Continuity: Season 5, episode 8 "A Distinctive Horn": When Audra is talking to Andy outside of the clinic, everything about her cigarette changes between shots (position, smoke, etc).
Other: Season 5, episode 5 "Van Nuys": When Andy pulls his piece of meat out of the fondue pot it hasn't been cooked at all. The fondue obviously hasn't been turned on (in the next shot he sticks some meat in his mouth, which has been cooked and is smaller).
Continuity: Season 5, episode 4 "Super Lucky Happy": When Roy Till and Ignacio are bound to the bed by Nancy, the black ties around Ignacio's left wrist keep changing places between shots.
