Plot hole: Dead Like Me: Life After Death DVD movie: When he's robbing the grocery store, we see Mason from the point of view of the security camera. We see the Mason that the reapers and the dead see, but we should be seeing the face that is visible to the outside world.

Continuity: Dead Like Me: Life After Death DVD movie: In the scene outside the theatre, the flower appears in Daisy's hand before Mason has given it to her.
Continuity: The waitress played by Patricia Idlette is named Kiffani starting with episode 12, but in the first couple of episodes of the first season her name badge reads "Gerri."
Pilot (2 hours) (season 1, episode 1)
Plot hole: People who do not have their souls retrieved, before they are killed, will suffer the effects of the death on their soul, eg. the autopsied man has scars and cuts on his soul. So how come the woman who was crushed by a piano (whose soul was only ever retrieved afterwards) is unharmed in soul form?
Continuity: After George dodges the toad on the floor, she walks backwards up onto some steps, yet she walks up the same step twice.
Continuity: When Mason and George are about to go into the house of the murdered accountant they show the lock from the inside of the apartment and it's unlocked. When they show it again it's locked and then Mason unlocks it.
Continuity: When George delivers the files to some guy, while at her job, her hair keeps on changing position over her shoulder.
Continuity: When George is filing, she is told it is her lunch hour. Between shots her hair flips over her shoulder.
Continuity: When Rube and George are speaking to George at the site of her death, Betty's scarf continuously changes postions.
Visible crew/equipment: During George's funeral, the camera swings around to follow the employee from Happy Time (the guy who George gives files, minus the red ones, to) and as he crosses the room to the other side, you see a crew member holding a boom mic, who looks over a little startled and moves back out of view.
Continuity: George drops the red file from her stack of files to deliver, yet in the next shot you can still see the red file in the stack.
Continuity: When George is flicking through files in a drawer, they change positions between shots, when the man tells her its her lunch hour.
Revealing: When Mason and George walk up to the TV's in the store, the TV images do not match Mason and George's movements, as it records them.
Continuity: When George is sitting on the train, watching the little girl she is supposed to reap, the position of the ticket on the seat beside her changes.
Continuity: When George writes "moist" on the fridge, to give her mum a sign, the position of the 'T' magnet changes between shots.
Revealing: Reapers look 'different' in reflections and recordings, yet sometimes you can catch them with 'normal' reflections, eg. Rube in the diner window.
Continuity: When the first Reaper who touched George magically disappears, people in the background appear out of nowhere and disappear out of nowhere.
Continuity: When Betty is leaving the diner table to meet someone, she puts her bag on her shoulder. In the next shot, it's in her hands.
Continuity: When Rube sits down on the bus station bench, before George gets on the train, to reap her first person, he crosses his legs twice.
Continuity: When everyone is lying on the bank floor during the robbery, George crawls over to Mason. In the following shot she is back where she was before.
Continuity: When George delivers the files to the guy at work, the top file on the stack changes positions between shots.
Curious George (season 1, episode 3)
Continuity: In the scene where Rube shakes hands with the animal protesters, he is not wearing the trash bag while shaking their hands. He is wearing the trash bag immediately before and after.
Reaping Havoc (season 1, episode 5)
Plot hole: When George gets into the lift at her office, her reflection is very obvious on the sliding doors, but we were told back in the pilot that Reapers have a different visage to the outside world that shows up in reflections, on camera etc.
Audio problem: Each time George mentions Millie's older brother Luke, the movements of her mouth don't match what's being said. It appears that the name was changed and then dubbed afterwards.
Sunday Mornings (season 1, episode 9)
Continuity: When Guggenheim is sitting on the bench after he dies, he reaches for the pink index card and his fingers pass through it. But then how did the index card get to the bench? He had it when the gas tank hit him, so it could make sense if he still had it "with" him, like his clothes, but if he can't touch it now, then it should have stayed with his body, inside the building.
Vacation (season 1, episode 13)
Other: In the copy room, George, after she speaks with Daisy in voice over, says "its 3am", but the clock on the wall behind Crystal shows 12:35 when George talks to Crystal.
