horizontal trajectory but the second shot shows a 45 degree angle trajectory, which makes the tire land much further than expected.

The Disciple (season 2, episode 18)

Mistake Continuity: After the Hulk breaks the wall to escape at the end of his second transformation, he is wearing black pants instead of his usual beige pants.

Mistake Revealing: When the Hulk is lifting his left foot at the beginning of his first transformation to break free from the "leg shackle", there is a close shot where we should see his toes. But we cannot see them because he is wearing some green pantyhose with brown soles to protect his feet.

Kindered Spirits (season 2, episode 20)

Mistake Continuity: Before his first transformation, David is wearing a white and blue striped shirt with blue jeans. During his transformation he is wearing a red and blue square motif shirt with brown pants. When fully transformed into the Hulk, he is wearing blue jeans again. But when he's removing the rocks to free the professor seconds later, he is wearing a different kind of brown pants. Finally he's wearing blue jeans again for the rest of this first transformation.

The Confession (season 2, episode 21)

Mistake Revealing: When the Hulk throws the large bell to break the tower's wall during his second transformation, we can see three cables attached to the bell, most likely used to guide its trajectory.

The Quiet Room (season 2, episode 22)

Mistake Continuity: While Dr. Morrow is saying, "I did some preliminary tests on David. The limbic system is definitely affected," he is holding a folder containing patient information and a picture. In the next shot the folder contains a different picture with no time to have turned any pages.

Mistake Revealing: During the first transformation, just before the Hulk breaks the wall, we can see the green shoes he is wearing to protect his feet.

Mistake Continuity: When David is on the hospital bed before his first transformation, he is wearing beige pants. During his transformation he is wearing light blue pants, but when fully transformed into the Hulk he is wearing beige pants again.

Vendetta Road (season 2, episode 23)

Mistake Continuity: After David's second transformation, we can see the dynamite sticks on the floor, tied with a piece of black tape at the top and another piece of tape much further at the bottom. When the Hulk takes the tied dynamite sticks in his hands, a third piece of black tape appears close to the existing piece on the top of the sticks.

Blind Rage (season 3, episode 2)

Mistake Revealing: During his first transformation, the Hulk gets behind two soldiers and smiles twice. His second smile is too broad and reveals a very shiny silver tooth filling on his right lower jaw. David does have fillings but they do not cover the external area of the teeth (Ref: when he hugs Katie at the end of the 2nd part of Prometheus, right after his last transformation).

Brain Child (season 3, episode 3)

Mistake Continuity: When David leaves at the end he's wearing beige pants, but in the next shot he's wearing dark pants.

Prometheus (1) (season 4, episode 1)

Mistake Revealing: When the crew at Prometheus are viewing the tape of Hulk destroying the helicopter, it is obvious the tape is just stock footage from the scene earlier when the Hulk actually does destroy the helicopter. The camera angles used are exactly the same, and they are also angles the other crew wouldn't be able to film from.
