Season 2 Episode 1 "The Work Outing" - When Roy is in the disabled toilet, it shows that the door opens outwards. However, the staff manage to kick the door down inwards. this wouldn't be possible. [It's a swinging door. Absurd as it sounds, it used to be a fire regulation that toilet doors open inwards - since this was impossible in practice, a lot of old buildings (like the theatre) have swinging doors on their dunnies.]

A fire extinguisher would not catch fire, then simply burn down slowly. Being a pressurised container, it would have exploded within a minute. [This is less of a mistake, and more a deliberate effort as part of a running joke that episode in which Moss distrusts the functionality of anything made in Britain. The fact that it doesn't even behave correctly when ignited adds to the joke.]
