Continuity: In an early scene "Ma'chello" is lying in the infirmary with a oxygen mask on, it cuts to Frasier and O'Neill talking about him, then cuts back and "Ma'chello" now has nose tubes.
Prodigy (series 4, episode 19)
Continuity: Carter and Cadet Haley enter the Stargate and we see them through the "puddle." When they come out on the other side, their positions are reversed.
