The Only Church in Town - S2-E3
Continuity mistake: The picture on the handbill the cop hangs up changes from an artist sketch to a black and white photo.
Other mistake: As the Military vehicle jumps off the end of the pier, the engine is heard running at the highest RPMs. But there is no engine to be seen in two shots.
Incident at Crystal Lake - S3-E25
Continuity mistake: The team were the only occupants of the chopper at the end. However, when B.A. gestures at the still running chopper, you can see the real pilot in the seat.
Continuity mistake: Just as Murdock gets to the point of jamming the brakes to stop the van, the chase scene is outside. If you look just to Murdock's left in the closeups, you can see a corrugated metal wall that isn't moving, with only the van bouncing a bit.
Bad Time on the Border - S2-E4
Continuity mistake: On the boat, B.A.'s mohawk is short on top, but in the sweatshop, it's longer.
Visible crew/equipment: When the team comes out of the van, at the left of the screen, the shadow of the camera man is visible.
Continuity mistake: After Hannibal tosses the grenade that flips the jeep, if you look closely, the man in the back end of the truck is wearing a white hat. When the camera is close up on Hannibal, he's not wearing a hat.
Continuity mistake: When Face starts tailing the black limo, the limo's tag is 55J-DKE. When it pulls up in the neighborhood, the tag is 62N-BHT.
Audio problem: When Murdock hits the hi-hat with his tomahawk, a crash cymbal sound is audible. (00:27:10)
Continuity mistake: As Murdock stands on the deck to dive in the water, he has two bottles of pressurized air on his back. The next shot shows him under water, with only one bottle on the back. The rest of the scene shows him once again with two bottles of air. (00:34:00)
The Rabbit Who Ate Las Vegas - S1-E7
Other mistake: When the two women first see the bus pull up and get on, the next scene we see when it picks up BA is the same video footage as when it arrived to pick them up. You can also see the same pink neon lights in the reflection of the bus and bus stop before it quickly cuts away. (00:02:50 - 00:04:20)
Continuity mistake: Murdoch has just walked across the fire in bare feet when he is clubbed on the back with a rock. When he wakes up, he has shoes and socks on.
Continuity mistake: As the goons are trying to get the van to stop, it changes from a Ford van to a Dodge with a messed up bumper, and back to the Ford between shots, and the bumpers don't show any damage.
Continuity mistake: The number B.A. gives Hannibal is 555-2944, but the number Face looks up is 555-7477.
Waste 'Em! - S3-E21
Revealing mistake: As the garbage truck follows Hannibal's van, when the camera has a wide shot of the goons in the garbage truck, you can the background scenery over the driver's left shoulder never moves.
Continuity mistake: As the van door is opened, B.A. has a dark green knit cap on. After the cut to the client and back, the cap disappears.
Other mistake: In the Cable van, Face plugs the red and black leads of his control onto the wires. As he moves the controller into camera view, the black lead comes off, so the interrupter shouldn't work, but does anyway.
Cup A' Joe - S3-E14
Continuity mistake: After Hannibal jumps from the semi and the trailer explodes, the tractor stops about 45° to the left. Later, they come back to the rig, and it now sits straight, and there is no scorched grass, or signs of any kind of explosion.

Revealing mistake: After Hannibal gets the two goons in the van and takes off, if you look through the driver's window, you can see the van rocking, but the trees behind them aren't moving as they should.
Answer: They never use an M14. The nearly always use a Ruger Mini 14, a totally different weapon.
