Visible crew/equipment: When Johnny follows the nurse as she gets off the elevator on the fourth floor, the outlines of the mic transmitter and wire are visible on the back of the nurse's white uniform.
Continuity mistake: When John and Roy are at the park with the injured football player and his son, the shadows that are cast over the entire area change significantly when the ambulance arrives.
Visible crew/equipment: While Squad 51 is parked at ER receiving, John and Roy tell Dr. Early about the squad's engine problem, and when Early fixes the engine problem, we can see the reflection of numerous set lighting on the squad's chrome spotlight.
Other mistake: During the fire at Hammond Chemical, when Engine 7 rolls up and lays duals, we can see the vinyl material adhered to the top of the cab covering the engine company's real number.
Loose Ends - S6-E12
Visible crew/equipment: During the fire at the train yard, when they carry out the second toxic fume inhalation victim from the boxcar, just before they pass the camera, the shadow of the camera tracking backwards is visible on the boxcar, to the left.
Paper Work - S6-E11
Other mistake: When Marco is taking inventory of what's in the refrigerator, the containers on the door have their brand names covered.
Visible crew/equipment: When Station 51 is being dispatched to 'auto accident with injuries' there's a closeup of Sam Lanier, the dispatcher, and in the next overhead shot of the apparatus bay, we can see the reflection/shadow of equipment moving on Engine 51's windshield, at the bottom left corner.
Continuity mistake: While Roy and John are with the man whose back went out, when Thadeus runs off with the BP cuff and Roy chases him, Roy has the stethoscope around his neck with the chest piece hanging down, but once Roy falls into the pool the chest piece is suddenly tucked down into the front of Roy's shirt.
Captain Hook - S6-E8
Visible crew/equipment: When Roy, John, and Chet are standing near the stove talking about Hookrader's party, as they walk towards the table, the shadow of the overhead boom mic is moving over the cabinets on the right.
Visible crew/equipment: After John and Roy rescue the elderly man from the burning house, while Roy gets the man's vitals in the closeups, the large 12-light par bank rig is visible behind Roy at the left of the screen.
Continuity mistake: When the guys are listening to the baseball game on the radio, dispatch drops tones for Engine 16 and only Squad 51, yet Chet turns the radio off, and all the engine company guys rush out of the day room then head to their rig, even though it's only Roy and John that were dispatched.
Other mistake: After Station 51 is dispatched to the structure fire, just as Roy and Johnny start heading upstairs we see footage from 4x3, "Gossip," when Johnny brings the hose to the stairs at the chemical fire, but the line doesn't reach so he leaves the hose at the bottom step, and they continue up.
Camera Bug - S4-E10
Other mistake: While Roy and John are at 68's, Roy goes to the stove to see what Bob was cooking when he started having pain, and the closeup footage we see of the pot of bubbling chilli is the same as some of the footage of Chet's chilli in 4x1, "The Screenwriter," when 'Miss October' stops by the station.
Other mistake: During the rescue at the top of the open drawbridge, in the shots of Engine 2 we can see the red, vinyl material with the company number imprinted on it, adhered to the front of the engine.
Visible crew/equipment: When Brackett, Early, and Morton enter treatment 1 to examine the hallucinating veteran, inside there's a young doctor standing to the left of the door. After Brackett tells Morton he wants an EEG, skull x-rays, and brain scan, when Morton heads to the phone to order the neurological workup, we can see the actor's white tape mark under the gurney, where the young doctor had been standing.
Visible crew/equipment: After the ambulance arrives at the lion pit, just as the attendants reach the fence where John and Roy are standing, and the stokes is loaded onto the stretcher, we can see the two white tape marks on the ground, where the zoo employee is standing.
Visible crew/equipment: When we hear Station 51 being dispatched to the structure fire at Kiddytime Toy Factory, just as Roy rushes to the squad's door to open it, we can see the reflection of the camera dolly through the squad's windshield as it moves.
Other mistake: During the paint factory fire, when Engine 2 rolls up, we can see the red, large, rectangular vinyl material that is adhered to the front of the engine, with the company number printed on it. There's also vinyl material adhered to the top of the cab as well as the back of the engine with nothing on it, presumably covering the engine company's real number.
Other mistake: When Squad 51 is dispatched to the possible heart attack, while Chet’s at the base station responding to the dispatcher, the lightbar atop Squad 51 has nothing covering the chrome, but en route in the close up there's black tape covering much of the chrome trim.
The Promise - S3-E11
Other mistake: At the very end, when Roy is cleaning the windshield while John's fiddling with the squad’s radio mic, the spray can on the hood of the squad has tape covering the brand name.
Answer: It is standard practice to wet down driveways so that they stand out in a long shot.
