Visible crew/equipment: Towards the end, right after they watch the house blow up, you can see a hand holding a black object popping up from the right side of the screen. This happens right before Malcolm says, "Pretty sure".

Visible crew/equipment: When Dewey yells for Lois because the bow was put on the present, if you look to the left of Lois, you will see a crewmember kneeling down, handing Lois the red dress, who is holding a bucket of water. He quickly gets out of the camera's view. [This is on the DVD of the first season. Someone said on the audio commentary that after catching it, they deliberately left it in to see if anyone would spot it. It's still a mistake, but that's why it's there.]

Continuity: In the episode "Bride of Ida", in the scene where the grandma is talking at the table with Reese and Malcolm, notice the bottle on the table next the the grandma, between takes the cork from the bottle goes from being on top of the bottle to next to the bottle, it changes in almost every different shot.
Pilot (season 1, episode 1)
Revealing: At the start, when Lois tells Hal to raise his arms so she can shave his armpits, if you look at the bottom of the screen you can briefly see that Hal is wearing something to cover up his privates.
Home Alone 4 (season 1, episode 3)
Visible crew/equipment: While Malcolm, Reese, Dewey, and Francis are deliberately making their house messy, you can see right above their ceiling a bright light shining down and a black background. It shows that they film their interior of their house on a set.
Across whole show
Continuity: Episode 111 Season 6 "Pearl Harbor" Lois and Jessica enter the house after coming back from going to a musical. As Lois comes through the door her purse slips off her shoulder and she catches it around her elbow. In the next shot a second later, the purse is back up on her shoulder.
Revealing: In season 5, second episode, in an imagination sequence Reese and Malcolm are throwing around Jamie, but you can see it is a doll.
Continuity: Season 6, episode 7: "Hal Sleepwalks"; At school, Dewey stops the shouting match between the two opponents. In the shots facing Dewey, the boy's arms are held way up high in front of him, but in the shots facing the kids, the boy's arms are down at his sides.
Family Reunion (season 4, episode 3)
Plot hole: Hal says that his father was never there when he got dumped. But in episode 7, "Francis Escapes" Hal tells Francis he's never been dumped.
Long Drive (season 4, episode 11)
Plot hole: Hal states that Malcolm was supposed to pick Dewey up at school, but the earlier episode "Humilithon" established that Dewey now walks home unsupervised. (Malcolm doesn't yet have a driver's license, so this can't be a matter of convenience.)
Red Dress (season 1, episode 2)
Continuity: When Lois is yelling at the boys for playing with fire, the location of the green scissors on the table keeps changing between shots.
Flashback (season 2, episode 25)
Visible crew/equipment: When Lois goes into labour with Malcolm, she runs outside taking Francis and Reese with her. When Francis says, "You're a liar, you're a liar," watch the back window of the car and you will see the camera and camera operator's reflection in the window.
Malcolm Babysits (season 1, episode 5)
Continuity: When Malcolm is trying to get the TV back to the channel he was watching, he picks up a big remote and finds the channel which the video camera feed is on, but the remote he is holding on the TV is a smaller one. It then cuts back to Malcolm who is moving his hand away from the remote he just put down and it's the bigger one again.
Pilot (season 1, episode 1)
Continuity: In the opening, we see Dewey's cold feet. Two shots later, his legs are in a different position.
Continuity: When Lois hangs up on Francis before saying "Oh, I love you," it seems as though the line is already disconnected (as can be heard from the dial tone from Francis' side), but in a later shot, Malcolm and Reese are just about to hang up the phone.
Kicked Out (season 4, episode 12)
Continuity: Malcolm's seat belt changes from on to off the first scene with him and his mom in the car. When it's a shot from the front it's on, but when it's a shot from the side (from the driver's/mom side) you can see that the part of the seat belt that's going across is now behind Malcolm, instead of on him like it was a second ago. This changes back and forth.
Reese Cooks (season 2, episode 18)
Audio problem: When the girl Krelboyne is talking loudly, you can see her mouth barely matches anything she's saying.

Continuity: Season 5, Episode 13, "Lois' Sister."
When Susan arrives Hal is holding Jaime, and the baby's entire body changes position between shots repeatedly - in some shots the front of Jaime's body faces Hal, yet in others Jaime's back faces Hal.
Continuity: Episode 16 Season 6 "No Motorcycles." Francis calls Hal out to the driveway where two motorcycles are parked. In the first camera shot the motorcycles are parked side by side with the front wheels turned in the same direction. All subsequent shots of the motorcycles in the driveway show them in a different position, one angled behind the other, with the front wheels turned in opposite directions.
Traffic Jam (2) (season 2, episode 1)
Audio problem: Near the end when the family have got back in the car Malcolm says something like "who would have thought a traffic jam could be so cool, we should do this again". Well near the end of the sentence we can see Malcolm's mouth and it isn't moving.
