Lost

Do No Harm - S1-E20

Revealing mistake: Jack's left arm is bandaged from Boone's blood transfusion. After the shot of Shannon, as she cries over Boone, Jack sits on the beach in the wideshot and the bandage is on his right arm, but in the following shots it's on his left. Yet another flipped shot. (00:41:35)

House of the Rising Sun - S1-E6

Visible crew/equipment: In the scene where Kate asks Jack if she is checking her out after he walks past her, in the top left corner you can see a black boom microphone appear in the shot for a second. (Not visible on DVD.) (00:21:30)

Dave - S2-E18

Revealing mistake: In Season 2, Episode 18 - Dave: When Ana Lucia is threatening "Henry Gale" in the hatch cell; every time he moves and his makeshift shirt slips, you can see a line on his right upper arm where they applied make-up. Apparently he's wearing makeup on all the exposed parts of his face and neck to make him look sunburned, exposed to the elements etc. This is not just a normal suntan line, as his shirt wouldn't normally fall down that much out in the open and he's been inside for days now.

Tabula Rasa - S1-E3

Audio problem: When the farmer says, "You've been here for almost three months now, and every time I ask you," to Kate, it cuts, showing the farmer from behind saying "you get that look on your eyes," but his mouth isn't moving at all during the last sentence. (00:17:45)

Exodus (1) - S1-E23

Continuity mistake: When Jack and Ana-Lucia sit in the airport lounge she takes out her cell phone when it rings, and in this shot her glass is nearly full and has plenty of ice. Yet, when she leaves a moment later, the glass is nearly empty with no ice whatsoever. (00:12:50)

House of the Rising Sun - S1-E6

Revealing mistake: After Jack brings some survivors back to the cave, there is a shot of Hurley turning on his CD player to listen to Willie Nelson. In the next shot of Jack, as he fills bottles with water, his large tattoo is on his right upper arm, but normally it is on his left. The shot is flipped. (00:39:40)

One of Them - S2-E14

Revealing mistake: When Sayid and Locke are talking to Henry Gale in the hatch, just as Jack comes in and finds them, the front part of the arrow supposedly piercing Gale's shoulder is at a different angle to the back part. This is visible in a number of shots.

Deux Ex Machina - S1-E19

Visible crew/equipment: When Boone climbs up the vines toward the plane, in the first wideshot there is a rather conspicuous round CG blur at Boone's left hip that moves as he moves. Then in some following wideshots the black stunt belt at Boone's waist is perfectly visible as he makes his way up. In Boone's close-ups no such aids are on his body. (00:30:35)

Greatest Hits - S3-E21

Factual error: In the first flashback, when Charlie first hears himself on the radio, there is a close-up of the UK number-plate on the front of the van. It is the wrong colour: this is a yellow plate. As it is on the front of the van, it should be white. Only rear plates in the UK are yellow. This false plate was made up in mirror-image by the production company, as the actual van used was Left-Hand Drive. The whole scene, apart from the shot of the radio, was filmed and then flipped for the final cut, to make the van appear to be Right-Hand Drive as you'd find in the UK. Hence the moles on the right of Charlie's neck are on his left in this scene.

Exodus (2) - S1-E24

Factual error: Hurley arrives at the departure gate and calls to the woman to wait and let him board. As he goes through the gate, a sign with the word 'authorized' is visible to the left. This scene supposedly takes place in an Australian airport, but this is the American spelling - in Australia, the word is spelled 'authorised'.

STP

Exodus (2) - S1-E24

Continuity mistake: In the shot of Michael, just as the raft hits the log, the "ladder" and sail are directly behind him and Sawyer is gone, whereas in the previous and following shots Sawyer stands on the ladder, in that exact spot. (00:34:00)

Solitary - S1-E9

Continuity mistake: When Sayid is pulled upside down in the trap, he swings toward the tree and is stabbed by the branch. It is an entirely different branch in each of the following shots: first - when he is stabbed, second - when it sticks out of his thigh, third - when he pulls it out. (00:03:20)

The Glass Ballerina - S3-E2

Benjamin Linus: Your flight crashed on September 22,2004. Today is November 29th. That means you've been on this island for sixty-nine days. And yes, we do have contact with the outside world, Jack. That's how we know that during those sixty-nine days, your fellow Americans re-elected George W. Bush, Christopher Reeve has passed away, Boston Red Sox won the World Series.
[Jack begins to laugh.]
Benjamin Linus: What?
Jack Shepard: [Still laughing.] If you wanted me to believe that, you probably should have picked somebody else besides the Red Sox.
Bejmain Linus: No, they were down 3-0 against the Yankees in the Championship Series and then won eight straight.
Jack Shepard: [Skeptically.] Sure, sure. Of course they did.
[Ben turns on a VCR.]
Joe Buck: Back to Foulke... Red Sox fans have longed to hear it! The Boston Red Sox are world champions! A clean sweep of the St. Louis Cardinals, and the Red Sox celebrate in the middle of the diamond here at Busch Stadium.

Trivia: The character Sun was created specifically for Yoon-jin Kim, after she auditioned for the role of Kate but the writers felt she was not right for the role.

The End (2) - S6-E18

Question: I recently submitted a question about whether everyone died on the plane or on the island. The answer I got was unsatisfactory. The answer was they did not all die in the plane crash but on the island where the events in the show really did happen. If this is the case, everyone in purgatory at the end makes no sense. If they all died on the island, then where were other characters like Michael, Ecko, etc. Also how did Kate, Sawyer, Hurley, Ben, etc. end up dead and in purgatory at the end of the series? Last we saw them they were still alive on the island. Are we to assume that everyone died at the end of last season when the bomb went off? I need more info here.

SAZOO1975

Chosen answer: None of the main characters died in the plane crash. Many died on the island after the crash (Jack, Charlie, Sun, Jin, Daniel, Juliet, others), some lived on the island for an apparently long time after the crash (Hurley, Ben, Bernard, Rose) but some lived lives off the island after the crash (Sawyer, Kate, Claire, Miles, Aplert, and Lapidus). Remember that Christian tells Jack that "time has no meaning here," (in Purgatory). When everyone meets at the church, they are at the end of their lives however long that may have been and will now "move on" together. They look like they did on the island because that is the way they best remember each other.

Guy

That has to be the clearest explanation of the ending I've ever read.
