Plot hole: When Leo tells Piper that he believes he found the demon Sargon in the Book of Shadows (which may be the demon Count Roget made a faustian deal with), Piper looks at it and says that they vanquished this demon five years ago. Which would've meant they vanquished the demon with Prue. When in fact the only demon that happened with was Zahn in season 6 episode 6, "Soul Survivor," and they did that with Paige.
Something Wicca This Way Comes - S1-E1
Continuity mistake: At the beginning of the episode they show Piper running up the stairs to her house in the rain. The umbrella she has is black, with a colored ring around the edge. When she enters the house the whole umbrella has changed to colors surrounding the whole umbrella now. (00:02:35)
Continuity mistake: When the banshee came to the attic, she smashed all of the windows, except for some in the middle of the big window, but in the next shot that glass was gone.
Gone With The Witches - S8-E20
Continuity mistake: When the leprechauns are called back to the manor from Magic School, O'Brien cradles his injured arm in his left hand. In the next shot, his arm is down by his side and he moves it to his arm again. (00:25:10)
Revealing mistake: In Chris's flashbacks, it's supposed to be the day he left to come to the future. But his hair is longer than it was in the season 5 finale when he came to the future.
Other mistake: When Piper and Mark are in the police parking lot she holds the door open for him to get into her SUV. When he gets in you can see the SUV go down from his weight in the vehicle. Ghosts are supposed to be weightless.
Audio problem: When Phoebe hits Cole over the head in the courtroom, the sound of something unfreezing is heard, but Cole wasn't frozen and Piper doesn't unfreeze the room until a few moments later.
Continuity mistake: When Prue leaves Buckland's, she's wearing a red tank top, but when she arrives at Camp Skylark, her shirt is different, both in style and color. She can't have stopped to change, because it's mentioned she left the auction house and headed directly to the camp. (00:07:00 - 00:09:20)
Continuity mistake: In the scene where Cole has caught Phoebe's karate kick at school, Phoebe's hands are both clutching her school books. In the following shot, her left arm is in a completely different position, and there's no reason for her to have moved it. (00:18:25)
Continuity mistake: When Candor interrupts the Noxon demons torturing a student at Magic School, Rondok turns around to look at him. In the following shot, he's facing the other way and turns around again. (00:06:10)
Sight Unseen - S3-E5
Factual error: In the scene where the invisible demon is entering Cole's office, the courthouse shown before the elevator is a New York City courthouse, not San Fransisco. It's confirmed by the presence of a NYC Taxi.
Plot hole: When Piper goes back in time to save her sisters from dying and get rid of the Hollow, right after Leo describes it as "time catching up with itself" and the new reality Piper takes over her body. Yet in Season 5 Episode 7 "A Witch in Time", Piper goes back to keep Phoebe from saving her boyfriend but once Piper changes the past, the original Piper fades away and the new reality Piper stays.
Thank You for Not Morphing - S1-E3
Continuity mistake: When Prue tells Victor to leave the auction house, Victor has his arms behind his back, but in the next shot, his arms are in front of him. (00:09:40)
Continuity mistake: When Paige and the reporters interrupt Piper and Leo's argument, Piper's arms move from out in front of her to her sides. (00:32:55)
Continuity mistake: When Paige goes to talk to Henry at the police station, her arms are on her hips in the shot from the front, but down by her sides in the shot from the back. (00:14:20)
Continuity mistake: When Paige arrives back at the manor after being stood up by Henry, Piper has both her arms on the table in front of her. Her arms are then crossed over her chest, then back on the table. (00:16:30)
Plot hole: When the Avatars agree to rewind time they say that they can either put the world right or save Brody but not both. This doesn't make sense since they're rewinding to right when they put everyone to sleep, which is before Brody was killed.
Other mistake: When Phoebe gets the premonition to see all the loss, she sees the scene where Piper dies in All Hell Breaks Loose. Since time was rewound this event never happened in this timeline, and we're never given any reason to believe Phoebe would know this happened after the time reversal.
Chosen answer: Her agent/manager negotiated a special billing as part of her contract. It distinguishes her from the rest of the cast, probably when she became a producer on the show.
