House of Cards (series 1)
Greene with Envy (a.k.a. Lawrence of Northwestern) (series 6)
Continuity: When Dr. Lawrence is working on the ER with Mark, he gets frustrated because the tag Kerry's issued to everyone gets in his way. He asks someone to get it out of his way, and as he moves around to the other side of the table, we can see it has been pulled up around the back of his neck. As he reaches the other side, it is suddenly hanging straight down again. A short time later it is tucked up around his neck again. At no time did he adjust it.

Other: Season 12, Episode "Blame it on the Rain": At the beginning of the episode, Abby is almost run over by a man driving an SUV. They exchange words and then Abby throws a supposedly full cup of coffee at him. The cup of coffee bounces off the seat of the man's car and it is obvious that the cup is empty. Submitted by Allyson
Plot hole: There are always family members right in the room while the doctors are literally cutting into the patients. The family is always creating a fuss or whatever, until someone asks for them to escorted out. A real hospital would never let family members into the room while they are cracking open their loved one's chest.
Missing (a.k.a. Mistaken Identity, a.k.a. Identity Crisis) (series 10)
Factual error: The kid from the car accident has had an orthopedic device called a halo put onto his head in order to immobilize his head and injured spine. This item is screwed directly into the skull and is very secure. However, when the doctors are giving the boy CPR, the halo stays stationary but the child's head moves back and forth inside of it. The halo should have kept his head still, or his head would have moved only if the halo moved.
Owner of a Broken Heart (series 14)
Continuity: When Morris is on the bed following his shock there are three point of view (POV) shots. Morris on the bed, Morris's POV of the medical staff hovering over him and a shot from behind the staff looking down on Morris. The placement of characters is misaligned between views, most notably when the shot is from behind the staff looking down: there is a sizable gap between the two on the left and right, where as from Morris's POV they are clustered close together with no gaps between the staff members. The Domino Heart (series 6)

Continuity: Abbie is asking Peter and Carter questions about infant developmental stages and they are standing up straight next to the crib. When she says "don't worry, Dr. Benton", the shot shows Peter with one elbow draped over the crib and Carter is leaning over with arms folded. They could not have changed so quickly in real time.

Plot hole: During Season 5, the search for an ER Chief takes nearly a year and two nationwide searches, but is ultimately unsuccessful. At the beginning of Season 6, Romano is chosen as Chief of *Staff* in the blink of an eye, and Kerry is appointed as ER Chief just as quickly. Then, in Season 9, Kerry and Romano's positions are switched as though such things happen everyday. There's no way decisions of this magnitude would be made in such a brief amount of time.

Continuity: When Carter first starts to confront the drunk patients Angel and Nathan, the door to their room is open behind him. Angel falls to the floor and the shot shows the door is now closed. The shot goes back to Angel laughing and the door is ajar again. Then it's closed again, and then open when Susan comes into the room. All this happens within a few seconds.
Make of Two Hearts (series 1)
Continuity: Carol goes to visit Tatiana while she is sleeping. As the camera shows a shot from above, the blanket over the girl has changed position noticeably. (Time)
Continuity: Carol and Carter are in the room with the patient Tatiana and the dog; Tatiana has a yellow marker in her hand. In some shots she appears to be hugging the dog and in others drawing something with the marker, with no time to change positions. (Time)
Love's Labor Lost (series 1)
Continuity: As Mark futilely pounds on Jodi O'Brien's chest, Carol can be seen in the background, removing her protective glasses with one hand. However, after the time of death has been called and they're showing the reactions of everyone present in the room (Susan, Carol, Carter, Lydia, Chen), Carol is seen taking the glasses off again, this time with both hands.
Be Still My Heart (series 6)
Visible crew/equipment: When the son of the accident victims is being led through the trauma rooms to see his parents' bodies, you can see Laura Innes crouching under a table in the background directing the scene. Innes was the director of this episode and it was established that this episode took place on her character's day off. Therefore, she should not have appeared in front of the camera at all.
Homeless for the Holidays (series 3)
Plot hole: When Mark slips and falls in the staff lounge, he puts both hands on the table to catch himself - but we can see him (the actor) quite deliberately lower his head to one hand - this is supposedly where he cuts himself, but he actually hits his hand and not the table. This would not produce the deep cut he ended up with. Also, the head makes contact on the left of his forehead and he ends up with a cut on the right. And when we actually see the cut, it is vertical and logic tells us that it would have been impossible for him to have a vertical cut from the angle he fell.
