Across whole show
Plot hole: Season 4, episode 5 "Hysterical Blindness": Peter "catches" Emma's ability and loses his speed ability when he touches her, to keep her from being hit by the bus. Yet in one of the previous episodes, he shook hands with Samuel without losing his ability and gaining Samuel's.
Continuity: In episode 211 "Powerless", right before Sylar shoot Maya, you can see she already has a bullet wound on her chest.
Continuity: In episode 211 "Powerless", Maya's necklace disappears between shots when she accuses Sylar of killing her brother.
Factual error: In Season 2, Episode 3, Peter and the Irish family rob the bookmakers and the security guards shoot at them. In Ireland, even the police don't carry weapons. let alone do security guards.
Continuity: "Our Father". At the end of the previous episode, "The Eclipse, Part 2", Hiro brought Claire to the rooftop of the Deveaux building to witness Kaito handing over baby Claire to Noah Bennet. To illustrate his point, Hiro showed Claire the exact same thing happening in the "9th wonder" comic book he brought with him, and she can follow the Japanese dialogue from there. But when the exact same scene is shown at the beginning of this episode, Hiro does not show Claire the comic at all.
Continuity: Season 2, ep. 3, "Kindred". When Peter gets the box with his belongings back from the Irishmen, as Caitlin says "Well?" Peter's hands are in his lap. The angle changes to Peter, who answers "Well, what?" and he suddenly has both hands on the box, with no time to move them between shots.
Visible crew/equipment: S02E11 - "Powerless" When Mohinder, Sylar, Maya and Molly are at Isaac's old flat (now Mohinder's lab), as Mohinder and Sylar walks down the stairs, you can see the moving camera crane reflected in the glass behind them.
All of series 1 (season 1)
Plot hole: In some episodes Claire voluntarily hurts herself (like when she grabs a pan out of a hot oven or when she sticks her fingers down the sink disposal to grab something). She is obviously badly injured these two times but shows absolutely no signs of pain whatsoever. Later in season 1 when she tries to do the back tuck off the tower she breaks her leg and is in agonizing pain. Also Peter has this power too, and he shows signs of pain a couple of times throughout the season. Do they have pain or not?
Continuity: Season one takes place over five weeks. Janice Parkman goes from not being visibly pregnant to being at least five or six months along during the course of those five weeks.
Continuity: Episode One, when Claire is talking to her mom, right after she sticks her hand in the garbage disposal, her ponytail goes from being over her shoulder to behind it, even though Claire never moves.
Don't Look Back (season 1, episode 2)
Revealing: Claire Bennett is accidentally tackled when she and Zach are arguing while watching the football game. Apparently, her neck is badly broken in the fall, even twisted a full 180 degrees. However, if you watch closely as she fall, you can see that she is completely fine, stretching out her arms to take the impact before rolling around. She lands on her front, arms stretched out, all in all well prepared for the landing, and rolls around. In the next shot her neck is twisted in a way that simply wouldn't be possible in such a fall. She would have had to have her head stuck under something before rolling over, which she certainly did not. The guy tackling her did not land on top of her either.
Deliberate "mistake": The newspaper that shows Petrelli's victory has repeated paragraphs through the article. The article seems to be composed of three paragraphs. One paragraph starts with "I was," and two paragraphs start with "The."
One Giant Leap (season 1, episode 3)
Factual error: When they show the comic book and Hiro looks up the artist's address, it is listed as 215 Reed St. 7. The door to the artist's loft clearly says his address is 861.
Continuity: After throwing the laptop, when Mohinder takes the key out of the book, the tape is bunched up and stuck to the key, but in the next shot the tape is fresh and sticking out freely.
Factual error: During Hiro's journey around New York, a Carl's Jr. restaurant is visible in the background. That restaurant isn't called "Carl's Jr." on the East coast; it's called "Hardee's."
Seven Minutes to Midnight (season 1, episode 8)
Continuity: When Mohinder is back in India, his mother tells him that his sister Shanti died at the age of 5, when Mohinder himself was 2. In "The Hard Part", Mohinder tells Molly Walker that Shanti died before he was born, and shows her a photo of Shanti (who looks 5 years old) and his pregnant mother.
