Continuity mistake: When Alex is giving Izzy her medication, he is holding a water bottle and an unpeeled banana In his right hand. Then the banana magically disappears and he is holding only the water bottle. Immediately, the banana reappears and it is now peeled. This all takes place in three quick shot which are from different takes.
Factual error: Derek performs CPR on Meredith in the back of the ambulance, but he does so incorrectly. He gives one breath and five compressions instead of two and thirty. Also, drowning victims need oxygen, and Derek should have used a BVM to ventilate Meredith, not mouth to mouth. Any doctor would have known how to provide proper treatment.
Factual error: Izzie tells her patient, Cheyenne and her mother, Mrs. Ward that they live in the same town she grew up in - Chehalis, Washington, yet mispronounces it. She says CH-halis (like CHurch) when it is SH-halis like Chinook Salmon.
17 Seconds - S2-E26
Visible crew/equipment: After Burke and Alex board the helicopter to get the heart transplant, in the shot of Cristina backing away from it, you can see a red-capped crewmember and the camera reflected in the chopper's window. (00:06:25)
Continuity mistake: When Addison gives Derek the juju cocoa, he raises it to his lips three times in different shots, then is instantly holding it down by his waist in the next. (00:04:00)
Continuity mistake: When Izzie is looking at the patient board, there are some lines on it which have been wiped out. But in the next scene we actually see Meredith walk to the board and wipe these exact same lines off (even though they were previously rubbed out...) (00:34:05)

Continuity mistake: When Callie places the hockey player's hand X-Ray on the chart, in the first shot it is on the left of the board. In the next it is dead centre and then back to being on the left again. (00:09:35)

Continuity mistake: After Alex and Izzie have had sex in the closet, just before Izzie leaves in the first shot her hair is hanging in front of her shoulders. In the next shot as she walks out, it is all pulled back behind them. (00:13:35)
Continuity mistake: When Derek gives Meredith a white bag to hyperventilate into when she is crying in the closet, the large orange sticker on the side of the bag disappears and then reappears again. (00:35:30)
Continuity mistake: At the beginning when Derek is going to cook the trout and Addison is angry at him, the fridge door is shut. But in the next shot it is open a little bit and Derek closes it. (00:02:20)
