Plot hole: In episode 3-13: "Bride and Gloom," Prue was married to a warlock in a dark ceremony, causing her to almost immediately become evil, and this caused her sisters and the Book of Shadows to become evil as well because all of the magic is connected. In the fourth season, Phoebe married the Source in a dark ceremony, and did eventually herself become evil (as evidenced by her ability to touch the Grimoire), but the evil did not extend to her sisters or the Book. So the mistake is twofold: Phoebe did not immediately turn evil, and her sisters and the Book never turned evil.

Charmed Again (1)
Continuity: After Phoebe has her premonition about Paige at Prue's funeral, demonic bounty hunters show up to "eliminate" Belthazor. We see the funeral hall empty where seconds before Phoebe's premonition, there was a line of people waiting to give their condolences to the girls, Victor, Leo and Cole.
Continuity: Piper does not light the candles before she summons Grams, but in the episode 'PreWitched' (when Grams summons Patty) we see that the candles must be kept alight to maintain the connection with the dead.
Continuity: During Prue's funeral when the demonic bounty hunters attack, one gets thrown into some chairs, which knocks some down. When we see the chairs again in another shot the ones left standing and the ones knocked down have changed.
Charmed Again (2)
Plot hole: After Cole tells Leo, Piper and Phoebe the source has surfaced, Piper asks Leo if he can "sense Paige". Leo replies, saying no, "she's too new". But aren't white lighters meant to help witches who are just coming into their powers, which is exactly what Paige is doing?
Plot hole: Phoebe and Piper seem surprised at the fact that a white-lighter can impregnate someone, because they are dead. Piper says "how does a white-lighter get somebody knocked up?", but when she went to the future she saw herself with Leo's baby, and Leo told her that he hadn't clipped his wings.
Hell Hath No Fury
Other: When Piper and Paige are talking at the Social Services building, Paige talks about one of her coworkers, but in the closed captions, the speaker is mislabled "Piper."
Other: After the "dirtbag" is brought back to the manor and Piper is ragging on him, Piper gets very angry and decides to blow her husband up. Less than a minute later, Cole says "Back away slowly, Leo." Leo says "What?" and Cole says "She's becoming a fury." Then it shows Pipers hands, which now have long black fingernails, but they just suddenly show themselves right when Cole decides to show up and say she's becoming a Fury. When she blew Leo into orbs there were no long black nails.
Enter the Demon
Continuity: When Anling unmasks Yenlo, Yenlo's left hand is covering his wound. In the next shot, his hand is down by his side.
Plot hole: When Piper and the Zen master switch bodies and Yen Lo uses the dragon blade to steal Piper's soul, the Zen master's image is shown as a visualization of the soul, but it should be Piper's image instead.
Revealing: In the beginning of the episode, Yenlo and Anling are fighting with swords. After Yenlo says, "I'm going to kill him, Anling. And then I'm going to kill you," Anling stabs him with her sword. Yenlos' wound is very bloody, but there is no blood on Anling's sword at all.
Continuity: After Phoebe and Paige have switched bodies and Phoebe is trying to teach Paige how to levitate, Phoebe's hair switches from her shoulder to her back then to her shoulder again.
Size Matters
Continuity: Shortly after Phoebe frees the blonde girl from the clay, she tells her to run "before Gammill sees [them]." But Phoebe hadn't learned the demon's name at any point. Leo and Piper do find out his name, but not until later.
Continuity: Piper suggests that Paige orb in place, because the clay is still wet enough that it will collapse if she's not there to support it. But when Paige actually does orb, the clay cracks and collapses into powder as if it had been bone dry.
A Knight to Remember
Plot hole: Piper is initially skeptical at Phoebe's theory that the Evil Enchantress could be Paige in a past life, because Paige lacks the Enchantress' ability to summon the elements. However, Piper knows that in Phoebe's past life she had a pyrokinetic power, which she does not have in the present.
Plot hole: When Phoebe tried to kick the Shocker Demon, she became electrocuted until Piper interposed with a large wooden stand. But when the prince attacked the Shocker Demon with his metallic sword, the demon was cowed and the prince was unharmed.
Revealing: When the prince is supposed to be frozen on Paige's living room couch, when they do a close-up you can see the actor blink.
Plot hole: According to Paige's "fairy tale" the witch ends up with the prince. If this is really a past life, the sorceress ended up with the prince in the Middle Ages. So why do they have to fix it? Everything was just going the way it did according to her fairy tale, and good magic was obviously still present in the world.
Continuity: When Phoebe picks up the cordless phone to call Paige before the Shocker Demon attacks for the first time, the cordless phone is white but then after the attack, when the cordless is on the floor, it's black.
Brain Drain
Other: Right after Piper comes out of her crazytown adventure, she comes down the stairs in completly different clothes and hairdo than she went in with.
