Continuity: Viper has a green headband on with a white bandage underneath. But as he leaves, then kisses Meredith, the bandage has vanished.
Make Me Lose Control (season 2, episode 3)
Continuity: We see a nurse writing down "Yang, C" on the assignments board; note the handwriting. Later, when Burke notices that Christina's name is on the board, the handwriting of "Yang, C" is completely different and other minor writing differences are noticeable too.
The Self Destruct Button (season 1, episode 7)
Continuity: When Izzy, George and Christina walk over to the vendor, talking about Dr. Sheppard and Meredith, Izzy is in the middle, eating snacks from a packet. Between shots she alternates from holding the pack either up or down.
Visible crew/equipment: When they arrive at work near the beginning, as Meredith exits the car, and walks past, the boom mike is reflected in the car, in front of her, following her along.

Continuity: Izzie's glasses disappear from her face, when the interns are getting the gurney from the airpad.

Continuity: Olivia walks into an elevator with Izzy already inside and they go up 3 floors. When Izzy leaves the elevator, it's the same floor from which Olivia came (same equipment and decor).

Character mistake: In episode 5-18, "Elevator Love Letter," when Dr Shepphard walks into the OR to start the brain surgery on Izzy, after already scrubbing, he touches his forehead and surgical cap. He would not have done this as he would then contaminate himself, forcing him to re-scrub.
The First Cut Is the Deepest (season 1, episode 2)
Continuity: When Meredith and Derek are at the elevator, Derek is on a palm pilot. Between shots, he closes it twice.
(As We Know It) (Part II) (season 2, episode 18)
Continuity: The scene where the bomb explodes shows the bomb squad captain walking toward the end of a hallway. Just as the bomb explodes you can see he is about 5-6 feet away from the end of the hallway where another bomb squad tech is waiting. After the explosion the scene cuts to Meredith flying backwards then to a shot of windows being blown out then back to a rear shot of Meredith hitting the floor. Notice here that a second floor is not visible at the end of the hallway. The scene finishes with a final aftermath shot of the hallway as the debris settles. Here we see an entirely different hallway with a visible second floor and the end of the hallway extended by about 10 feet.
Deny, Deny, Deny (season 2, episode 4)
Continuity: When Dr. Miranda Bailey tries to resuscitate/reanimate her patient Jeremiah, the assistant puts the anesthesia bag down next to Jeremiah's shoulder, in the next shot however, she has the bag back in her hands and lies it down once more.

Continuity: In episode 6-02, "Goodbye," in the scene where Izzy is getting chemo, the amount of popsicle that is bitten off on Meridith's pop changes from more to almost nothing from shot to shot.
Damage Case (season 2, episode 25)
Visible crew/equipment: When Izzie closes the door with blinds hanging on it, before going to sit on Denny's bedside, you can see a crewmember with their filming equipment reflected in the glass of the picture hanging behind her.
Let It Be (season 2, episode 8)
Continuity: At the start of the episode when George drops his bagel, he drops it with the white napkin stuck to it. In the very next shot when the pigeon begins to eat it, the napkin has totally vanished.
Visible crew/equipment: When the pigeon starts eating George's bagel which he dropped on the pavement, you can clearly see a red clasp with a long wire attached to its left leg so that it won't fly away.
Make Me Lose Control (season 2, episode 3)
Continuity: When Derek sits in the rocking chair in the room with the premature baby and then Addison starts listing their relationship options, the blanket hanging over the back of the rocking chair constantly changes position, evident by the green stripes on it.
Let The Truth Sting (season 4, episode 3)
Continuity: When Meredith's talking to Lexie in the clinic, in one shot from Lexie's right there's a chunk of hair hanging in front of her ear. We cut to a shot from the other side, then back, and without her touching it, it's now neatly tucked back.
Winning a Battle, Losing the War (season 1, episode 3)
Continuity: When Christina is talking to Shelley about allowing her husbands organs to be donored, the girls crayons on the desk keep changing position.
The First Cut Is the Deepest (season 1, episode 2)
Continuity: When Meredith drops all the files in the elevator to make out with Derek, they are in a different position from when she picks them up.
Deterioration of the Fight or Flight Response (season 2, episode 27)
Continuity: At the very start of the episode when it shows Burke laying on the floor after being shot, there is a very large pool of blood with his glasses right next to the blood. But in the next shot there is no pool of blood anymore but it returns in the next. Additionally, the pair of glasses change position.
