Revealing: On 12/24/03 and 12/25/03, Sheridan is supposed to be breastfeeding baby Martin. But Sheridan's shirt is barely unbuttoned, and pulled aside only marginally (and mostly from the opposite side of where the baby is). For his part, the baby is not even turned towards Sheridan's body. So, the way the scenes are shot, there's no way he could be nursing.

Continuity: The week of 12/8/03, Child Protective Services took Little Ethan from Theresa. As one would expect, she became hysterically upset. But after her son was gone, Theresa calmed down enough to call the police station and Luis' cell phone, all while speaking and acting completely rationally. When she realized that everybody was at Sarah's funeral with their cell phones off, she decided to go there. Inexplicably, by the time she got there, she was completely hysterical again.
Plot hole: On 11/29/03, Ivy tries to cook Thanksgiving dinner, and the turkey explodes. Comedic value aside, if the turkey was going to explode then it would have happened while cooking, and even that would have been highly implausible. It makes no sense at all for the turkey to explode after being pulled from the oven, when prodded with a spoon.
Continuity: On 11/7/03, Antonio finally realizes that Luis and Sheridan are in love and he has a series of rapid flashbacks of compromising situations in which he has caught them. One of those scenes is when Antonio almost saw Luis and Sheridan cuddling post-coitally in Sheridan's bedroom, but "luckily" went temporarily blind an instant before he looked in the window. So why could he see them together in his flashback?
Plot hole: In October, 2003, when Sheridan nearly drowned after being dumped off the boat, she found herself in a heavenly setting where her dead mother Katherine guided her "into the light." The next week, Death came and removed her spirit from her hospitalized body while she struggled against it. But wasn't she already dead?.
Other: In the summer of 2004, in Puerto Arena, Paloma met a man, Nick, who told her that Luis and the other Lopez-Fitzgeralds were just trying to use her, and didn't really love her. Later that night, Nick kidnapped Paloma and tried to rape and murder her. She stopped him by bludgeoning him with a rock, and later throwing him into a support pillar that resulted in a structure collapsing on him and presumably killing him, before reuniting with Luis, Sheridan, and the Wheelers. And yet that fall, after she had returned to Harmony, Paloma flashed back to what Nick had told her and behaved as if she believed what the murdering rapist had told her about the people who ultimately rescued her from him.
Plot hole: Sheridan's and Paloma's respective ages do not make sense with respect to the Martin/Katherine storyline. In Martin and Katherine's flashbacks, we saw that Sheridan was a little girl, no older than eight, when they left, while Paloma was a newborn. So Sheridan is about eight years older than Paloma. But when Paloma appeared in Mexico, in May of 2004, she was still a high school student. At the oldest, then, she might be eighteen. Sheridan is about two years older than Ethan, and Ethan has been a practicing lawyer since the show started in July of 1999. So Sheridan must be at least thirty, making her more than twelve years older than Paloma.
Continuity: On 9/27/99, in LA, Henry gives Chad a newspaper that was left in his family's apartment when Chad was abandoned at age eight. It is a Harmony Herald. Then we see Theresa and Whitney walking in front of a Harmony Herald newspaper box in Harmony, and the paper is identical to the one that Chad's family left behind about a decade earlier.
Continuity: On 3/17/06, Sam and Ivy had a conversation about Kay that was largely at odds with previous events and information from the show. In particular, they talk about how Kay pursued Miguel from a very early age. However, when the show began in 1999, Kay and Simone were in high school and their dialogue established in the very first episode that Kay had only very recently developed a romantic interest in Miguel. Furthermore, most of Harmony, including Sam and Miguel himself, didn't learn of Kay's interest until several years after that.
Continuity: When the show started in July of 1999, Gwen Hotchkiss and Sheridan Crane barely knew each other. When Gwen and Ethan were on the beach in Harmony, Gwen asked Ethan many questions about Sheridan. Later, when Gwen and Sheridan met in Paris, Sheridan asked Gwen a lot about her relationship with Ethan. Both exchanges made it clear that Gwen and Sheridan were not well acquainted. However, years later, Gwen and Sheridan often spoke about having been lifelong friends, even though they had only known each other for a few years.
Continuity: In July of 1999, in talking about Martin's disappearance, Pilar said that he left for work one morning, but never showed up there or at home ever again. However, flashbacks years later showed that Martin left Harmony with Katherine in the evening, after the Founder's Day dance that he attended with Pilar.
Continuity: On 7/28/05 and 7/29/05, a few episodes after Harmony, which is in Maine, was hit by an earthquake, the two men at the Tsunami Warning Center say that a tsunami is heading directly towards Harmony. It had previously been said that the earthquake's epicenter had been just offshore of Harmony, so the statement makes sense. However, they then looked at a computer graphic that showed the tsunami originating from far out in the Atlantic and heading westwards towards the entire eastern seaboard, as if the quake had occurred in the central or eastern Atlantic instead of near New England.
Continuity: On 7/19/05, Luis mentions his and Sheridan's past life in ancient Egypt. The viewers know about this past life, because Tabitha told Timmy about it. However, unlike their past lives on the Titanic and in 19th century America, which they experienced firsthand, Luis and Sheridan themselves never learned about their past life as Antony and Cleopatra.
Continuity: On 7/4/05, it was late at night and Alistair had just kissed Theresa. Pilar was warning her to stay away from him. Theresa was wearing a black, sleeveless dress, and Pilar was wearing a blue blouse and a matching light jacket. On 7/5/05, it was morning, which made sense because the characters had been saying through the past several episodes that it was almost morning. Theresa and Pilar were in the middle of the same conversation, unmoved from their previous location in front of the Crane mansion, but Theresa was wearing a white blouse with a floral print and Pilar's outfit was black.
Continuity: On 6/24/05, Sam and Luis were trying to break through a wall in the Crane mansion. The sound of metal on metal is heard, and Pilar asks what has happened. Luis and Sam explain that they've just hit a metal barrier that they think must conceal the room they're looking for. But several square feet of metal are already exposed. Why wasn't the metallic sound heard earlier, and why is everyone acting like they've only just discovered this second barrier?
