Plot hole: At the beginning Sam stated that the missing teacher comes to work by subway every day. But in the flashback of his conversation with the drug dealer, he's about to get into his car.

Continuity: In episode "One for All," the names of the students on the bottom of the seating chart do not match the names written on the "desks" to show where the girls sit.

Other: Slightly after the opening credits, Jack Malone is investigating the disappearance of Annie Miller and is standing in her mother's bedroom. He turns to Carol Miller's boyfriend and asks to "speak to Mrs. Malone alone." The subtitles correctly have 'Miller' written, but when Anthony LaPaglia says his line, the two words rhyme.