Fallout (season 1, episode 11)
Continuity: When Peter is having his vision of New York, D.L. can be seen picking up Micah and running away. When the camera pans out to show Matt holding them back, D.L. is picking up Micah again.
The Fix (season 1, episode 13)
Revealing: The money that Micah dumps out of his backpack is very obviously fake. In fact, the $20 bills have Benjamin Franklin's portrait on them.
Distractions (season 1, episode 14)
Continuity: In the scene where Sylar and Mrs. Bennet begin to talk, she becomes suspicious about his interest in Claire. When Mrs. Bennet picks up the phone to call her husband, Sylar visibly moves his finger to use his telekinetic power right before the angle changes. In the next shot, Sylar again does the same motion with his finger, but this time the phone is removed from Mrs. Bennet's hand.
Continuity: When Peter and Claude are invisible and walking in New York, just after Claude has released his hold on the T-shirt rack, they bump into a man in a coat. Cut to a different angle, showing the two on camera, and the man in the coat is gone. Instead there are two girls and a man with glasses walking directly behind Peter and Claude, none of whom were there a split second ago.
Run! (season 1, episode 15)
Continuity: When Matt Parkman gets thrown out of a window by Niki Sanders, he lands at least a few stories down and has a cut upon his head. The amount of blood on his head differs significantly from shot to shot.
Continuity: When Meredith and Nathan are talking in her trailer, as he says, "I could have been a much better person back then", he has his sunglasses in his hand and places that hand on the table. Cut to a shot from behind him, and his hand is resting on the table with the sunglasses beside it, with no time to put them down and move his hand in between the shots. Also, when Meredith says, "You and me, we were never gonna work out", his hand goes from being on the table to his lap between shots, once again without having had the time to move it.
Continuity: At the end of ep. 1-14, "Distractions", Meredith calls Nathan to tell him about Claire. In that episode he is holding the phone to his ear, with nothing else in it. In the beginning of this episode, when the same conversation is shown, he is holding a pen in the same hand as the phone. Also, in ep. 14 Meredith said to him, "We haven't spoken in a long time, but... Our daughter just found me". In this episode, she says, "We haven't spoken in a long time, but I just had... Our daughter just found me".
Continuity: In ep. 1-14 "Distractions", when Claire asks Meredith about her father, Meredith answers, "Well, that's complicated. Why don't you let me work on that for next time?" But when the same scene is shown in a flashback in this episode, Meredith says, "Well, that's complicated. He's kind of a big shot".
Company Man (season 1, episode 17)
Continuity: At the end of this episode, Noah is having a flashback. It shows Claire helping him pick out his new glasses. In this scene, Noah first tells Claire that she is adopted. However, in the episode "One Giant Leap", Claire tells Brody Mitchell that her parents told her that she was adopted when she was really little. In "Company Man" she is around 14 or 15 years of age in the flashback.
Continuity: When Claire gets killed she is wearing reddish brown boots, with her jeans tucked into them. She's still wearing them when her body is placed on the bed. A few moments later, running from the house with her mother, she's wearing sneakers.
.07% (season 1, episode 19)
Revealing: When Mrs. Petrelli is sitting with the dead Peter, just before she lies down on his chest and starts to cry, you can see the "dead" man tilt his head slightly backwards.
Continuity: When Sylar comes for Isaac the painting of Hiro and the dinosaur is visible in the studio in several shots, even though Hiro took this painting to Linderman several episodes ago. The episode director even comments on it during the audio commentaries.
The Hard Part (season 1, episode 21)
Continuity: In the scene where Mr. Bennet, Matt Parkman and Ted Sprague are at the used car lot, Ted is using his powers to defrost a blue station wagon. As the frost is melting you see "Automatic" written on the windshield. The shot cuts to Bennet and Matt talking by the pay phone. When the Shot returns to Ted, he's finished defrosting, but you see "Low Mileage! 1 Owner" written on the windshield instead. Ted didn't switch cars during the conversation because written on the used car beside the blue wagon is "Like New" for both shots.
