Other: Season 6, Episode 5, "Interred" : After a jump from the roof, you see the guy lying on the street from above. There's no blood or any ugly mess. He's just lying there. This is the cleanest crime scene I've ever seen.

Continuity: Season 5, episode 2 "Independence day" : At the beginning of sitting in a traffic jam, Johnny and Bruce unlock their seatbelts. During their waiting Bruce's seatbelt is suddenly back locked, then unlocked again.

Continuity: Season 4 Episode 12, "A Very Dead Zone Christmas": In the beginning, Johnny takes the keys of the old man, having a vision of a young man being chased by cops in old uniforms. Johnny's hands switch from bare to wearing gloves two times.

Continuity: Season 5, episode 4, "Articles of faith": When Johnny touches the arm of the suspect through the jail bars to get a vision, the man stands right before him. But when the vision starts, Johnny's arm is suddenly stretched out widely. It doesn't match.

Plot hole: The female police officer has a theory that Johnny was the Strangler in the case the papers claimed he helped solve but this hardly makes sense. If she was researching him as was seen in the episode, surely it would have said something about Johnny being in a coma during the bulk of the murders since the story was about Johnny.

Continuity: When Dana and Johnny are drinking wine, the amount in the glasses change between shots. Also whenever they pretend to drink, if you watch closely the liquid in the glasses doesn't even so much as touch their lips when they drink.

Other: In some of the "frozen" shots for Johnny's visions, people are supposed to be completely still but clothing is blowing in the wind at normal speed. Watch for people wearing trenchcoats and long clothing for the most obvious examples.

Other: Typing error : The East German passport has 2 missing 'A's - the name of the country was 'Deutsche Demokratische Republik' but in the cover print and in the passport stamp you can read 'Deutsche Demokrtische Republik'. By the way, the spoken German is absolutely correct.

Revealing: The windows were down on his VW when Johnny dropped his girlfriend off before his accident. He walked her to the house as it started to pour, and the windows were up when he ran back to the VW.

Continuity: When Johnny, the health inspector and the men in bio-suits return to the school, there are four men in bio-suits. The inspector gives three of them orders on where to search for clues and the fourth one seems to have magically vanished.

Continuity: Why does Johnny have trouble getting into the booby trapped room? He shouldn't, as it was made clear in the previous episodes that he can freely explore the scenes in his visions and see things the person or object touched did not. Why is he suddenly limited in this episode and never once previously, and never again in the series?

Character mistake: When Johnny is at the hospital, having a vision of his mother, Gene Purdy and Sarah is waiting for news of him following his accident, he states that a vision earlier was related to his accident 'Nine years ago'. Later, when he is in the ambulance on the way to where his accident occurred, he asks the paramedic if it was the same ambulance. The paramedic replies, 'Yeah, I've been driving the same rig for nearly eight years'.

Cold Hard Truth (season 3, episode 4)

Factual error: When Johnny and Bruce are riding in the truck, listening to the 'shock jock', Bruce turns off the radio by turning the volume control on the left side of the radio. Johnny says he wants to hear it and turns the radio back on by turning the knob on the right, which is the tuning dial.
