Across whole show

Entry Every episode has the patient list by the nurse's station. On the patient list, different names are mentioned, like Marguiles or Tierney. They put these names there as a spoof of the show.
Entry Doug Ross frequently hung his head low, appearing ashamed or thoughtful or privately amused, depending on the scene. This wasn't just an element of the character: George Clooney had taken to writing his lines on papers, sheets, and other props.
Entry It is finally revealed (11-14: Just As I Am) that Dr. Weaver uses the crutch because of congenital hip dysplasia.

24 Hours (1) (a.k.a. The Longest Day (1)) (series 1)

Entry The first four episodes of the show all began the same way - with one of the doctors being awakened early in the morning from 'Exam 8' at the end of the hallway, in this order: Greene (Pilot), Lewis (Day One), Benton (Going Home), Carter (Hit and Run). Ross (George Clooney) was the only one of the original five who never slept in Exam 8.

Ambush (series 4)

Entry In the live broadcast of 1997, the baseball game that George Clooney's character is watching in the break room was the Cubs-Astros game, also being broadcast live that night on WGN.
Entry If anything had gone wrong during the broadcast of the live episode, e.g. a technical failure or forgotten dialogue; the producers had additional actors ready to improvise a scene that would have been inserted to cover. This was never used.

Secrets and Lies (series 8)

Entry In this episode, Kovac mentions that he played Hamlet at home in Croatia. Goran Visjnic, who plays Kovac, was discovered by a casting director for ER while playing Hamlet with the Croatian National Theater in Zagreb.

Chaos Theory (a.k.a. Unknown Origin) (series 9)

Entry The character of Haleh was supposed to appear in this episode but was written out, and with good reason. This episode picks up right where the previous season's finale left off, but the actress who plays Haleh had lost over 100 pounds over the summer hiatus.

Freefall (series 10)

Entry In "Freefall" as Dr. Romano and Neela are going up the elevator to return the forgotten watch, the Musak playing is "Freefalling" by Tom Petty and the Heartbreakers. Fitting.