Landslide (season 1, episode 22)
Continuity: This mistake is repeated in the next episode too. After D.L kills Linderman he is lying face down, his face is flat against the floor and we can see right into the hole in is head, he has already stopped moving. Then after the camera starts to pan around, Linderman's head is now facing right, and we get a good look at his face.
How to Stop an Exploding Man (season 1, episode 23)
Continuity: Hiro's sword changes position between the shot in which Hiro arrives to the scene and the shot when he is about to kill Sylar.
Deliberate "mistake": At the plaza, Peter tells Claire to shoot him because there is "no other way". Nathan then comes flying in and declares, "Yes, there is, Claire!" However, Nathan could not have heard Peter and Claire from the distance he flew in and the rushing of air past his ears.
Continuity: When Parkman is shooting at Sylar in Kirby Plaza, he fires four shots. But when we see the bullets hovering in the air before Sylar's hand, just before he flings the bullets back toward Parkman, there are five of them.
(Volume Two: Generations) Four Months Later... (season 2, episode 1)
Continuity: S02E01 "Four Months Later". When Claire hugs Mr. Bennet she is holding her note in her right hand. In the next shot as she turns around and runs off to school the note is in her left hand.
Revealing: After Kensei knocks Hiro down a goat licks Hiro's face. The kibble used to lure the goat in for the shot can be seen on Hiro's cheek and collar.
Lizards (season 2, episode 2)
Continuity: Season 2, ep. 2, "Lizards". When Alejandro finds Maya outside the tunnel, she has black ooze from her eyes running down her cheeks. As he starts his healing, we get a close-up shot of how the ooze fades and disappears from Maya's face. But in the next shot of her face, when she falls forward, the black stuff is back on her cheeks. It's gone again in the next shot of her.
Kindred (season 2, episode 3)
Continuity: When Claire and West are talking about their abilities on the beach, the close up shots of Claire show that she is wearing earrings, the camera goes to West but when it focuses on Claire again she is no longer wearing them.
Four Months Ago... (season 2, episode 8)
Continuity: Season 2, episode 8, "Four Months Ago". When Maya is arrested, she starts manifesting her powers on the policeman. In a close-up, we can see the black ooze coming out of her eyes. Cut to a shot from above, when the policeman's eyes turn black and he drops the handcuffs, and Maya's cheeks are suddenly clean again, with no trace of the black stuff.
Cautionary Tales (season 2, episode 9)
Visible crew/equipment: S02E09 - When Claire runs away from Bob after he calls her "Ms. Bennet", you can see Bob's shadow and another person's shadow on the ground. Since no one else was standing next to them, it has to be a crew member, most likely a camera man based on the odd shape of the shadow.
Truth & Consequences (season 2, episode 10)
Continuity: Season 2, episode 10, "Truth and Consequences": In the previous episode, "Cautionary Tales", when Noah was brought back to life he regenerated, then sat up, looked around him and said something sounding like "Home". In this episode, when the exact same scene is shown, we see Noah regenerating and sitting up, but this time he just looks straight ahead before lying back down, and does not say anything.
Powerless (season 2, episode 11)
Continuity: Season 2, episode 11, "Powerless":. In Mohinder's kitchen, when Sylar cocks the hammer of the gun he is holding on Mohinder, he is holding the gun with his arm straight and the gun completely level. The angle changes just as Sylar says "Maya, I need you to stay calm", and now his arm is close to his body and bent so the gun is pointing at Mohinder at an upwards angle. There was no time for him to move his arm in between shots.
All of series 3 (season 3)
Continuity: In season 3 episode 4 (I Am Become Death) when Claire Bennett is cutting Peter Petrelli's chest she makes two cuts then the angle changes and the blood is gone, the angle changes again and it is back.
One of Us, One of Them (season 3, episode 3)
Revealing: When Tracey looks at Nikki's dead body in the coffin (both played by Ali Larter) we can see Nikki's eyelids flicker.
