Plot hole: The female police officer has a theory that Johnny was the Strangler in the case the papers claimed he helped solve but this hardly makes sense. If she was researching him as was seen in the episode, surely it would have said something about Johnny being in a coma during the bulk of the murders since the story was about Johnny.

Dinner with Dana (season 1, episode 11)
Continuity: When Dana and Johnny are drinking wine, the amount in the glasses change between shots. Also whenever they pretend to drink, if you watch closely the liquid in the glasses doesn't even so much as touch their lips when they drink.
Precipitate (season 2, episode 5)
Other: In some of the "frozen" shots for Johnny's visions, people are supposed to be completely still but clothing is blowing in the wind at normal speed. Watch for people wearing trenchcoats and long clothing for the most obvious examples.
Zion (season 2, episode 12)
Other: In one of the final scenes, Bruce is talking to his father and everyone in the church is "frozen" in a vision. Watch carefully and you can see some people moving slightly.
Plague (season 2, episode 14)
Continuity: When Johnny, the health inspector and the men in bio-suits return to the school, there are four men in bio-suits. The inspector gives three of them orders on where to search for clues and the fourth one seems to have magically vanished.
The Hunt (season 2, episode 16)
Continuity: Why does Johnny have trouble getting into the booby trapped room? He shouldn't, as it was made clear in the previous episodes that he can freely explore the scenes in his visions and see things the person or object touched did not. Why is he suddenly limited in this episode and never once previously, and never again in the series?
Collision (season 3, episode 3)
Character mistake: When Johnny is at the hospital, having a vision of his mother, Gene Purdy and Sarah is waiting for news of him following his accident, he states that a vision earlier was related to his accident 'Nine years ago'. Later, when he is in the ambulance on the way to where his accident occurred, he asks the paramedic if it was the same ambulance. The paramedic replies, 'Yeah, I've been driving the same rig for nearly eight years'.
Cold Hard Truth (season 3, episode 4)
Factual error: When Johnny and Bruce are riding in the truck, listening to the 'shock jock', Bruce turns off the radio by turning the volume control on the left side of the radio. Johnny says he wants to hear it and turns the radio back on by turning the knob on the right, which is the tuning dial.
