Meat (season 2, episode 4)

Mistake Continuity: While he's in the van with Jack, Reese's seat belt shoulder strap vanishes and reappears multiple times as the shots change.

Ghost Machine (season 1, episode 3)

Mistake Continuity: When she sees the little boy's ghost, Gwen's hair alternates repeatedly between neatly-combed and windblown/messy, depending on the shot.

Mistake Factual error: 3-1 Children of Earth, Day Three: Frobisher tells the 456 he is the Permanent Secretary of the Home Office of the "United Kingdom and Northern Ireland". It should be the United Kingdom of Great Britain and Northern Ireland. Northern Ireland is part of the UK.

Mistake Factual error: 3-1 Children of Earth, Day Three: Andrew Staines was a junior Scots Guards officer in the 1960s according to the Torchwood database. However, the photograph of him in that era shows him with a general's capbadge.

Captain Jack Harkness (season 1, episode 12)

Mistake Factual error: "Captain" Jack Harkness actually wears the rank insignia of a group captain, equivalent to an army colonel and never abbreviated to "captain". In fact, the RAF does not have a rank of captain (the equivalent rank is flight lieutenant). Neither does it have the position of "squadron captain", as he is described - a squadron commander in World War II usually held the rank of squadron leader (equivalent to an army major).

Mistake Factual error: 3-1 Children of Earth, Day One: Lois says that Colonel Aduya's uniform "isn't British Army". In fact, it is entirely (and accurately) British Army with the exception of the small UNIT badge over the pocket.

Mistake Factual error: 3-1 Children of Earth, Day One: On the government database, Torchwood is described as being founded by HRH Queen Victoria. As a sovereign, she was actually HM (Her Majesty) not HRH (Her Royal Highness).

A Day In The Death (season 2, episode 8)

Mistake Continuity: When the old man dies and Owen is trying to revive him, the position of the man's head changes several times between takes, from lying flat to propped up on the pillow and back again.

Mistake Continuity: When Maggie walks away from the car accident, the blood smears on her white gown change repeatedly from shot to shot.

Captain Jack Harkness (season 1, episode 12)

Mistake Factual error: When Gwen finds Toshiko's hidden message from 1941, she realizes that it was written in blood, and her flashlight reveals the bright red letters on the now-yellowed piece of paper. But after 60+ years, the iron in the blood would oxidize. The writing on the note should be rust-brown, not red.

Combat (season 1, episode 11)

Mistake Factual error: Between getting disconnected by the bad guy and calling Ianto, Jack gets a dial tone on the dead man's cell phone. Cell phones don't generate dial tones.

Greeks Bearing Gifts (season 1, episode 7)

Mistake Continuity: While she's talking to Mary in the flat, Toshiko's hair keeps changing between shots, going from combed evenly to split apart with one section hanging in the middle of her forehead and the rest brushed back.

Day One (season 1, episode 2)

Mistake Continuity: In the opening scene, Gwen has a long, loose strand of hair hanging over her left temple - but every time the camera angle changes, it disappears.

To The Last Man (season 2, episode 3)

Mistake Continuity: In the scene on the pier with Toshiko and Tommy, the soldier from WWI, Toshiko is holding her hand bag but drops it when he picks her up. Immediately, the next shot she is holding it again.

Something Borrowed (season 2, episode 9)

Mistake Factual error: Jack calls Gwen, and when Reese takes the phone and hangs up on him, Jack's cell phone, for the second time in the series, sounds an audible, impossible dial tone.

Out Of Time (season 1, episode 10)

Mistake Continuity: John is about to eat liver and potato with Emma before she storms out saying, "I don't like liver". While they are talking, the potato is facing Emma and Gwen. After Emma leaves, the liver faces Gwen and the spot where Emma was.
