Continuity mistake: S6-E02. When Zach Braff is talking to his girlfriend about whether or not they'll keep their baby, he says something like "I sure do," and is propping up his head with a hand. In the next shot (which continues his line of dialogue) he is suddenly no longer propping up his head.
Continuity mistake: Carla's about to eat a piece of steak. In the next shot she's holding her wine glass without enough time to change them. (00:19:25)

Continuity mistake: When Cox asks Elliot how her patient is doing he has a cup on the table. His hand goes from holding the cup to not holding it between the shots.

Continuity mistake: When Sean is talking to Betty the seal his arms go from being by his sides to clasped in front of him between shots.
Continuity mistake: The pen on the counter Dr. Casey used changes positions a couple of times while talking to J.D. (00:17:15)

Continuity mistake: In the cafeteria, there is a bottle of water on the table in front of Turk. This vanishes before Dr Cox comes to speak with him. It is not one of the items on the tray that Kirsten removes from the table.
My Bed Banter & Beyond - S1-E15
Continuity mistake: When JD and Elliot are talking when they first enter the hospital, JD's stethoscope moves between shots from sitting over his name tag, to resting on top of it.
My Catalyst - S3-E12
Continuity mistake: While Turk is talking to Elliot note that Turk's hot sauce bottle changes position without being touched. (00:19:50 - 00:20:20)
Continuity mistake: As Dr Cox is talking to J.D. towards the end of the episode and telling him that he won't let him treat the patient he traded, Dr Cox is putting on his white overcoat. If you look at his collar on the right side it is a little crumpled and not quite straight. The next shot we see over his shoulder and the collar has straightened out. The following shot it returns back to being crumpled up a bit.
My Old Lady - S1-E4
Continuity mistake: When J.D. picks up the granddaughter to blow out the candles on the cake, there are 3 candles placed at the front of the cake in the camera close up, but there weren't any before he picked her up. (01:17:35)

Continuity mistake: When Dr Cox is sitting on the couch talking to Elliot, a clipboard suddenly appears in his hands.
Continuity mistake: When JD is talking to Eliott in the cafeteria, his straw in his orange juice changes direction between shots.
Continuity mistake: My Lips Are Sealed Episode Number 89 Series number 4 When JD is brushing his teeth and then finds out that his brother is in the bathtub the amount of foam around his mouth changes.
Continuity mistake: When Elliot goes to the bus depot to stop Shawn from leaving, she jumps out of the car leaving the car door open. When she's on the phone with Shawn, the car is behind her and the car door is closed. As the bus pulls away, the car door is open again. (00:19:30 - 00:20:25)
My Overkill - S2-E1
Continuity mistake: Carla, Laverne and Dr Cox are in the restaurant, Dr Cox is in front of Carla and behind her are two hospital staff members, Carla walks away and it cuts to Dr Cox, but the two staff members have vanished.
Continuity mistake: JD is trying to tempt Turk to do a favor for him with a package of gum. The shot from the front shows JD holding the package with his thumb on the bottom and forefinger on top of the package. The shot switches to over JD's shoulder, and he is now pinching the package on the bottom corner.
My Big Move - S4-E22
Continuity mistake: Episode 22 - season 4. When Turk goes mad and throws the chips all over the table the chips' positions on the table differ between shots.
Continuity mistake: When Carla and Elliot are talking at the nurses' station, Carla's stethoscope moves from the right to the left of her name tag between shots.
Continuity mistake: The whipped cream on Eliott's Jello goes down between shots.
My Old Man - S1-E19
Continuity mistake: When the Janitor's dad tells him to drop and give him 30, the Janitor throws his mop and it lands to the floor. But in the next shot it's standing up in the mop-bucket. (00:13:10)
Answer: Dan was drinking beer and Cox feared that Dan might get so intoxicated he would fall asleep and drown in the tub.