Factual error: Towards the end of the episode, Mason reads a report on Daisy's death indicating she died 13 December 1938 in Marietta Georgia of smoke inhalation. It apparently was common knowledge that Daisy died on the set of Gone With The Wind (supported in the Halloween episode by someone who knew her when she was alive). The filming of the big fire scene in GWTW was 10 December 1938 so the dates are pretty close. But these early parts of the production of GWTW (and I believe the rest of the movie as well) were filmed in Culver City, CA, not Marietta.
Continuity: When the reapers are inputting the data, they are only typing in the final thought. However, in every shot of the spreadsheets, there are entries for final thought, name, date, and location.
Send in the Clown (season 2, episode 1)
Factual error: Rube refers to Bacchus and Neptune as "Greek Gods" - in fact, they're Roman (the Greek equivalents being Dionysus and Poseidon), which Rube (being well-read enough to mention them in the first place) should know.
The Ledger (season 2, episode 2)
Continuity: When Rube is going to give his keys to George, Daisy has a cup in her right hand, at her lips, and is looking at George. Next frame, opposite angle, she has the cup in her left hand, at chest level, and is looking at Rube.
The Shallow End (season 2, episode 4)
Continuity: When Daisy takes Stan to church, she takes a seat in the fourth row and sets her purse down. After the window breaks, she runs outside, leaving her purse in the pew. When she returns to find Stan gone, the purse is in the fifth row.
Hurry (season 2, episode 5)
Continuity: In the airport, when Mrs. Thibideaux (Roxy's reap) sees the cluttered luggage holding up her suitcase, she climbs over the first few bags and tosses the others down. Before she even started climbing, the conveyer belt was stopped which meant the luggage that she climbed over (including what looked like a bag with skis) would still be in the way. Yet when she falls, it's a clean slide down to break her neck.
Death Defying (season 2, episode 10)
Other: When Rube is searching the hard-copy (paper) archives, the young man with him opens the last box and pulls out an old "Wanted" poster with Rube's face on it. He tells Rube "This guy looks just like you." Since season 1, episode 1, we've known that Grim Reapers don't look like they did when they were alive (i.e., Millie/Un-George). Yet somehow, Rube has not changed since his life/death.
Continuity: Near the beginning when Mason takes the notebook from Roxy, the camera is off the corner of the table showing all four seated at the booth. Roxy reaches across the table with one hand and for her gun with the other and starts to explain she will shoot Mason. The camera moves for a close up on Roxy and her hands are now in front of her, folded on the table.
Continuity: In the bar, just after Ray lost a pool game to Mason, they are watching a boxing match on TV. Ray begins to rack the balls and pulls out 4-5 balls and places them in the rack. The camera switches back and forth, Ray again pulls 4-5 balls out and racks them, but the original ones are gone. The camera switches back and forth again and when Ray is done racking the balls, the balls are in a different order than he placed. He is watching Mason, not looking where he places the balls. Watch the 2 and 11 balls.
Ashes to Ashes (season 2, episode 11)
Continuity: When Daisy is in the diner with the other reapers talking about her acting job, the fruit in her hand changes from a red grape or cherry to a green grape.
Audio problem: When Daisy is at the bar (acting), she gives Walter "5 reasons men come to a bar." When she says "Three: They wanna see that they're missing," her lips are not in sync with the words. It's tricky to see because they used a shot with the camera behind her.
Continuity: When Rube's talking to the graffiti artist, he's painting what looks like a blue cat. We cut away, hear a brief paint spray, then cut back. There's now far more painted than that spray could have achieved, and what was there already has changed as well.
Forget Me Not (season 2, episode 12)
Continuity: When George writes Nina's name on the sticky note she holds it up and we see the writing is small but then we see a close up and the writing it bigger and the handwriting is a little different.
Other: When Mason is punching the air and Nina imitates him the chair wiggles with the motion of her body but since she is dead she would pass through it.
