Love's Labor Lost (season 1, episode 20)

Mistake Continuity: As Mark futilely pounds on Jodi O'Brien's chest, Carol can be seen in the background, removing her protective glasses with one hand. However, after the time of death has been called and they're showing the reactions of everyone present in the room (Susan, Carol, Carter, Lydia, Chen), Carol is seen taking the glasses off again, this time with both hands.

Mistake Other: Season 12, Episode "Blame it on the Rain": At the beginning of the episode, Abby is almost run over by a man driving an SUV. They exchange words and then Abby throws a supposedly full cup of coffee at him. The cup of coffee bounces off the seat of the man's car and it is obvious that the cup is empty.

House of Cards (season 1, episode 22)

Mistake Continuity: When Malik finds Carter's family noted amongst the richest in Chicago, he says that Carter's father is named 'Roland'. However, in later episodes, Carter's father's name is given as John Truman Carter, Jr.

Missing (a.k.a. Mistaken Identity, a.k.a. Identity Crisis) (season 10, episode 9)

Mistake Factual error: The kid from the car accident has had an orthopedic device called a halo put onto his head in order to immobilize his head and injured spine. This item is screwed directly into the skull and is very secure. However, when the doctors are giving the boy CPR, the halo stays stationary but the child's head moves back and forth inside of it. The halo should have kept his head still, or his head would have moved only if the halo moved.

Chicago Heat (season 1, episode 7)

Mistake Visible crew/equipment: After Dr. Benton talks to the police detective and then goes to see the grocery store owner, as Dr. Benton swings open the doors, the camera crew can be seen in the reflection of the glass of the door.

Greene with Envy (a.k.a. Lawrence of Northwestern) (season 6, episode 3)

Mistake Continuity: When Dr. Lawrence is working on the ER with Mark, he gets frustrated because the tag Kerry's issued to everyone gets in his way. He asks someone to get it out of his way, and as he moves around to the other side of the table, we can see it has been pulled up around the back of his neck. As he reaches the other side, it is suddenly hanging straight down again. A short time later it is tucked up around his neck again. At no time did he adjust it.

Ground Zero (season 4, episode 6)

Mistake Continuity: Carter is talking to Carol about his grandmother. The shot is mainly on her but we can see him put his pen away and pick up a chart - then the shot goes to him and we see him do the same thing again.

Good Touch, Bad Touch (season 4, episode 5)

Mistake Revealing: When Greene and Benton are at the coffee stall, Greene calls Benton "Eriq," which of course is the actors real name.

Owner of a Broken Heart (season 14, episode 14)

Mistake Continuity: When Morris is on the bed following his shock there are three point of view (POV) shots. Morris on the bed, Morris's POV of the medical staff hovering over him and a shot from behind the staff looking down on Morris. The placement of characters is misaligned between views, most notably when the shot is from behind the staff looking down: there is a sizable gap between the two on the left and right, where as from Morris's POV they are clustered close together with no gaps between the staff members.

Ambush (season 4, episode 1)

Mistake Continuity: Mark is intubating the man who is paralyzed from the neck down. We can see his hands working - we know it is him by the splint on his wrist and he is talking as he works. However, Malik comes in and calls him away, and he rushes off, but the hands working on the patient don't change.

Let the Games Begin (season 3, episode 2)

Mistake Continuity: Carol is at her house with her mother, who makes 2 cups of coffee. She brings them to the table and Carol takes one but in the next shot, both mugs are still sitting on the table in the original position.

Love's Labor Lost (season 1, episode 20)

Mistake Continuity: Peter goes into the operating room, demanding to help with his mother's surgery. When he goes in, his cap is pulled down over his hair, but when he is talking to the surgeon in charge, about an inch of his hair shows for a couple of shots. The surgeon orders him out, and he leaves, and his hair is completely covered again. At no time did he adjust the cap.

Across whole show

Mistake Plot hole: There are always family members right in the room while the doctors are literally cutting into the patients. The family is always creating a fuss or whatever, until someone asks for them to escorted out. A real hospital would never let family members into the room while they are cracking open their loved one's chest.

The Domino Heart (season 6, episode 11)

Mistake Continuity: When Louise is talking about her mother's Parkinson's, the camera shot from behind her shows her left arm folded up in front of her with something red over it - but the shot from the other side shows the red item draped over her right arm.

Abby Road (season 6, episode 12)

Mistake Continuity: Near the end, when Elizabeth and her mother are having wine, in one shot her mother has put the glass down on the table, but in the next shot, from behind her, she has the glass in her hand again. There was no time for the change.

Viable Options (season 6, episode 17)

Mistake Continuity: Luka is in talking to the first possible kidney transplant patient - he puts his pen into his pocket and is holding the chart in his other hand, also up by his chest. As he says "Minneapolis", the shot changes and we see him put his pen in his pocket again and his chart is down by his waist - there was no time for the shift.

Sand and Water (a.k.a. Way Too Soon) (season 7, episode 2)

Mistake Continuity: When Mark is standing in his kitchen talking to Elizabeth, the position of his hand on his cereal bowl changes from shot to shot.

Make of Two Hearts (season 1, episode 17)

Mistake Continuity: Doug and Peter are running to meet the ambulance transporting the boy with the crushed leg. From the back we see Doug with his arms out as he runs, but the shot changes to the front and he has his hands in his pockets, with no time to change naturally.

Be Still My Heart (season 6, episode 13)

Mistake Visible crew/equipment: When the son of the accident victims is being led through the trauma rooms to see his parents' bodies, you can see Laura Innes crouching under a table in the background directing the scene. Innes was the director of this episode and it was established that this episode took place on her character's day off. Therefore, she should not have appeared in front of the camera at all.

Random Acts (season 3, episode 20)

Mistake Revealing: When Dr. Greene is being attacked in the toilet, he is thrown into a supposedly brick wall. It bends and wobbles like a set wall would.
