Visible crew/equipment: S02E01 "Four Months Later". As the samurai warriors are about to engage the attack, a close-up shot of Hiro turning around reveals the white screen reflected in his glasses.

Factual error: The solar eclipses in the show that affected the characters' powers were referred to several times as "global events". In the episode with the second eclipse it's shown to be visible in locations thousands of kilometers away from each other at the same time. This is simply impossible, since a solar eclipse occurs when the moon passes between the Earth and the sun, casting a moving shadow that can be only about 250 km in diameter (for a total eclipse). The partial eclipse can be viewed in a much wider region, but the eclipse in the show was a total one. It's not a global event, but a local one.

Plot hole: During the twelfth episode of the fourth season, "The Fifth Stage", Peter absorbs René "The Haitian"'s abilities (erase memory and neutralize abilities). Later on he fights Sylar and uses René's ability to force Sylar out. Sylar faints and turns into Nathan. If Peter absorbed René's abilities to neutralize abilities, shouldn't Sylar be unable to shape-shift into Nathan's looks?

Continuity: In episode 211 "Powerless", right before Sylar shoot Maya, you can see she already has a bullet wound on her chest.

Continuity: Episode 4.18, "The Wall" - The dried blood on Claire's neck from when she was shot toward the end of the previous episode keeps disappearing and reappearing throughout the scenes at the Carnival.

Plot hole: Season 4, episode 5 "Hysterical Blindness": Peter "catches" Emma's ability and loses his speed ability when he touches her, to keep her from being hit by the bus. Yet in one of the previous episodes, he shook hands with Samuel without losing his ability and gaining Samuel's.

Continuity: Episode 3x13, "A Clear and Present Danger": When Usutu is talking with Matt in Matt's apartment, Usutu's walking stick jumps back and forth from horizontal across his shoulders, to vertical in his hands, and back to his shoulders.

Continuity: (S04e18) Claire has red nail polish when she is in the house of mirrors, however, when she is outside talking to Samuel it is clear nail polish.

Continuity: Episode 3x23, "1961": When Claire is looking at the "Alice in Wonderland" dust jacket, she holds in one hand looking at the back cover, then in the next shot, she's holding it with both hands looking at the front cover.

Continuity: "Our Father". At the end of the previous episode, "The Eclipse, Part 2", Hiro brought Claire to the rooftop of the Deveaux building to witness Kaito handing over baby Claire to Noah Bennet. To illustrate his point, Hiro showed Claire the exact same thing happening in the "9th wonder" comic book he brought with him, and she can follow the Japanese dialogue from there. But when the exact same scene is shown at the beginning of this episode, Hiro does not show Claire the comic at all.

Continuity: Season 2, ep. 3, "Kindred". When Peter gets the box with his belongings back from the Irishmen, as Caitlin says "Well?" Peter's hands are in his lap. The angle changes to Peter, who answers "Well, what?" and he suddenly has both hands on the box, with no time to move them between shots.

Visible crew/equipment: S02E11 - "Powerless" When Mohinder, Sylar, Maya and Molly are at Isaac's old flat (now Mohinder's lab), as Mohinder and Sylar walks down the stairs, you can see the moving camera crane reflected in the glass behind them.

Factual error: In Season 2, Episode 3, Peter and the Irish family rob the bookmakers and the security guards shoot at them. In Ireland, even the police don't carry weapons. let alone do security guards.
